Cisapride or cimetidine in the treatment of functional dyspepsia. Results of a double-blind, randomized, Swiss multicentre study.

BACKGROUND: Functional dyspepsia is a major diagnostic and therapeutic challenge for the clinician. Several systems for the identification of 'high-risk' patients and classifications of dyspepsia subtypes and treatment schemes have been proposed in the past with limited experimental evidence to support the claims made. The present trial was designed to compare two different treatment modalities in a group of functional dyspepsia patients selected on the basis of a standardized diagnostic procedure as 'non-risk' for organic disease and to assess the result in the major symptom sub-groups of fun

Extracorporeal life support for cardiac assist in pediatric patients. Review of ELSO Registry data.

The collected data on extracorporeal membrane oxygenation (ECMO), now referred to as extracorporeal life support (ECLS), for pediatric cardiac support has not been analyzed. The purpose of this study was to review the Extracorporeal Life Support (ELSO) Registry data to evaluate the results, identify possible predictors of outcome, and attempt to establish criteria. From 1981 to June of 1990, 189 patients were placed on ECLS for cardiac assist. The age range was 0-204 months (median, 7 months). Mean time on ECLS was 115 +/- 75 hours. Fourteen patients were placed on ECLS as a bridge to transpla

Lactate-to-Albumin Ratio for Predicting Intensive Care Unit Admission in Patients with Diabetic Foot Infection.

BackgroundEarly identification of diabetic foot infection (DFI) patients at risk for clinical deterioration is critical for timely intervention. Serum lactate reflects tissue hypoperfusion, whereas hypoalbuminemia indicates systemic inflammation and poor nutritional status. The aim of this study is to evaluate the ability of the lactate-to-albumin ratio (LAR) to predict ICU admission in patients with DFI.MethodsThis retrospective study was conducted in the emergency department of a tertiary care center between 01.01.2022 and 01.01.2025. Adult patients (≥18 years) with confirmed DFI were includ

[A Histopathologically Diagnosed Neurocysticercosis Case from Türkiye].

Cysticercosis is a parasitic tissue infection caused by larval cysts of Taenia solium. Although the disease affects many tissues, it primarily affects the brain and muscles. The most common form is neurocysticercosis, a term used for human central nervous system involvement with T.solium cysts. Neurocysticercosis is an important public health problem in many parts of the world. Its prevalence is particularly high in places where T.solium tapeworms are common, such as Mexico, Central America, South America, Southeast Asia, Africa, China, India, and Nepal. Its incidence has been increasing rapid

Using Routine Data Sources to Feed an Immunization Information System for High-Risk Patients-A Pilot Study.

BACKGROUND: Vaccine-preventable diseases among high-risk patients are a public health priority in high-income countries. Most national immunization programs have included vaccination recommendations for these population groups but they remain hard-to-reach and coverage data are poorly available. In a pilot study, we developed and tested an automated approach for identifying individuals with underlying medical conditions to feed an immunization information system (IIS). METHODS: We reviewed published recommendations on medical conditions that indicate vaccination against influenza, pneumococca

The diagnosis of a conductive olfactory loss.

OBJECTIVES/HYPOTHESIS: Two of the most common causes of olfactory loss include upper respiratory infection (URI) and nasal or sinus disease. The etiology of most URI-related losses is thought to be viral and, as yet, there is no available treatment. In contrast, nasal or sinus disease produces an obstructive or conductive loss that often responds dramatically to appropriate therapy. Therefore, the distinction is important but in many cases may be difficult because such patients often present with no other nasal symptoms, and routine physical findings may be nonspecific. The purpose of this rep

Rapid, high performance method for the determination of vitamin K(1), menaquinone-4 and vitamin K(1) 2,3-epoxide in human serum and plasma using liquid chromatography-hybrid quadrupole linear ion trap mass spectrometry.

Unlike the other fat-soluble vitamins, vitamin K circulates in the human bloodstream at very low levels because of a low intake in the diet. Mammals have developed an efficient recycling system, known as vitamin K-epoxide cycle, which involve quinone, hydroquinone and epoxide forms of the vitamin. Phylloquinone (K(1)) is the main homologue, while menaquinone-4 (MK-4) is both a member of the vitamin K(2) family and metabolite of K(1) in extra-hepatic tissues. Notwithstanding the recent advances, many aspects of the complex vitamin K physiology still remain to be investigated. Therefore, there i

Early Acoustic Voice Changes After Thyroidectomy Without Laryngeal Nerve Injury.

OBJECTIVES: The incidence of thyroid cancer has been increasing annually, and surgery remains the primary therapeutic approach. Clinically, postoperative voice disorders may occur in patients undergoing thyroidectomy, even in the absence of intraoperative injury to the laryngeal nerves. This retrospective study aimed to objectively evaluate postoperative acoustic voice alterations after thyroidectomy using Praat software in patients with confirmed preservation of recurrent laryngeal nerve (RLN) and superior laryngeal nerve (SLN) function. The study focused on changes in acoustic parameters wit

Development and validation of interpretable machine learning models to predict intensive care unit outcomes in patients on hemodialysis: a multicenter study.

BACKGROUND: Hemodialysis patients are at high risk for ICU admission due to elevated mortality, cardiovascular disease, and infection rates. Traditional ICU scoring systems (e.g., APACHE-II, SOFA) demonstrate limited accuracy in this population. This study aimed to identify key risk factors and develop interpretable machine learning (ML) models for predicting ICU outcomes to enable early intervention. METHODS: This multicenter study analyzed data from three cohorts: The First Affiliated Hospital of Sun Yat-sen University (n = 248), MIMIC-IV (n = 769), and eICU-CRD (n = 1,878). Primary outcome

Chronic Obstructive Pulmonary Disease and Obstructive Sleep Apnea Coexistence: From Pathophysiology to Clinical Complications.

BACKGROUND: Obstructive sleep apnea (OSA) and chronic obstructive pulmonary disease (COPD) are two common chronic respiratory diseases. Their co-existence forms overlap syndrome (OVS), a distinct clinical entity that imposes a dual burden on patients due to its unique pathophysiological mechanisms and significant clinical implications. PURPOSE: This narrative review describes the consequences of OVS by tracing its pathophysiological from molecular mechanisms to clinical phenotypes, including non-cardiovascular complications. The primary aim is to synthesize this knowledge into a conceptual fr

A non-invasive AI-based system for precise grading of anosmia in COVID-19 using neuroimaging.

COVID-19 (Coronavirus), an acute respiratory disorder, is caused by SARS-CoV-2 (coronavirus severe acute respiratory syndrome). The high prevalence of COVID-19 infection has drawn attention to a frequent illness symptom: olfactory and gustatory dysfunction. The primary purpose of this manuscript is to create a Computer-Assisted Diagnostic (CAD) system to determine whether a COVID-19 patient has normal, mild, or severe anosmia. To achieve this goal, we used fluid-attenuated inversion recovery (FLAIR) Magnetic Resonance Imaging (FLAIR-MRI) and Diffusion Tensor Imaging (DTI) to extract the appear

A systematic review of the research progress of non-coding RNA in neuroinflammation and immune regulation in cerebral infarction/ischemia-reperfusion injury.

Cerebral infarction/ischemia-reperfusion injury is currently the disease with the highest mortality and disability rate of cardiovascular disease. Current studies have shown that nerve cells die of ischemia several hours after ischemic stroke, which activates the innate immune response in the brain, promotes the production of neurotoxic substances such as inflammatory cytokines, chemokines, reactive oxygen species and - nitrogen oxide, and mediates the destruction of blood-brain barrier and the occurrence of a series of inflammatory cascade reactions. Meanwhile, the expression of adhesion mole

A Machine Learning Tool Using Digital Microscopy (Morphogo) for the Identification of Abnormal Lymphocytes in the Bone Marrow.

Morphological analysis of the bone marrow is an essential step in the diagnosis of hematological disease. The conventional analysis of bone marrow smears is performed under a manual microscope, which is labor-intensive and subject to interobserver variability. The morphological differential diagnosis of abnormal lymphocytes from normal lymphocytes is still challenging. The digital pathology methods integrated with advances in machine learning enable new diagnostic features/algorithms from digital bone marrow cell images in order to optimize classification, thus providing a robust and faster sc

A rare case of multiple phosphaturic mesenchymal tumors along a tendon sheath inducing osteomalacia.

BACKGROUND: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome characterized by renal phosphate wasting, hypophosphatemia, reduction of 1,25-dihydroxyl vitamin D, and bone calcification disorders. Tumors associated with TIO are typically phosphaturic mesenchymal tumors that are bone and soft tissue origin and often present as a solitary tumor. The high production of fibroblast growth factor 23 (FGF23) by the tumor is believed to be the causative factor responsible for the impaired renal tubular phosphate reabsorption, hypophosphatemia and osteomalacia. Complete removal of the t

Multicenter Standardized Mortality Review for Quality-of-Care Improvement: Implementing an Automated Workflow.

BACKGROUND: Mortality review is a key mechanism for identifying preventable harm and improving hospital-based quality and safety. Traditional review processes are manual, often delayed, and inconsistent across services, resulting in variable completion rates and limited ability to generate improvement strategies. Structured, multidisciplinary review models have shown benefits, but many institutions continue to rely on decentralized, retrospective approaches that lack electronic integration or automation. Traditional approaches often fail to identify systemwide patterns, yielding insights with

Early diagnostic strategy for central nervous system bacterial infections after neurosurgery: A retrospective study.

Accurate diagnosis of post-neurosurgical bacterial infection of central nervous system is challenging due to the nonspecific nature of clinical signs and cerebrospinal fluid (CSF) parameters, which often overlap with sterile postoperative inflammation. This study aimed to develop and validate a stepwise diagnostic strategy integrating readily available clinical and basic laboratory indicators to improve the early identification of post-neurosurgical bacterial infection of the central nervous system. A retrospective cohort study was conducted at Tianjin Huanhu Hospital, a tertiary neurosurgical

Laser desorption rapid evaporative ionization mass spectrometry (LD-REIMS) demonstrates a direct impact of hypochlorous acid stress on PQS-mediated quorum sensing in Pseudomonas aeruginosa.

UNLABELLED: To establish infections in human hosts, Pseudomonas aeruginosa must overcome innate immune-generated oxidative stress, such as the hypochlorous acid (HOCl) produced by neutrophils. We set out to find specific biomarkers of oxidative stress through the development of a protocol for the metabolic profiling of P. aeruginosa cultures grown in the presence of different oxidants using a novel ionization technique for mass spectrometry, laser desorption rapid evaporative ionization mass spectrometry (LD-REIMS). We demonstrated the ability of LD-REIMS to classify samples as untreated or tr
