Secondary prevention of recurrent ischemic stroke: from guidelines to precision medicine.

Recurrent ischemic stroke remains a major global health challenge, accounting for substantial disability and mortality despite advances in acute management. Secondary prevention has undergone a fundamental paradigm shift-from standardized, population-based regimens toward individualized, mechanism-targeted precision medicine. This review integrates evidence from recent AHA/ASA and ESO guidelines[2,3] and landmark clinical trials including NAVIGATE ESUS[5], RE-SPECT ESUS[6], CHANCE-2[2,13], INSPIRES[18], and COMPASS[24], highlighting how etiological diagnosis and multimodal risk stratification

Association of bipolar disorder and schizophrenia in women with risk of gestational diabetes: a systematic review and meta-analysis.

BACKGROUND: Psychiatric disorders are severe mental illnesses associated with insulin resistance, systemic inflammation, and neurohormonal dysfunction. To synthesize the available evidence and provide recommendations for the clinical management of this patient population, this study aimed to comprehensively examine and meta-analyze the association between bipolar disorder, schizophrenia, and the risk of gestational diabetes mellitus (GDM). METHOD: A systematic search was conducted in PubMed, Embase, Scopus, Web of Science, and Google Scholar. Studies reporting an association between pre-pregn

Peripheral artery disease.

Peripheral artery disease (PAD) is characterized by blockage of the arteries that supply the lower extremities, often occurring as a result of atherosclerosis and thrombosis. PAD affects approximately 230 million people worldwide, with a growing prevalence owing to population ageing and concomitant cardiovascular risk factors, including smoking, diabetes mellitus, hypertension and dyslipidaemia. Patients with PAD have an increased risk of major cardiovascular and limb events, and substantially poorer walking performance compared with those without PAD. The screening and identification of PAD i

Improving the patient care pathway and multidisciplinary management are major challenges in Hidradenitis suppurativa treatment.

Hidradenitis suppurativa (HS) is a chronic inflammatory disease resulting from the close interplay between genetic predisposition, dysregulation of the mechanisms involved in autoinflammatory pathways, dysfunctional response to bacterial biofilm changes, and environmental factors that potentially exacerbate the disease. Its cutaneous manifestations may represent just the tip of the iceberg, as numerous comorbidities may be associated with the disease. The risk of occurrence of inflammatory bowel disease is three times higher than that in the general population. At least 5% of patients also hav

Nutritional therapy in human immunodeficiency virus (HIV) infection.

Malnutrition remains highly prevalent among individuals living with human immunodeficiency virus (HIV), and it continues to represent a critical determinant of health outcomes. The double burden of malnutrition-comprising both undernutrition and overweight/obesity-has significant clinical implications, as it not only affects disease progression but also complicates the management of HIV and its associated conditions. Undernutrition can exacerbate immune suppression and increase susceptibility to opportunistic infections, while overweight and obesity contribute to the rising burden of metabolic

Update on Treating Painful Diabetic Peripheral Neuropathy: A Review of Current US Guidelines with a Focus on the Most Recently Approved Management Options.

Painful diabetic peripheral neuropathy (DPN) is a highly prevalent and disabling complication of diabetes that is often misdiagnosed and undertreated. The management of painful DPN involves treating its underlying cause via lifestyle modifications and intensive glucose control, targeting its pathogenesis, and providing symptomatic pain relief, thereby improving patient function and health-related quality of life. Four pharmacologic options are currently approved by the US Food and Drug Administration (FDA) to treat painful DPN. These include three oral medications (duloxetine, pregabalin, and

From the kitchen to the medicine cabinet: Examples of functional herbs and spices.

In many cultures, "food as medicine" has existed for centuries as the foundation of health. It is a practice built on the knowledge that food and diet play important roles in disease prevention and management. Foods possessing therapeutic properties are often referred to as functional foods. Many herbs and spices contain numerous nutritional and non-nutritional components that can interact with pharmacologically relevant receptors, either directly or indirectly via their metabolites, to regulate cellular biochemical processes. Although opinions are changing, the concept of food as a therapeuti

Atropine: Updates on myopia pharmacotherapy.

The prevalence of myopia has rapidly increased over the last 30 years, with the World Health Organization estimating a worldwide incidence of 23%, projected to increase to 50% by 2050. The myopia epidemic has prompted a reincarnation in efforts to overcome this challenge. The exploration of atropine use in myopia was a result due to a lack of treatment in effect. This study aimed at reviewing the role of atropine in the management of myopia worldwide based on currently available findings. A literature search was conducted using PubMed/MEDLINE and Google Scholar for studies published up to Apri

Current opinion of presentation of dysphagia and dysphonia in patients with coronavirus disease 2019.

PURPOSE OF REVIEW: Dysphagia and dysphonia are common presentations of both acute and long coronavirus disease 2019 (COVID-19). The majority of peer-reviewed publications in 2020 and early 2021 were expert guidance and consensus statements to support dysphagia management in multidisciplinary teams while protecting clinicians and patients from the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus. This review discusses dysphagia and dysphonia primary data published in 2021-2022, focusing on patient presentations, pathophysiology, and evidence for interventions. RECENT FINDINGS

Strengthening Equity and Inclusion in Urban Greenspace: Interrogating the Moral Management & Policing of 2SLGBTQ+ Communities in Toronto Parks.

There is growing recognition that greenspace provides invaluable benefits to health and wellbeing, and is essential infrastructure for promoting both social and environmental sustainability in urban settings. This paper contributes towards efforts to build 'just' and equitable urban sustainability, and more specifically greenspace management, by drawing attention to hostility and exclusion experienced by two-spirit, lesbian, gay, bisexual, transgender, queer, genderqueer, pansexual, transsexual, intersex and gender-variant (2SLGBTQ+) park occupants. There is evidence that access to greenspace

Where to start? Upfront therapy for follicular lymphoma in 2018.

The initial approach to the management of follicular lymphoma (FL) is challenging for patients and physicians. Most FL patients present with minimal symptoms; given the lack of a survival benefit to early treatment in this population, a period of observation without therapy is often appropriate. Once there is disease progression beyond low-tumor-burden criteria or symptoms prompting intervention, patients may be considered for an array of potential treatment options. These range from single-agent rituximab (anti-CD20) to various forms of chemoimmunotherapy, including rituximab or the newer ant

Acute-on-Chronic Liver Failure: Getting Ready for Prime Time?

UNLABELLED: Acute on chronic liver failure (ACLF) is the culmination of chronic liver disease and extrahepatic organ failures, which is associated with a high short-term mortality and immense health care expenditure. There are varying definitions for organ failures and ACLF in Europe, North America, and Asia. These differing definitions need to be reconciled to enhance progress in the field. The pathogenesis of ACLF is multifactorial and related to interactions between the immunoinflammatory system, microbiota, and the various precipitating factors. Individual organ failures related to the kid

Cefotaxime-resistant E. coli in dairy and beef cattle farms-Joint analyses of two cross-sectional investigations in Germany.

Resistance to third-generation cephalosporins and other beta-lactam antibiotics is of major concern for animal and human health. Knowledge of the prevalence of resistant bacteria in primary production is an important element to estimate transmission along the stages in the food production chain and the exposure of the human population. The primary objective of this study was to determine the prevalence of cefotaxime-resistant commensal E. coli in dairy and beef cattle production units throughout Germany. Secondarily, the association between management factors and the presence of cefotaxime res

[Management of pleural drainage].

In the neonatal population, pleural effusion and particularly tension pneumothorax can be a deadly situation. Pneumothorax occurs more often in the neonatal period that any other time of life. Tension pneumothorax can result in very high pressures within the pleural space, collapsing the lung on the involved side and resulting in immediate hypoxia, hypercapnia and subsequent circulatory collapse. For these reasons, the ability to recognize, understand and treat these pathologies is essential for neonatal health and a good outcome. Neonates have many factors that can contribute to. these proble
