We report the case of a 47-year-old man who presented with several episodes of left precordial pain, one of which had been severe, but was unrelated to exertion or posture. Transthoracic echocardiography and cardiovascular magnetic resonance showed evidence of congenital partial absence of the left pericardium and severe tricuspid regurgitation. Both diagnoses were confirmed at surgery when the pericardial defect was repaired and the tricuspid valve was replaced at the same operation. He went on to make a good recovery.

OBJECTIVE: To investigate the risk factors of chronic prostatitis.
METHODS: An epidemiological survey of 1,168 patients with chronic prostatitis was conducted and both single factor and multifactorial logistic regression analyses were performed.

Blood pressure homeostasis is particularly challenged by moving to an upright position. The autonomic nervous system, structural factors, infravascular volume, and circulating and local hormones influence the body's response to a change in posture. Disorders of the autonomic nervous system can result in orthostatic hypotension. Both neurogenic and nonneurogenic disorders can contribute, and distinguishing between the two causes is important for both diagnosis and management. It is also important to recognize various factors in daily life that influence orthostatic hypotension.

Low tongue posture may impede the lateral expansion of the maxilla resulting in a constricted maxillary arch with resultant posterior crossbites and functional shifts. Excessive molar eruption may also occur, resulting in anterior open bites and increases in the anterior lower vertical height.

The successful use of electroconvulsive therapy (ECT) for a patient in catatonic stupor with joint contracture is reported. A 53-year-old woman, who had been diagnosed with schizophrenia, developed a catatonic stupor unresponsive to neuroleptics. Physical emaciation gradually developed under food refusal and poor nutrition. Joint contracture of her extremities resulted from the long period of being bedridden in the same posture. In this case, modified ECT was dramatically effective for catatonic stupor without any complication despite the joint contracture.

Twelve infants with severe unilateral or bilateral congenital ptosis were surgically repaired before 1 year of age in an attempt to achieve early functional and cosmetic improvement. In all cases, a frontalis suspension using polyfilament nylon suture (Supramid Extra) was performed. Postoperatively, 10 patients achieved near normal eyelid position and all had resolution of their chin-up head posture. Two slings eventually failed, requiring repeat procedures for recurrent ptosis. Two developed signs of cellulitis that were successfully treated with oral antibiotics.

A patient with glioblastoma of the rostral brainstem and hypothalamus exhibited bilateral internuclear ophthalmoplegia and vertical nystagmus; he suffered episodes of cataplexy, narcolepsy, and sleep paralysis. A peculiar fluctuation of posture and tone ("limp man syndrome") proved to be a manifestation of continuous cataplexy, as documented by H-reflex recordings. This is the first report of a remarkable movement disorder caused by continuous, fluctuating, partial cataplexy, and is the second report of an association between cataplexy and a tumor of the rostral brainstem.
