Surgical Management of Deep Gluteal Syndrome Causing Sciatic Nerve Entrapment: A Systematic Review.

PURPOSE: To assess the causes, surgical indications, patient-reported clinical outcomes, and complications in patients with deep gluteal syndrome causing sciatic nerve entrapment. METHODS: Three databases (PubMed, Ovid [MEDLINE], and Embase) were searched by 2 reviewers independently from database inception until September 7, 2016. The inclusion criteria were studies reporting on both arthroscopic and open surgery and those with Level I to IV evidence. Systematic reviews, conference abstracts, book chapters, and technical reports with no outcome data were excluded. The methodologic quality of

Return to Sports and Physical Activities After the Modified Lapidus Procedure for Hallux Valgus in Young Patients.

BACKGROUND: The modified Lapidus procedure has successfully relieved pain and corrected deformity in patients with hallux valgus, but its effect on participation in specific sports and physical activities remains unclear. Our goals were to assess sports and physical activities in young patients following the modified Lapidus procedure and to compare these with clinical outcomes. METHODS: Fifty-eight eligible patients were identified from a retrospective registry review. Of these, 48 (83%) were reached for follow-up at a mean of 2.8 (range, 1.0 to 6.1) years and had a mean age at surgery of 37

Introduction to interventional radiology's role in palliative care for children with cancer: A COG Diagnostic Imaging Committee/SPR Oncology Committee White Paper.

As palliative care continues to be an area of increasing emphasis in pediatric oncology programs, it is important to raise awareness about minimally invasive, image-guided procedures that can supplement more conventional palliative interventions, such as systemic analgesics, external beam radiation, and locoregional anesthesia. These procedures, when performed for appropriately selected patients, can often facilitate discharge from an inpatient facility, and help patients meet their end-of-life goals. This article specifically discusses three palliative procedures performed by interventional r

Application of Intralesion Ultrasound-Guided Laser Ablation for Plantar Foot Mass Involving Arteriovenous Fistula: A Case Report.

Penetrating and blunt trauma to the plantar aspect of the foot are common. Both penetrating and blunt trauma may be associated with an injury to the blood vessels, which may go undetected clinically. A minor puncture, blunt trauma and repeated blunt trauma can injure small arteries leading to an arteriovenous fistula (AVF). The arterial rupture leads to a system of veins creating a small AVF and appearing clinically as a blue colored, painful lesion. A case of such an injury is presented where the clinical examination, magnetic resonance arteriogram and point of service ultrasound with duplex

The development of Fournier's gangrene following rubber band ligation of haemorrhoids.

The development of Fournier's gangrene in an 80-year-old male patient with diabetes after a routine outpatient haemorrhoid banding procedure is described. Four days following the procedure, the patient noticed an increasing amount of pain and swelling of the perianal region. When the patient presented to the emergency department 18 days later, immediate radical debridement of ischiorectal necrotic tissue was performed. A defunctioning loop sigmoid colostomy was also formed. Subsequent operations required excision of the scrotum and abdominoperineal excision of the rectum. Histology studies lat

New developments in hysteroscopy.

Diagnostic and operative hysteroscopy have become standards in gynaecologic practice. Many hysteroscopic procedures have replaced old, invasive techniques, such as dilatation and curettage. As instruments have reduced in size, office hysteroscopy has begun to replace operating-room procedures. New instruments and techniques continue to emerge, and the prospects for improvement seem unlimited. Discussed are hysteroscopic sterilisation, bipolar electrosurgery, hysteroscopic morcellation, and virtual hysteroscopy. Furthermore, the evidence for supporting the vaginoscopic approach, cervical prepar

Gingival augmentation with an acellular dermal matrix: human histologic evaluation of a case--placement of the graft on bone.

The importance of gingival augmentation to increase the amount of keratinized tissue is a controversial subject. In this case report, an acellular dermal matrix was used to obtain an increase in the amount of keratinized tissue around four implants. In this case, the acellular dermal matrix was placed on bone. The surgical procedure resulted in an increase in the amount of keratinized tissue. Therefore, it met the clinical goals of the surgical procedure. However, the clinical findings and patient pain levels during the healing seemed to resemble a denudation procedure. Additionally, the histo

Laparoscopic retroperitoneal needle suspension urethropexy.

Recent advances in laparoscopic surgery have allowed urologists to perform laparoscopic bladder suspension; however, laparoscopic suturing techniques have a steep learning curve and make the procedure more difficult and time consuming. We describe a new technique that combines extraperitoneal laparoscopic bladder neck mobilization through a single port with the ease of a needle suspension urethropexy in order to avoid the need for vaginal incisions and the use of laparoscopic suturing techniques. The procedure has been performed in four patients, all of whom are continent and voiding with comp

Use of endoluminal illuminated bougie during laparoscopic and thoracoscopic surgery of the esophagus.

Newly developed endoscopic instruments and devices, along with advanced endoscopic surgical techniques, have made it possible to perform an increasing variety of endoscopic procedures. These procedures consist of the same steps employed in the open procedure that preceded them but avoid their large incisions. This in turn eliminates many of the disadvantages associated with laparotomy and thoracotomy incisions. Postoperative pain is greatly decreased, postoperative hospital stay is shortened, and return to normal activities is expedited. Postoperative complications as well as early and late mo

Simple in-office sphincterotomy with partial fissurectomy for chronic anal fissure.

Chronic anal fissure is an extremely common problem of the anorectum. It is often confused with symptomatic hemorrhoids and is frequently missed by the examining physician. Symptoms are most commonly pain and bleeding after defecation. A simple office procedure involving partial lateral internal sphincterotomy with or without excision of a sentinel tag has been used over the past five years with extremely satisfactory results. This procedure was used upon 86 patients. Anatomic and symptomatic relief was obtained in 96.4 per cent of the patients. Due to diagnostic related groups and attempts at

Management of Urosymphyseal Fistula and Pelvic Osteomyelitis: A Comprehensive Institutional Experience and Improvements in Pain Control.

BACKGROUND: Urosymphyseal fistula (USF) is an uncommon but painful and devastating condition typically occurring after radiotherapy for pelvic malignancy, with a delayed diagnosis and a risk of opioid dependence. OBJECTIVE: To characterize our institutional experience and determine the impact of intervention for USF on pain control and opioid use. DESIGN, SETTING, AND PARTICIPANTS: We evaluated data for 33 adult patients diagnosed with USF and managed at a quaternary institution between 2009 and 2021. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: We assessed chronic opioid use, pain scores

Lumbar interlaminar epidural injections in central spinal stenosis: preliminary results of a randomized, double-blind, active control trial.

BACKGROUND: Chronic, persistent low back and lower extremity pain is often caused by spinal stenosis. Surgery and other interventions, including epidural injections, have been used to relieve this pain. However, there is little in the medical literature to support interlaminar, or transforaminal epidural injections under fluoroscopy for managing lumbar pain of central spinal stenosis, while the caudal epidural approach has been studied. STUDY DESIGN: A randomized, double-blind, active, control trial. SETTING: A private, interventional pain management practice, specialty referral center in th

Coronary artery embolism in a patient with Fontan palliation: a rare complication.

Coronary artery embolism is extremely rare in children with Fontan palliation. We report a 10-year-old boy with hypoplastic left heart syndrome post-Fontan procedure who presented with acute onset chest pain and ST-segment changes on electrocardiogram. Cardiac catheterisation revealed coronary artery emboli requiring stent placement. Our case highlights the importance of evaluating chest pain in a systematic manner in Fontan patients as acute coronary artery embolism can be a part of the spectrum of well-known thromboembolic complications.

Interventional Management for Pelvic Pain.

Interventional procedures can be applied for diagnostic evaluation and treatment of the patient with pelvic pain, often once more conservative measures have failed to provide relief. This article reviews interventional management strategies for pelvic pain. We review superior and inferior hypogastric plexus blocks, ganglion impar blocks, transversus abdominis plane blocks, ilioinguinal, iliohypogastric and genitofemoral blocks, pudendal nerve blocks, and selective nerve root blocks. Additionally, we discuss trigger point injections, sacroiliac joint injections, and neuromodulation approaches.

[Perforated duodenal ulcer in a Roux-en-Y gastric bypass operated patient can be a diagnostic challenge].

Roux-en-Y gastric bypass (RYGBP) is an increasingly used procedure when treating morbid obesity. Due to the extensive gastrointestinal rearrangement, diagnostic evaluation of patients with gastric bypass and acute abdominal pain can be difficult. We present a case of a perforated duodenal ulcer in a RYGBP operated patient, where free abdominal fluid, but hardly any pneumoperitoneum was seen on a computed tomography. Free intraperitoneal fluid is an important finding and should give suspicion of the need for emergency surgery in RYGBP operated patients with abdominal pain.

[Treatment of specific complications of locally advanced prostate cancer].

Despite improved screening methods and earlier detection of prostate carcinoma, the complications of locally advanced disease remain a challenge for urologists. The spectrum of possible complications includes subvesical obstruction, hydronephrosis, hematuria, pain, and local invasion of adjacent organs. Urinary diversion, palliative transurethral resection of the prostate, and endoscopic diathermy are classic procedures. Urethral stenting, laser evaporation, and selective arterial embolization are alternate approaches for treating these complex cases. Cystoprostatectomy and pelvic exenteration

Prolonged abdominal wall pain caused by transfascial sutures used in the laparoscopic repair of incisional hernia.

The use of transfascial sutures in the laparoscopic repair of incisional hernia has considerably facilitated the accurate intraperitoneal placement and fixation of the synthetic mesh. The laparoscopic procedure has a number of advantages, including less morbidity and pain. Moreover, the use of a mesh prosthesis results in a low rate of hernia recurrence. Despite the benefits associated with this technique, several (minor) complications have been documented. Herein we describe a case of prolonged abdominal wall pain after laparoscopic hernia mesh repair that was caused by the use of transfascia

[Epiduroscopy in patients with chronic low back pain without remarkable findings on magnetic resonance imaging].

Two patients with chronic low back pain and sciatica failed to respond to conservative treatments. In these patients, magnetic resonance imaging (MRI) showed no remarkable findings corresponding to their symptoms. We treated these patients using epiduroscopy. Epiduroscopic visualization of the spinal canal permits efficient adhesiolysis and irrigation. One patient got better after two epiduroscopic procedures, and the other did not. Epiduroscopy may be an effective, minimally invasive treatment as well as examination for patients with chronic low back pain without remarkable findings on MRI.

[Direct screwing in spondylolysis: results and observations on indications].

Ten patients suffering from persisting low-back pain underwent direct repair of spondylolysis L5 with hook screws. At a follow-up three to four years after the operation, four patients showed good and six unsatisfactory results. Four patients had a secondary procedure. Average age was relatively high in the study group compared with the literature, and pain tended to be chronic. In five of the six patients with a poor clinical result, radiological consolidation of the lytic defect could not be demonstrated. We are discussing indications based on a literature review.
