Peripatum cardiomyopathy is a rare disease, which appears as cardiac failure, at the end of pregnancy and puerperium without an apparent cause. The diagnosis is done clinically based in accepted criteria by Demakis, and it is confirmed by auxiliary tests. Treatment includes rest, digitalic and diuretic medications. Prognosis is bad when there is not symptomatic regression, with a high mortality soon after. The experience at Hospital Central Militar from 1967 to 1995, is presented. There were nine cases, two of which had died; and the presentation of the last case, is done herein.

Experiments were performed in two groups of anaesthetized rats to study the genesis of pulmonary oedema and to determine the pharmacokinetic parameters following a subcutaneous (s.c.) injection of Tityus serrulatus scorpion venom. The burden of pregnancy in patients with severe mitral stenosis continues to present a therapeutic challenge. We present two such cases successfully treated with balloon valvuloplasty using the Inoue balloon. The unique features of this balloon may render it the technique of choice for selected pregnant patients with severe mitral stenosis.

Cardiovascular monitoring in pediatrics was previously limited by inappropriate material. Following M.M. Pollack's publication, bedside cardiovascular catheterization in children have been developed and used for the last 8 years. These techniques allow the survey and the treatment of latent or patent hemodynamical disturbances with objective data. We describe these invasive techniques and their indications in pediatric intensive care.

Is respiratory infection mortality in Down's syndrome (DS) individuals due mainly to their congenital heart disease (CHD) or to other factors which subject most mentally retarded persons to risk? Detailed clinical and autopsy records of 137 institutionalized DS patients and 480 non-DS controls over 31 years yielded 42 DS subjects and 13 non-DS controls with congenital heart disease. We treated a 61-year-old Japanese man with a strut fracture of the new Björk-Shiley mitral valve. This fracture occurred about two months after mitral valve replacement. Emergency reoperation was performed as soon as the chest X-ray showed the fracture, despite the presence of cardiopulmonary shock. He died on the 3rd postoperative day. Similar reported cases of literature are reviewed.

Traditional operations to obstruct the IVC are often unsatisfactory because the morbidity and mortality is appreciable: poor risk patients do not tolerate surgical and anesthetic trauma. Two intravascular oxygen electrodes (of the International Biophysics Corporation and Roche) were tested in vitro and in long-term monitoring of arterial oxygen partial pressure in 19 patients. There were no complications. Both electrodes proved suitable for use for monitoring in an intensive-care situation. But both could still be improved by having greater stability and a shorter response time. This is a report of a patient who developed acid pulmonary aspiration syndrome following pulmonary aspiration of gastric contents at a pH of 3.5. The volume of the stomach contents was large and 15 ml of magnesium trisilicate was insufficient to prevent the effects of acid aspiration. After operation, considerable pulmonary shunting was demonstrable for several days. The patient was discharged home well, and a chest x-ray 2 months later showed no abnormality.
