[Balloon dilatation of valvular pulmonary stenoses in adults].

Balloon valvuloplasty of pulmonary stenosis has become the treatment of choice in children and adults. This is a report about the long term results in adult patients. Forty-six patients (mean age 37 +/- 17 years) with pulmonary stenosis were treated between 1984 and 1994 by this method. Thirty-four of 46 patients were re-examined 3 months to 9 years (mean 3.4 years) later by right heart catheterization and echocardiography. These 34 patients were representative for the whole group concerning age, severity of the pulmonary stenosis, and acute results after the intervention. The pressure gradien

[Anatomic and hemodynamic comparisons in patients with aorta and pulmonary artery arising from the right ventricle].

The anatomy and hemodynamics were compared in 59 patients with aorta and pulmonary artery origin from the right ventricle. The patients' age was from 3 months to 23.5 years. The pathophysiology of the patients' circulation was determined by many factors, the most important factors were the following: the anatomic type of the malformation, the presence or absence of pulmonary and aortic stenoses and other concurrent congenital heart malformations. The oxygen saturation of the arterial blood was associated with the direction of physiological blood flows in the right ventricle. The most "successf

Percutaneous transluminal balloon pulmonary valvoplasty--long-term results.

Thirty-two patients with isolated valvar pulmonary stenosis (21 male; 11 female, age range 4 to 53 years, mean 14 years) underwent cardiac catheterization and balloon valvoplasty. Right ventricular systolic pressure before valvoplasty ranged from 65 to 210 mm Hg (mean 120.2 +/- 44.8 mm Hg). It fell to 24-200 mm Hg (mean 73.1 +/- 42.4 mm Hg) immediately after dilation. Peak systolic gradient across the pulmonary valve before valvoplasty ranged from 42 to 193 mm Hg (mean 98 +/- 45.3 mm Hg) and decreased significantly to 5 to 182 mm Hg (mean 52.7 +/- 43.1 mm Hg) immediately after dilation. At rep…

Updates in interventional cardiology in children with cardiac disease.

UNLABELLED: The field of pediatric and congenital interventional cardiology is advancing rapidly, with recent innovations such as patent ductus arteriosus closure in premature infants, expanded options for transcatheter pulmonary valve replacement, ductal stenting for ductal-dependent lesions, and the development of novel, pediatric-specific devices. Despite these advancements, significant variability in care persists across centers, largely due to the absence of standardized guidelines. This gap can be addressed through the use of registries and multicenter studies, which offer evidence-based…

A case report of late physiologic repair of congenitally corrected transposition of the great arteries and pulmonary stenosis in a severely cyanotic patient: better late than never.

BACKGROUND: Patients with congenitally corrected transposition of great arteries (ccTGA) not infrequently seek medical attention for the first time late in life. Optimal management of natural history ccTGA is debated and must be tailored. CASE SUMMARY: A 38-year-old male patient was referred to our centre because of severe cyanosis and worsening dyspnoea. Investigations disclosed situs solitus, mesocardia, double discordance, large ventricular septal defect (VSD), severe pulmonary stenosis, and no significant atrio-ventricular valves regurgitation. The patient underwent physiologic repair: VS

Infective Endocarditis in a Patient with Celiac Disease after Central Venous Catheter Insertion.

There is an increasing incidence of infective endocarditis secondary to central venous catheters, which is termed as 'healthcare-associated infective endocarditis'. There is an increased risk of getting infective endocarditis in conditions with malnutrition and also if the tip of the central venous catheter is deep in the right atrium close to the tricuspid valve. We present a case of 31-year-old female who had all these risk factors. She was admitted to the hospital for the work up of the weight loss and was diagnosed with celiac disease. Central venous access was obtained because of poor per

Modified Damus-Kaye-Stansel procedure using aortic flap technique for systemic ventricular outflow tract obstruction in functionally univentricular heart.

Damus-Kaye-Stansel procedure is a useful method to relieve the systemic ventricular outflow tract obstruction in functionally univentricular heart. Regurgitation of pulmonary valve and recurrence of systemic ventricular outflow obstruction are the major concerns at the late phase of this procedure. Modification of original Damus-Kaye-Stansel procedure that can prevent the use of prosthetic materials is evaluated. The modified Damus-Kaye-Stansel procedure using aortic flap technique was performed in eight patients with functionally univentricular heart. Patients' ages ranged from 3 to 28 months…

Transarterial approach of the pulmonary artery in anatomically corrected malposition of the great arteries by manipulating a catheter inverted with balloon floating maneuver.

We describe two neonates with anatomically corrected malposition of the great arteries (ACM). A 1-day-old female newborn, who had the salient clinical findings of Goldenhar syndrome, behaved like a patient with the tetralogy of Fallot. The cardiac malformations included ACM, perimembranous inlet and outlet ventricular septal defect with tricuspid valve straddling, conoventricular septal malaligement, bilateral cori without aortic-mitral continuity, pulmonary stenosis at subvalvular, valvular, and supravalvular levels, and bicuspid semilunar valves. After the procedure of left Blalock-Taussig s

Assessment of right-to-left shunt flow in atrial septal defect by transesophageal color and pulsed Doppler echocardiography.

To investigate the clinical significance and problems of right-to-left (R-L) shunt flow dynamics in atrial septal defects, we performed transesophageal color and pulsed Doppler echocardiography in 30 patients with atrial septal defects of the ostium secundum type. The 30 patients consisted of 20 with a pulmonary artery systolic pressure of less than 40 mm Hg, four with a pressure of 40 to 60 mm Hg, three with a pressure of 90 mm Hg or more, two patients with pulmonic stenosis, and one patient with Ebstein's anomaly. R-L shunting was determined by the presence of a shunt flow signal across the

Vanishing De Vega annuloplasty for functional tricuspid regurgitation.

Annuloplasty is performed for significant functional tricuspid regurgitation even if it is presumed that in some cases the regurgitation will regress spontaneously after correction of the left-sided lesion. In an attempt to avoid the drawbacks of a permanent annuloplasty, we used a reabsorbable De Vega annuloplasty in a selected group of patients. Of 73 patients with functional tricuspid regurgitation operated on between May 1989 and May 1991, 25 with pulmonary arteriolar resistance below 400 dyne.sec.cm-5 underwent a De Vega annuloplasty with 2-0 polydioxanone suture. Left main coronary artery compression by the right pulmonary artery in a patient with congenital pulmonic stenosis.

Congenital pulmonary stenosis (PS) can be associated with pulmonary artery (PA) dilatation. In some cases, this can cause compression of nearby structures including the left main coronary artery (LMCA). This compression causes angina and is considered an indication for surgical treatment. We present the case of a patient with PS and angina secondary to LMCA compression by the right PA and review the main indications and options for surgical treatment.

Unilateral pulmonary artery stenosis and late-onset cataract in an adult: a case of suspected congenital rubella syndrome.

Congenital rubella syndrome is characterized by the triad of deafness, cataract and cardiovascular malformations. The great majority of the cases in the literature have been usually diagnosed in infancy and childhood because of various defects at birth. However, we report a rare case of suspected congenital rubella syndrome in an adult with unilateral pulmonary artery stenosis and late-onset cataract.

Endovascular therapy for radiation-induced pulmonary artery stenosis: a case report and review of the literature.

Although pulmonary artery stenosis and its treatment has been well characterized in pediatric populations, its diagnosis and even how to determine the significance of a stenosis can be difficult and optimal management is currently unknown. This case report and review of the literature outlines how we successfully diagnosed, evaluated, and stented a patient with radiation induced pulmonary artery stenosis. This case further adds support to a very limited data pool suggesting that percutaneous angioplasty of pulmonary artery stenosis may be a safe and effective treatment option for this unique p

[Secondary pulmonary hypertension in the elderly: pathophysiological and clinical aspects from an observed case].

The authors report the case of a 76-year-old woman with severe pulmonary hypertension (80 mmHg). She had been suffering for years from chronic bronchitis and presented tricuspid insufficiency. The case report refers to the association between valvular insufficiency and/or chronic bronchitis and pulmonary hypertension. This condition--if severe--results in progressive disability and death. The treatment of severe pulmonary hypertension has not been taken into account, since it has proven to be ineffective and has side effects. The early detection and the prevention of the underlying causes repr

[Architectonics of pulmonary vessels in pulmonary stenosis].

The authors aimed their work on the study of the architecture of pulmonary blood vessels and blood flow via pulmonary vascular network in patients with Fallot malformation. In 34 children with valvular stenosis of a. pulmonalis and in 12 children with Fallot tetralogy they analysed in detail their angiograms, paying attention to evaluation of the architecture of truncus pulmonalis and its branches, the discharging part of the right ventricle, and the state of the arterial phase of the pulmonary vascular network. On the basis of their results the authors explain the preferential blood distribut
