The Authors' Technique for Volar Plating of Scaphoid Nonunion.

Internal fixation of the scaphoid using a plate has been reported in the literature as far back as 1977. Recently, a specific plate designed for scaphoids has been developed, which provides considerably more buttress support than intramedullary headless screws, and offers a reliable method of rigid internal fixation for complex fractures. Indications to use such a plate include complex acute fractures, such as those with significant waist comminution or steep reverse oblique fractures, and complex nonunions with central bone loss resulting from failed previous headless screw fixation. This is

Effects of concentrated growth factors (CGF) on the quality of the induced membrane in Masquelet's technique - An experimental study in rabbits.

AIMS: The aim of the study was to test if the addition of CGF to the Masquelet technique contributes to the quality of the membrane formed surrounding the polymethylmethacrylate (PMMA), in terms of inflammation, proliferation and vasculazition in the Masquelet technique in the early and late phases in a rabbit model. MATERIALS AND METHODS: A critical bone defect of 15 mm was created in radius diaphysis, leaving 3 cm of intact bone to the joint. To mimic the Masquelet technique and to increase stability, a 6-hole 1.5 mm plate with two screws was applied, although it was initialy stable because

Effects of the Elbow Flexion Angle on the Radial Nerve Location around the Humerus: A Cadaver Study for Safe Installation of a Hinged External Fixator.

BACKGROUND: This study aimed to investigate whether the distance between the radial nerve and rotational center of the elbow joint when observing from the lateral surface of the humerus changes according to passive elbow joint flexion for safe external fixation with a hinged fixator of the elbow joint. METHODS: Twenty fresh-frozen cadaveric arms were dissected. The points where the radial nerve crosses over the posterior aspect of the humerus, crosses through the lateral center, and crosses over the anterior aspect of the humerus were defined in the lateral view of the elbow joint, using fluo

Magnetic resonance imaging investigation of radio-lunate relations: use in assessing distal radial fracture reduction.

UNLABELLED: Reformatted magnetic resonance imaging scans of 27 normal wrists were examined in incremental degrees of rotation around the central axis of the radial shaft to 30° in both directions from true lateral. A line was superimposed on the outer border of the radial dorsal cortex and continued distally to the carpal region. Measurements were made from the superior pole of the lunate to this line. This distance did not change significantly with rotation. This allows a quick and accurate way of assessing distal radial fracture reduction from poorly taken lateral radiographs. In addition, t

Synergistic effects of HBO and PRP improve bone regeneration with autologous bone grafting.

Bone defects remain a challenge for patients and orthopaedic surgeons. Autologous transfer of cancellous bone grafts remains the standard of care. However, in recent years various osteoinductive substitute materials, such as platelet rich plasma (PRP) and hyperbaric oxygen therapy (HBO) have been shown to improve bone healing. This study evaluates the effects of a combined application of PRP and HBO with autologous bone grafting in an animal model. In 48 New Zealand White rabbits bone defects at the radius were filled with autologous bone harvested at the iliac crest. This was combined with ap

Successful human long-term application of in situ bone tissue engineering.

Tissue Engineering (TE) and Regenerative Medicine (RM) have gained much popularity because of the tremendous prospects for the care of patients with tissue and organ defects. To overcome the common problem of donor-site morbidity of standard autologous bone grafts, we successfully combined tissue engineering techniques for the first time with the arteriovenous loop model to generate vascularized large bone grafts. We present two cases of large bone defects after debridement of an osteomyelitis. One of the defects was localized in the radius and one in the tibia. For osseus reconstruction, arte

Positioning evaluation of corrective osteotomy for the malunited radius: 3-D CT versus 2-D radiographs.

The authors retrospectively investigated the postoperative position of the distal radius after a corrective osteotomy using 2-dimensional (2-D) and 3-dimensional (3-D) imaging techniques to determine whether malposition correlates with clinical outcome. Twenty-five patients who underwent a corrective osteotomy were available for follow-up. The residual positioning errors of the distal end were determined retrospectively using standard 2-D radiographs and 3-D computed tomography evaluations based on a scan of both forearms, with the contralateral healthy radius serving as reference. For 3-D ana

Radiological criteria for acceptable reduction of extra-articular distal radial fractures are not predictive for patient-reported functional outcome.

The purpose of this study was to assess the association between arm-specific disability measured with the QuickDASH questionaire and radiological criteria in patients with extra-articular distal radial fractures. A consecutive series of 385 patients were initially treated non-operatively for an extra-articular distal radial fracture and 257 (69 %) patients were included in the study. In 203 of these patients (78%) the quality of radiographic reduction was judged to be adequate by radiological parameters, which included dorsal tilt, radial inclination, and radial shortening. In 54 patients (22%

Case of bilateral Galeazzi fractures associated with dislocation of the right elbow.

We report an unusual case of bilateral Galeazzi fractures associated with dislocation of the right elbow and fracture of the right scaphoid caused by a motorbike accident in a 32-year-old man. Bilateral radiuses were fixed with plates, and the right scaphoid was fixed with a screw after closed reduction of the right elbow. A satisfactory result was obtained with a return to work and no radiographic problems 7 years after surgery. The mechanism of the injury was thought to be a high-velocity fall on the outstretched hands combined with extreme pronation of the forearms and extension of the wris

Atypical Essex-Lopresti injury of the forearm: a case report.

The Essex-Lopresti injury of the forearm is difficult to diagnose. A missed diagnosis can lead to persistent pain and instability of the wrist. We report a 33-year-old man with a pre-existing cubital varus deformity who sustained an atypical Essex-Lopresti injury after a fall on his elbow. Accurate diagnosis requires understanding of force transmission and anatomy of the radius and ulna, as well as the proximal and distal radioulnar joints as a unit.

Operative management of distal radial fractures with 2.4-millimeter locking plates: a multicenter prospective case series. Surgical technique.

BACKGROUND: In the past decade, there has been a trend toward open reduction and internal fixation of unstable distal radial fractures. There are now more than thirty different implant designs specific for the fixation of distal radial fractures. A multicenter prospective study of a case series was conducted to determine the efficacy of the operative management of distal radial fractures stabilized with 2.4-mm locking plates. METHODS: One hundred and fifty patients were entered into the study over a two-year period. The mean age of these patients was fifty-one years, and the cohort included e

Distal radial fractures in the elderly: operative compared with nonoperative treatment.

BACKGROUND: There is much debate regarding the optimal treatment of displaced, unstable distal radial fractures in the elderly. The purpose of this retrospective review was to compare outcomes for elderly patients with a displaced distal radial fracture who were treated with or without surgical intervention. METHODS: This case-control study examined ninety patients over the age of sixty-five who were treated with or without surgery for a displaced distal radial fracture. All fractures were initially treated with closed reduction and splinting. Patients who failed an acceptable closed reductio

Mechanical evaluation of a 4-mm cancellous "rescue" screw in osteoporotic cortical bone: a cadaveric study.

OBJECTIVES: Obtaining sufficient fracture fixation in osteoporotic bone is challenging. The purposes of the current study were (1) to compare the pullout strength of a 4-mm cancellous screw (cancellous screw group) with that of a 3.5-mm cortical screw (cortical screw group), and (2) to measure the pullout strength of a 4-mm cancellous screw placed as a rescue screw (rescue screw group) in a stripped 3.5-mm cortical screw (stripped screw group) hole while controlling for bone density and cortical thickness. METHODS: We inserted 4 screws, one from each experimental group, into 11 osteoporotic c

Is early internal fixation preferred to cast treatment for well-reduced unstable distal radial fractures?

BACKGROUND: In the treatment of distal radial fractures, physicians often advocate internal fixation over cast treatment for potentially unstable fracture patterns, citing the difficulties of successful nonoperative treatment and a decrease in patient tolerance for functional deficiencies. This study was performed to evaluate whether early internal fixation or nonoperative treatment would be preferred for displaced, potentially unstable distal radial fractures that initially had an adequate reduction. METHODS: A decision analytic model was created to compare early internal fixation with use o

[Therapy for radial head fractures].

The treatment of radial head fractures is challenging for the trauma surgeon due to the complex functional anatomy of the elbow. Satisfactory results can only be achieved if the function and stability of the humeroradial joint are entirely restored. Thus, depending on the fracture type, ranging from non-displaced Mason type I fractures to comminuted Mason type III fractures, an individual treatment concept has to be established. Treatment protocols range from conservative treatment with short-term cast immobilization to open reduction and internal fixation according to AO-principles or to the

Sonography for monitoring closed reduction of displaced extra-articular distal radial fractures.

BACKGROUND: Closed reduction and cast immobilization are employed in the primary treatment of most distal radial fractures, and conventional radiographic techniques have been essential and effective in monitoring these reductions. Radiation-free ultrasonography, however, can provide both real-time and dynamic multiple-plane images with a small and simple-to-use transducer that can be operated with only one hand. We therefore wanted to see if the real-time and dynamic multiple-plane observation capabilities of ultrasonography would allow an orthopaedic surgeon to perform a closed reduction with

[Posterior radiocarpal fracture-dislocation (eight case reports)].

The Authors report a retrospective series of eight cases of posterior radiocarpal fracture-dislocation. Average age of patients was 32 years (23/54), with a male predominance (7 men and a woman). Injury was often due to a fall on the hand in extension from an elevated place (5/8). The wrist in back of fork is the characteristic deformity. Diagnosis was confirmed by AP and lateral views of the wrist. Radial styloïd fracture, posterior dislocation of the carpus and posterior rim avulsion were constant. Ulnar styloïd fracture was present in half of the cases and distal radio-ulnar dislocation in

Marinacci communication. Case report.

Marinacci or reverse Martin-Gruber communication is a rare anatomical variation. Until now, in all reported cases the branch was diagnosed using electrophysiological studies but not substantiated anatomically. The authors report a case of ulnar-median nerve communication in the distal forearm that was discovered during carpal tunnel release surgery and confirmed postoperatively on electromyographic studies.
