Approximately 70% of war wounds involve the musculoskeletal system, and military orthopaedic surgeons have assumed a pivotal role in the frontline treatment of these injuries in Iraq. Providing battlefield orthopaedic care poses special challenges; not only are many wounds unlike those encountered in civilian practice, but patients also must be triaged and treated in an austere and dangerous environment, undergo staged resuscitation and definitive surgery, and endure prolonged medical evacuation, often involving ground, helicopter, and fixed-wing transport across continents. Most orthopaedic w

Advanced life support (ALS) includes all the procedures and maneuvers used to restore spontaneous circulation and breathing, thus minimizing brain injury. The fundamental steps of ALS are airway control with adjuncts, ventilation with 100% oxygen, vascular access and fluid and drug administration, and monitoring to diagnose and treat arrhythmias. Airway control can be achieved by means of oropharyngeal airway, endotracheal intubation, and alternative methods (laryngeal mask and cricothyroidotomy). Vascular access can be achieved by the peripheral venous, intraosseous, central venous, and trach

Regional tissue distress caused by microcirculatory dysfunction and mitochondrial depression underlies the condition in sepsis and shock where, despite correction of systemic oxygen delivery variables, regional hypoxia and oxygen extraction deficit persist. We have termed this condition microcirculatory and mitochondrial distress syndrome (MMDS). Orthogonal polarization spectral imaging allowed the first clinical observation of the microcirculation in human internal organs, and has identified the pivotal role of microcirculatory abnormalities in defining the severity of sepsis, a condition not

A 59-year-old man presented with severe dyspnea caused by advanced laryngeal cancer. As he had disregarded the dyspnea for a month, we did not have enough information about the extent of the tumor. The pulse oximeter showed 88% with oxygen inhalation. Because of severe dyspnea, he could not maintain supine position. Fiberoptic laryngoscopy showed tumor bulk obstructing airway directly. In the operating room, at first, a cricothyroid membrane puncture was attempted under local anesthesia but the procedure was abandoned when the patient became hypoxic and unconscious. Immediately tracheostomy an

The causes of non-haemorrhagic obstetric shock (pulmonary thromboembolism, amniotic fluid embolism, acute uterine inversion and sepsis) are uncommon but responsible for the majority of maternal deaths in the developed world. Clinically suspected pulmonary thromboembolism should be treated initially with heparin and objective testing should be performed. If the diagnosis is confirmed, heparin is usually continued until delivery, following which anticoagulation in the puerperium is achieved with either warfarin or heparin. Amniotic fluid embolism is a rare complication of pregnancy, occurring mo

There has been a steady increase in the number of deaths occurring from volatile substance abuse per year. The majority of deaths are in males, the peak age group being 15-19 years. Fuel gases i.e. cigarette lighter refills (butane), propane and gasoline and other solvents such as typewriter correction fluid, dry-cleaning fluids (trichloroethane) and fire extinguishers (bromochlorodifluoromethane) are responsible for approximately 60% of these deaths. Volatile substances sensitise the heart to circulating catecholamines, such that sudden alarm or exercise may precipitate sudden death. The main

The ability to deliver large volumes of intravenous (IV) fluids may be critical to the successful prehospital resuscitation of hypovolemic patients. We compared the time required to deliver one liter of crystalloid solution, using an administration set-up consisting of a 16-gauge (g), 1.25 inch, intravenous cannula, a pneumatic pressure bag, and either conventional intravenous tubing (3.2 mm internal diameter [ID]) or large bore (4.4 mm internal diameter [ID]) "shock" tubing. With the fluid bag positioned at 110cm (46 inches) above the level of the cannula, the mean elapsed time to deliver 1,0

BACKGROUND: When performing splanchnic nerve neurolysis (SNN), systemic hypotension may occur due to upper abdominal sympathetic blockade; therefore, appropriate periprocedural fluid resuscitation is crucial.
OBJECTIVES: The aims of this retrospective observational study were: 1) to validate the efficacy and safety of our prophylactic periprocedural fluid resuscitation in order to prevent systemic hypotension post-SNN, and 2) to explore the indicators that predict the need for additional fluid administration post-SNN.
STUDY DESIGN: This was a retrospective observational study using medical r

The arc of Buhler is a persistent connection between the celiac trunk and the superior mesenteric artery, which is an embryogenic remnant. This case report details the series of events that led to the discovery of an arc of Buhler after a patient was found to have significant hemoperitoneum secondary to the rupture of this pathology. It includes the patient's initial presentation, resuscitation, and surgical management.

Diabetes insipidus (DI) and other features of hypothalamic dysfunction were observed in two patients resuscitated following cardio-respiratory arrest (CRA). Both patients also developed marked hyperglycemia probably related to hypertonic dehydration. The development of DI in survivors of CRA suggests severe hypothalamic hypoxic injury and poor prognosis.

A 74-year-old man with mixed bicuspid aortic valve disease, combining severe calcific stenosis with severe regurgitation, and a left ventricular ejection fraction of 26% presented in normotensive cardiogenic shock with anuria and an arterial lactate of 12 mmol/L. Computed tomography showed a fibrocalcific left-right raphe and an aortic angulation of 66 degrees. Emergency transfemoral transcatheter aortic valve implantation was undertaken. The delivery system could not be advanced across the aortic arch despite escalation of guidewire support, and ventricular fibrillation degenerated into pulse

PURPOSE OF REVIEW: Hemodynamic monitoring has undergone a profound transformation over the last 30 years. The field has transitioned from the "standard of care" invasive pulmonary artery catheterization (PAC) toward minimally invasive and noninvasive technologies. This evolution is characterized by a shift in clinical philosophy.
RECENT FINDINGS: A very strong shift away from the measurement of static pressures and flow defining specific hemodynamic profiles to the assessment of functional physiology and fluid responsiveness characterize the main changes in hemodynamic monitoring over this ti

BACKGROUND: New literature and military conflicts inspired the second edition of the Guidelines for the Management of Penetrating Traumatic Brain Injury (pTBI). In an effort to make the evidence-based guidelines more impactful to clinical practice, expert consensus and treatment algorithms were incorporated.
METHODS: The published pTBI guidelines were reviewed, and an executive summary of the recommendations specifically relevant to trauma and critical care were created. These include resuscitation, imaging including screening for vascular injuries, critical care management (intracranial pres

Lung ultrasound (LUS) is increasingly used in the delivery room to assess respiratory transition and guide early respiratory management. The objective of this systematic review was to evaluate current evidence on the diagnostic applications of neonatologist-performed LUS during neonatal resuscitation and immediate postnatal stabilization in late preterm and term neonates. Twelve studies were included in this qualitative systematic review, addressing physiological LUS patterns after birth, the use of LUS scoring systems to predict the requirement of respiratory support, and applications of LUS

Albumin administration in sepsis and septic shock remains controversial because of uncertain clinical benefits and complex acid-base effects. Unlike crystalloids, albumin influences acid-base equilibrium through effects on chloride balance, strong ion difference (SID), buffering systems, and weak acid concentration. These effects may be interpreted differently by the traditional bicarbonate-centered framework and Stewart's physicochemical approach. In the traditional paradigm, albumin acts mainly as a non-bicarbonate plasma buffer influencing base excess and anion gap interpretation. In the St

Hemorrhagic shock remains one of the leading causes of preventable death for both civilian and military trauma. Fluid resuscitation is the primary treatment but requires constant monitoring, particularly for volume non-responsive patients susceptible to fluid overload, pulmonary edema, and other life-threatening conditions. To overcome fluid non-responsiveness, vasoactive drugs or vasopressors can be necessary adjuvants to fluid therapy but require tedious titrations that can be difficult to manage during mass-casualty situations. This study developed and evaluated automated closed-loop vasopr

Extracorporeal membrane oxygenation (ECMO) has a life-saving role in trauma management, with particular relevance to patients with acute respiratory failure. This narrative review describes the current evidence on the use of ECMO in the resuscitation of major trauma victims presenting with acute respiratory distress syndrome, cardiogenic shock, poisoning, burns, and traumatic brain injury, and highlights recent advances in the field. A comprehensive literature search of PubMed and EMBASE was performed for articles, using the MeSH/keywords. Over the past decade, ECMO has been increasingly appli
