Median nerve compression, with tendon ruptures, after Colles' fracture.

A rare case of rupture of the flexor pollicis longus tendon following a Colles' fracture is described. The patient also had a ruptured flexor digitorum profundus to the index finger and compression of the median nerve of the same hand. The ruptures were noted after four weeks of plaster immobilisation. Decompression of the median nerve and corrective osteotomy was performed but no tendon repair was attempted. The patient regained good function of the hand.

The prevalence of ruptured chordae tendineae in the mitral valve prolapse syndrome.

Echocardiographic studies were performed on 134 consecutive patients with idiopathic mitral valve prolapse syndrome. Fifteen patients (11.2%) were noted to have ruptured chordae tendineae on M-mode examination and in 12 of them the diagnosis was confirmed by bidimensional studies. Only four patients were referred for surgery as a result of severe mitral regurgitation. At operation one patient was found to have rupture of the anterior mitral chorda and the other three had posterior mitral chordal rupture. Eleven patients with chordal rupture had either mild symptoms or were completely asymptoma…

Postcontrast computed tomography in subarachnoid hemorrhage from ruptured aneurysms.

Computed tomography (CT) scans of 49 patients with subarachnoid hemorrhage (SAH) secondary to ruptured aneurysms were reviewed. Subarachnoid blood was detected in 95% when CT was performed within 5 days after the bleeding. Prediction of the site of the ruptured aneurysm from distribution of the cisternal blood varied from 92% for the anterior cerebral artery aneurysms to 70% for the middle cerebral and 47% for the internal carotid artery aneurysms. Following plain CT scans, postcontrast studies were performed in 27 patients. Aneurysms were directly visualized in 8 cases and subarachnoid enhanc…

Successful evacuation of a pontine hematoma secondary to rupture of a venous angioma.

A middle-aged woman presented with progressive brainstem neurological deficits. Clinical manifestations and preoperative radiological examination were indicative of an expanding intrapontine mass. At operation, an intrapontine hematoma and a venous angioma adherent to the floor of the fourth ventricle were discovered. Successful evacuation of the hematoma was followed by an excellent recovery. Postoperative angiography demonstrated the venous angioma. To our knowledge, this is the first case of angiographically proved ruptured venous angioma of the pons treated successfully by an operation.

[Treatment of fresh and old ruptures of the extensor tendons (author's transl)].

Knowledge of topographical anatomy and physiological interplay of several structures is considered essential for regaining the normal function of the extensor apparatus of the finger after an acute or an old injury. For better comprehension of the extensor apparatus and because of its anatomical and functional complexity, two types of structural systems are described, namely, the tendinous and the retinacular system. Aetiological aspects and the pathophysiological mechanisms of deformities such as the "swan-neck deformity, boutonnière deformity and mallet finger" are also described and possibi

Rupture of a papillary muscle of the tricuspid valve in primary pulmonary hypertension.

Rupture of a papillary muscle of the tricuspid valve is a rare occurrence, and nontraumatic rupture is still rarer. We describe a 26-year-old male with primary pulmonary hypertension presenting with severe dyspnea and paroxysmal nocturnal dyspnea following spontaneous rupture of the septal papillary muscle of the tricuspid valve. The clinical diagnosis was confirmed at autopsy.

Anorexia nervosa: death from complications of ruptured gastric ulcer.

Death from the complications of a ruptured gastric ulcer in a 15 year old girl with anorexia nervosa is reported. A Medlars II search of the literature indicates no previously reported deaths from this cause in anorexia nervosa. This patient presented the classical clinical characteristics of the syndrome and the course of her illness serves to re-emphasize the seriousness of the disease and the importance of early management of anorexia nervosa.

Echocardiographic diagnosis of ruptured aortic valve leaflet in bacterial endocarditis.

Aortic valve rupture, secondary to aortic valve endocarditis, was diagnosed echocardiographically and closely followed preoperatively. The ruptured left coronary cusp of the aortic valve was seen as dense irregular echoes, located anteriorly during ventricular diastole, and protruding into the left ventricular outflow tract in an otherwise normally appearing aortic valve. These echocardiographic findings, when correlated with changes in the clinical status of the patient, prompted immediate cardiac catheterization and aortic valve replacement. Early echocardiographic detection of abnormal aort…

Rupture of the spleen in a patient with a perforated duodenal ulcer and infectious mononucleosis.

A 72-year-old patient presented as a sealed perforated duodenal ulcer. This was later confirmed at operation when in addition a haemoperiton … cells confirming the diagnosis. Infectious mononucleosis in the elderly is rare and a complicating rupture of the spleen at this stage has not been reported previously. The diagnosis may depend on the histology alone because the peripheral blood film and serology can be negative. The possible role of the perforated duodenal ulcer is discussed.

Intraperitoneal rupture of pancreatic pseudocyst: report of a case and review of the literature.

A case is presented of pancreatic pseudocyst which ruptured as a result of an automobile accident. Sixty-three cases from the literature of ruptured pseudocyst of the pancrease due to a variety of causes are reviewed. Special reference is made of the site of perforation and treatment. It is concluded that each case should be individualized, tailoring the operative procedure to the operative findings. While external drainage may be life saving in some cases and sometimes the only procedure possible, internal drainage is superior and should be considered when it can be done without endangering t…

The treatment of acute traumatic rupture of the aorta: a 10-year experience.

Forty-three patients with aortic rupture secondardy to blunt trauma have been treated at the University of Micigan within the past 10 years with an overall salvage rate of 70%. The diagnosis should be suspected in anyone who has sustained a high speed decelerating injury, if the chest roentgenogram shows media-stinal widening, whether or not there is hypertension of the upper extremities; systolic murmur, or external evidence of chest injury. Aortography should be employed to confirm the diagnosis and to determine the site or sites of rupture. Repair of the lesion should be undertaken as soon …
