Guineaworm infection in the Wa district of north-western Ghana.

The Ghana-5 schistosomiasis project is situated in an exclusively rural area of north-western Ghana. Since the inhabitants rely for the most part on natural sources of drinking water the transmission of both urinary schistosomiasis and guineaworm infection must often occur at the same sites, and the epidemiology and the problems of control of these diseases might be expected to have features in common. An epidemiological survey of 8 300 people in 1967-68 showed that guineaworm had a scattered distribution, 35 of 43 villages having an annual incidence of less than 10%. Intensive study of 5 of t

Financial burden of noncommunicable diseases on households in Bangladesh: a quasi-experimental analysis using national survey data.

BACKGROUND: Noncommunicable diseases (NCDs) are a rapidly emerging health challenge in low- and middle-income countries, including Bangladesh, where research into their household financial burden remains limited and predominantly descriptive. Existing studies lack robust adjustment for confounders, creating uncertainty about differences in financial outcomes between NCD-affected and non-NCD households. To address this gap, this study uses a quasi-experimental analytic framework and repeated cross-sectional data to estimate adjusted differences in household financial outcomes over time. METHODS

Building and Sustaining a Statewide Telepsychiatry Network: Lessons Learned from the North Carolina Statewide Telepsychiatry Program (NC-STeP).

BACKGROUND: North Carolina faces persistent shortages of psychiatric professionals, particularly in rural and underserved regions, resulting in prolonged emergency department (ED) boarding, avoidable psychiatric hospitalizations, and inequitable access to behavioral health services. The North Carolina Statewide Telepsychiatry Program (NC-STeP), launched in 2013, is one of the nation's longest-running statewide telepsychiatry programs. OBJECTIVE: To summarize the development, implementation, outcomes, and lessons learned from NC-STeP across ED, community, maternal, pediatric, and university se

Esketamine for Treatment-Resistant Depression in Pakistan: A Promising Strategy.

Depression, projected to be the leading cause of global disability by 2030, affects up to 19.62% of Pakistan's population. Treatment-resistant depression (TRD), characterized by poor response to at least two antidepressants, poses a major clinical challenge. Although interventions like ECT exist, their use is limited by systemic barriers. Esketamine, an NMDA receptor antagonist, offers a rapid-onset antidepressant effect and has shown higher remission rates compared to quetiapine in TRD trials. Despite promising evidence, esketamine is not currently available in Pakistan, and cost-related barr

Recreation as a Potential Stress-Relief Mechanism for People Incarcerated in Jail.

Incarceration is a significant stressor with long-term health implications. Limited autonomy in jail restricts stress-reducing opportunities, but recreation time (rec-time) may help mitigate stress. We estimated the association between rec-time attendance and stress among people incarcerated in a rural Southwestern U.S. jail. From 2017 to 2018, we recruited participants from Coconino County Detention Facility for a cross-sectional health study. Participants completed the Perceived Stress Scale (PSS) and reported rec-time attendance. A log-binomial regression estimated prevalence ratios for rec

Interhospital Care Transitions: Coordinating Care Across Institutions.

Interhospital transfers (IHT) are indispensable for providing timely access to specialized services, particularly for rural populations and high-acuity conditions such as trauma, stroke, and myocardial infarction. However, IHTs introduce substantial risks, including transport complications, care discontinuity, increased costs, prolonged hospitalization, and elevated mortality. Although Emergency Medical Treatment and Labor Act and Centers for Medicare & Medicaid Services regulations establish baseline safeguards, coordination deficiencies persist. Centralized transfer centers, standardized doc

Tracheostomy reversal years after patient lost to follow-up.

BACKGROUND: Pediatric tracheostomies occur for various reasons, including prologned intubation, and require a multidisciplinary approach with routine follow-up. CASE PRESENTATION: This report reviews the history and clinical outcome of a 29 year old female patient who was lost to follow-up for nearly two decades after a pediatric tracheostomy. When she presented to the Otolaryngology service as an adult the original indication for tracheostomy had resolved and decannulation was initiated, but a profound psychological dependence had developed. CONCLUSION: This case outlines the importance of

Combined Intimate Partner Violence and HIV/AIDS Prevention in Rural Uganda: Design of the SHARE Intervention Strategy.

Intimate partner violence (IPV) has a bidirectional relationship with HIV infection. Researchers from the Rakai Health Sciences Program (RHSP), an HIV research and services organization in rural Uganda, conducted a combination IPV and HIV prevention intervention called the Safe Homes and Respect for Everyone (SHARE) Project between 2005 and 2009. SHARE was associated with significant declines in physical and sexual IPV and overall HIV incidence, and its model could be adopted as a promising practice in other settings. In this article we describe how SHARE's IPV-prevention strategies were integ

Dynamics of healthcare inequalities in type 2 diabetes mellitus across the COVID-19 pandemic: a real-world population-based study.

BACKGROUND: healthcare inequalities have been widely documented, yet the COVID-19 pandemic may have altered their magnitude and direction. This study aimed to analyse the dynamics of healthcare inequalities in patients with type 2 diabetes mellitus (T2DM) before and after the pandemic using a real-world data approach. METHODS: we conducted a real-world observational study including T2DM patients aged ≥ 45 from the CARhES cohort, a population-based dataset integrating clinical and administrative information from Aragón (Spain), between 2017 and 2022. Healthcare utilisation was assessed at two

Trends and inequities in primary and emergency care use among older adults in British Columbia: an analysis across age, sex, income, and rurality (2018-2023).

BACKGROUND: Access to care in Canada has become increasingly unstable, exacerbated by workforce shortages and the COVID-19 pandemic, which disrupted in-person care and accelerated the shift to virtual modalities. This hugely affects older adults (65+) as a high-need population. Our study addresses gaps in the literature and the trends of health care service utilization by providing a population-level analysis of trends and inequities in primary and emergency care use among older adults in British Columbia (BC) from the pre-pandemic period through to recovery (2018-2023). METHODS: We conducted

Targeted Behavior Change Communication Using a Mobile Health Platform to Increase Uptake of Long-Lasting Insecticidal Nets Among Pregnant Women in Tanzania: Hati Salama "Secure Voucher" Study Cluster Randomized Controlled Trial.

BACKGROUND: Malaria remains a significant cause of maternal and neonate morbidity and mortality in sub-Saharan Africa. Long-lasting insecticidal nets (LLINs) represent an important component of malaria prevention and can decrease the adverse health outcomes associated with malaria infection during pregnancy. Voucher programs have been successfully implemented for a variety of initiatives across sub-Saharan Africa, including the distribution of subsidized LLINs in Tanzania. However, mobile messaging for behavior change communication (BCC), in combination with an e-voucher program, has not been

Battling Alone on Multiple Fronts - How Gender Norms Affect the Soldiers' Wife as Caregiver in India.

OBJECTIVES: Women form the backbone of caregiving in palliative home care throughout the world. They put in more intense care for longer hours, perform more intimate tasks, and face more physical and mental strain, comorbidities, anxiety, and depression. However, gender norms that perceive caregiving as a natural part of femininity dismiss this as part of their duty toward their family, thus making their care work invisible, taken for granted, and devalued. This results in women bearing more burden with less support and no appreciation and suffering more negative mental and physical health out

A systematic literature review of schistosomiasis in urban and peri-urban settings.

BACKGROUND: Schistosomiasis is a parasitic disease caused by trematode worms of the genus Schistosoma and belongs to the neglected tropical diseases. The disease has been reported in 78 countries, with around 290.8 million people in need of treatment in 2018. Schistosomiasis is predominantly considered a rural disease with a subsequent focus of research and control activities in rural settings. Over the past decades, occurrence and even expansion of schistosomiasis foci in peri-urban and urban settings have increasingly been observed. Rural-urban migration in low- and middle-income countries a
