Acromioclavicular joint reconstruction using peroneus brevis tendon allograft.

We describe the use of a double-strand peroneus brevis allograft to reconstruct the coracoclavicular and acromioclavicular (AC) joint ligaments. Through sharp dissection, the distal clavicle, the AC joint, and the torn superior AC and coracoacromial ligaments are identified. The coracoid process and injured coracoclavicular ligaments are identified with blunt dissection. A 1-cm segment of the lateral clavicle is resected. Vertical and connecting horizontal tunnels are created (4.5 mm) in the lateral clavicle and in the medial acromion process. The 5.5- to 6.0-mm-diameter allograft is looped ar

Restoration of shoulder abduction by transfer of the spinal accessory nerve to suprascapular nerve through dorsal approach: a clinical study.

BACKGROUND: In recent years, transfer of the spinal accessory nerve to suprascapular nerve has become a routine procedure for restoration of shoulder abduction. However, the operation via the traditional supraclavicular anterior approach often leads to partial denervation of the trapezius muscle. The purpose of the study was to introduce transfer of the spinal accessory nerve through dorsal approach, using distal branch of the spinal accessory nerve, to repair the suprascapular nerve for restoration of shoulder abduction, and to observe its therapeutic effect. METHODS: From January to October

Current concepts in the recognition and treatment of superior labral (SLAP) lesions.

Pathology of the superior aspect of the glenoid labrum (SLAP lesion) poses a significant challenge to the rehabilitation specialist due to the complex nature and wide variety of etiological factors associated with these lesions. A thorough clinical evaluation and proper identification of the extent of labral injury is important to determine the most appropriate nonoperative and/or surgical management. Postoperative rehabilitation is based on the specific surgical procedure as well as the extent, location, and mechanism of labral pathology and associated lesions. Emphasis is placed on protectin

[Adhesive capsulitis: evaluation of a treatment coupling capsular distension and intensive rehabilitation].

INTRODUCTION: Adhesive capsulitis is a clinical entity characterized by a loss of motion and a decrease of the joint volume capacity. Results of clinical trials have shown that distensions can help to decrease pain and increase the range of motion. OBJECTIVE: The purpose of the study was to measure the effect of repeated distension arthrography combined with an intensive program of rehabilitation on pain, range of motion, and occupational outcome in patients with adhesive capsulitis. DESIGN: 39 cases of idiopathic capsulitis (7 cases) or secondary capsulitis (19 post-traumatic, 13 post-surge

Bilateral vocal cord palsy following interscalene brachial plexus nerve block.

Acute respiratory complications of an interscalene brachial plexus block include ipsilateral phrenic nerve and recurrent laryngeal nerve palsies. A 71-year-old woman who had undergone a total thyroidectomy for papillary carcinoma 35 years ago was administered a right interscalene brachial plexus block for a shoulder hemi-arthroplasty. Subsequently she developed acute respiratory distress associated with marked stridor secondary to an acute right vocal cord palsy, which was superimposed on what was assumed to be a preexisting left-sided vocal cord palsy. On extubation the patient was noted to d

Arthroscopic Bankart procedure by suture technique: indications, technique, and results.

The arthroscopic Bankart procedure by suture technique was performed in 31 patients (1984-1987). In 22 of the cases, the minimum follow-up was 2 years. Fourteen patients had sustained a primary traumatic complete dislocation resulting in unidirectional instability, and eight patients experienced recurrent subluxation. Clinical examination demonstrated positive labrum pathology in all cases, which was proven in five patients by contrast computed tomography scan. None of the 22 patients experienced intraoperative complications or infection. A slight loss of abduction was found in two patients, a

Anatomy of the axillary nerve and its relation to inferior capsular shift.

Axillary nerve injury is a recognized complication of the capsular slide procedure for multidirectional instability of the shoulder. Axillary nerve dissection followed by an anterior or posterior capsular shift procedure was carried out on 12 autopsy subjects to observe: (1) the normal relationships of the nerve; (2) its proximity to structures dissected in the procedure; and (3) the effects upon it of both anterior and posterior capsular shift procedures. The axillary nerve arises immediately posterior to the coracoid process and conjoint tendon. It crosses the inferolateral border of the sub

MR imaging of the normal shoulder.

Magnetic resonance (MR) images of the shoulders of a healthy volunteer were obtained in axial, sagittal, and coronal orientations using a 0.5-T imaging system. Multiple high-resolution spin-echo images were generated using an off-center zoom technique and a specially designed surface coil. Several anatomic structures, including the rotator cuff, long biceps tendon, articular capsule, muscles, and bones, were visualized. The coronal and sagittal views were the most useful for demonstrating the rotator cuff. MR imaging has potential as a new non-invasive tool for the evaluation of the shoulder r

Biomechanics of the rotator cuff: European perspective.

The rotator cuff has an important role in the stability and function of the glenohumeral joint. To understand the biomechanical proprieties of the rotator cuff, it is essential to understand the pathogenesis and effects of rotator cuff tears. The rotator cuff provides a stabilizing effect to the shoulder, because of compression of the humeral head against the glenoid cavity. The wide range of motion of the shoulder is allowed by the variety of rotational moments of the cuff muscles. Rotator cuff muscles action must be precisely coordinated to obtain the desired movement. Rotator cuff tendons a

Posterior superior internal impingement: an evidence-based review [corrected].

Over the last decade, the concept of internal impingement has continued to evolve, and the frequency with which this condition is recognised continues to increase. This syndrome should be clearly differentiated from the classical (external) impingement that is thought to be caused by compression of the subacromial bursa, long head of the biceps tendon and rotator cuff (RC) by the coraco-acromial arch. Internal (posterosuperior) impingement syndrome is typified by a painful shoulder due to impingement of the soft tissue, including the RC, joint capsule and the posterosuperior part of the glenoi

Scapular deformity in obstetric brachial plexus palsy: a new finding.

While most obstetric brachial plexus palsy patients recover arm and hand function, the residual nerve weakness leads to muscle imbalances about the shoulder which may cause bony deformities. In this paper we describe abnormalities in the developing scapula and the glenohumeral joint. We introduce a classification for the deformity which we term Scapular Hypoplasia, Elevation and Rotation. Multiple anatomic parameters were measured in bilateral CT images and three-dimensional CT reconstruction of the shoulder girdle of 30 obstetric brachial plexus palsy patients (age range 10 months-10.6 years)

[Arthroscopy and MRT of the shoulder--a comparative retrospective analysis].

MR Imaging is proving to an effective means for evaluating the shoulder. The use of a surface coil and high resolution scanning techniques have allowed detailed analysis of normal anatomy and suspected pathology of the shoulder. On the other way arthroscopic inspection provides a more extensive visualization of the joint in different positions. 31 patients, well documented, were studied with MR Imaging and correlated with findings in arthroscopy and open revision in some cases. The MR studies were retrospective interpreted without the knowledge of the results of other diagnostic procedures. Fo
