[Suggestions for criteria for uniform evaluation of changes in the spine in vertebral dysostosis of the Scheuermann type].

The author recommends in his proposal of uniform criteria to evaluate pathological changes of the spine according to a pattern where the first place is held by morphological changes (unevenness of the covering plates, changes in the shape of bodies of the vertebrae, changes in the height of disc and changes in the curvature of the spine). The second place in held by functional changes and the third one by the general extent and quality of pathological changes. In the latter group the author draws attention to the appearance of a stigmatized spine. It is essentially an abortive form of vertebra…

[Perforation of the cervical esophagus and hypopharynx complicating surgery by an anterior approach to the cervical spine].

Difficult diagnostic and therapeutic problems are raised by perforations of the cervical oesophagus or hypopharynx in patients undergoing surgery to the cervical spine via an anterior approach. Based on their experience of three recent cases, the authors review the diagnostic approach, based on clinical examination and diatrizoate sodium oesophageal series, and propose conservative treatment consisting of surgical drainage with or without suture of the perforation and without removal of the osteosynthesis material, appropriate antibiotic therapy and hypercaloric enteral nutrition via nasogastr…

[Flexion-distraction injuries of the thoracolumbar spine with complete pancreatic rupture. Description of a case].

A case is reported of an unstable fracture dislocation of the thoracolumbar vertebral column combined with complete rupture of the pancreas. This combination of injuries is rarely encountered but is dangerous in severely injured patients. Rapid and decisive operative treatment of both injuries should be performed, whereby adequate treatment of the pancreatic rupture has the highest priority.

[Osteogenesis imperfecta and localized involvement of the spine: a peculiar form? Apropos of a familial case].

The authors report a particular type of Osteogenesis Imperfecta (O.I.), where the symptoms of bony fragility affected only the spine in three members of the same family, a father and his two daughters. This type of O.I. represents a type IA variety according to the classification of D. Silence. The authors while underlining the wide variety of this condition think that it is very likely that non recognition of the condition is in part due to the mild symptoms attributable to this lesion in the spine. This diagnosis has to be considered for all symptomatic osteoporosis of the vertebrae in child…

Closed subarachnoid drainage for management of cerebrospinal fluid leakage after an operation on the spine.

A retrospective review was conducted to assess the effectiveness and safety of a temporary subarachnoid shunt to treat patients who have a leak of cerebrospinal fluid after a spinal operation. The shunt is percutaneously inserted in the lumbar spine and is removed after four days. This technique was used in nineteen patients over a ten-year period. Of the seventeen patients who had the shunt in place for the full four days, fourteen had resolution of the drainage of cerebrospinal fluid from the wound. One of two patients whose shunt was removed early also had a successful result. Eleven of the…

[Commentary on the article "Is the mechanical traction of the cervical spine still acceptable?" by R. Dorian ].

Referring to literature Dorian reported on severe complications after apparative traction of the cervical vertebral column up to cross-sectional paralysis and sudden death during treatment. Based on these reports he presents a list of contraindications which comprises nearly all diseases of the cervical vertebral column. The author summarizes his statements claiming that the apparative traction at no means can be a routine method because of it's inherent extreme risks. The comment in hand states that none of the complications claimed have been exemplified by the author in the original paper. N

The surgical treatment of late instability of flexion-rotation injuries in the lower cervical spine.

The authors reviewed 12 cases of anterior dislocation of the lower cervical spine due to flexion-rotation trauma, with late instability after closed treatment. The 12 patients were treated at the Instituto Ortopedico Rizzoli between 1977 and 1982. Epidemiology, causes, type of lesion, and associated complications were studied. After an average period of 3 years (range: 2-6), correction appeared stable in all of the patients. In 4 patients, posterior stabilization with plates was performed after presurgical correction in a halo-thoracic cast. In 4 patients, reduction with combined, treatment co…

[Diagnosis of hyper- and hypomotility of the upper cervical spine using functional computerized tomography].

At present, there are few methods that are of any assistance in evaluating the ligaments of the upper cervical spine. It is obvious that a lesion of these ligaments could result in instability of the cranial cervical junction. Nine healthy adults and 45 patients with suspected rotatory instability of the upper cervical spine after trauma were examined using functional CT-scans. After CT scans of the upper cervical spine had been obtained with the patient in a neutral position, the head was passively rotated to the left and right and held in these positions by adhesive tape. CT scans were perfo…

MRI of the brain and cervical spine: first choice in the detection of abnormalities. Preliminary study.

The preliminary results are presented of a comparative study between MRI and CT in 150 patients from the University Hospital Utrecht. The purpose is through a prospective study to make more definitive assessments about the role and the value of MRI in the region of the brain and cervical spine. If compared with CT, no abnormalities were missed with MRI in 150 cases; MRI detected more lesions than CT, and often more information was provided by MRI. Spin-echo pulse sequence with long multiple echoes and short repetition time proved to be a sensitive detection method with sufficient tissue charac…

Occipito-cervico-thoracic spine fusion in a patient with occipito-cervical dislocation and survival.

A 16-year-old man with occipito-cervical dislocation and survival was treated at Rancho Los Amigos Medical Center. The patient had ligamentous instability between the occiput and the cervical spine. His neurologic level was complete at the C1 level, and he was dependent on a mechanical respirator. Sternocleidomastoid and other neck musculature were not functional. A fusion of the occiput to cervical spine and cervical spine to thoracic spine was performed to obviate the need for external neck support. The patient went on to fuse and is now independently mobile with a tongue-switch driven wheel…

Comparison of histomorphometry and computerized tomography of the spine in quantitating trabecular bone in renal osteodystrophy.

Computerized tomography (CT) has the advantage of allowing the isolation of trabecular bone of the axial skeleton, which is the more sensitive to metabolic changes. By means of single-energy X-ray CT we have devised a method for assessing trabecular bone mass (TBM) at L-4 vertebral body, which has an acceptable reproducibility (CV = 2.76%). Normal values where obtained after studying 29 normal individuals from both sexes and different ages. In order to know if the method is accurate, TBM was assessed in 17 patients (6 with creatinine clearance less than 6 ml/min and 11 on chronic hemodialysis)

[Histopathological studies of the osteophytes and ossification of the posterior longitudinal ligament (OPLL) in the cervical spine].

To obtain information on ossification around the vertebral body of the cervical spine, we carried out histopathological investigation on 30 autopsy cases. The results were as follows; Intervertebral discs were untouched, anterior osteophytes developed along the anterior longitudinal ligament and they corresponded to anterior spur in spondylosis deformans. Posterior osteophytes around the posterior edges of vertebral body resulted in intervertebral insufficiency, and corresponded to hypertrophic spur in osteochondrosis. Histopathological findings of hyperostotic anterior osteophytes were simila…

[Importance of the general radiograph of the cervical spine in vertigo and pathologic proprioceptive cervical nystagmus].

The general roentgenogram of the cervical spine often shows manifold pathological alterations by patients without any clinical complaints. On the other hand, there are cervical vertigo and different symptoms caused by the cervical spine, its joints, muscles and ligaments without any X-ray manifestation. Nevertheless, some connections are assumed to exist between vertigo and alterations of the cervical spine an assumption supported especially by the frequently successful elimination of cervical troubles by chirotherapy. We compared the X-ray manifestations in patients complaining of vertigo wit…

Relationship of spine deformity and pelvic obliquity on sitting pressure distributions and decubitus ulceration.

The distribution of pressure points in 16 patients with paraplegia, nine with ulcers, and six who were ulcer free were compared with the distribution in 15 normal individuals using an instrument capable of simultaneously measuring multiple pressure points under the buttocks and thighs. The nine patients with ischial and sacral decubiti showed redistribution of their sitting pressures posteriorly, asymmetrical loading of the ischiae, and higher than normal pressures under the sacrococcygeum. These abnormal pressures were associated with unbalanced scoliosis, pelvic obliquity, and the loss of ph

[Posterior arthrodeses of the upper cervical spine in children and adolescents. Apropos of 19 cases].

The authors have analysed 19 cases of cervical spine fusion in children and teenagers with a review of the literature. In the present series, the levels fused were between the occiput, C1 and C2. The ages of the patients ranged between 2 months and 18 years. There was a great variety of indications for these operations, the majority being congenital deformities or Morquio's disease. The main disabilities were neurological impairment, or instability or deformity of the neck. Recent advances in radiological investigation made it possible to make a more precise diagnosis of lesions with bony defo…

[Osteosynthesis of the low lumbar spine and spondylolisthesis. Modification of the Scholner plate and pedicular screwing].

A modification of Roy Camille's Technic for better screw location into the pedicle of vertebra of the low lumbar column and lumbo-sacral junction is reported. A small 5 millimeters resection of the inferior articular process allows a more cranial drill-hole which is more steadily in the pedicle of the vertebra as confirmed by X rays controls and anatomic specimens. A 10 degree slope to a caudal direction is needed at the L5 level and a 30 degree slope to a caudal direction is required at S1 level. 50 millimeters length screw can be used for adult patients. Some modification of Scholner's plate

Dural arteriovenous malformations of the spine. Clinical features and surgical results in 55 cases.

The clinical and angiographic findings of 55 patients with a spinal dural arteriovenous malformation (AVM) are reviewed, and the results of surgery assessed. The symptoms of dural AVM are usually gradual in onset, and hemorrhage from this type of AVM is less common than in true spinal cord angiomas. Other clinical features and the myelographic findings are similar to those of spinal cord angiomas. On angiography, the nidus of dural AVM's usually projected lateral to the spinal cord. Clipping of communicating vessels between the AVM and the coronal plexus was carried out in 50 patients, and dec
