Highly purified capsaicin has emerged as a promising injectable compound capable of providing sustained pain relief following a single localized treatment during orthopedic surgical procedures. To further assess its reliability for clinical use, the potential effect of highly purified capsaicin on articular cartilage metabolism as well as tendon structure and function warrants clarification. In the current study, rabbits received unilateral supraspinatus transection and repair with a single 1 ml injection of capsaicin (R+C), PEG-only placebo (R+P), or saline (R+S) into the glenohumeral joint (

BACKGROUND: There is scant literature regarding the treatment of myotendinous Achilles ruptures. The purpose of this study was to retrospectively examine clinical outcomes from uniform nonsurgical treatment of these injuries.
METHODS: Between November 2005 and May 2011, 30 patients presented with an acute, complete myotendinous Achilles rupture. The location of the Achilles injury was confirmed on magnetic resonance imaging (MRI) for all patients. All patients were treated nonsurgically, which involved 3 weeks of non-weight-bearing and then 3 weeks of progressive to full weight-bearing in an

INTRODUCTION: Hamstring strain injuries are common in sports that involve high-speed running. It remains uncertain whether the hamstrings are susceptible to injury during late swing phase, when the hamstrings are active and lengthening, or during stance, when contact loads are present. In this study, we used forward dynamic simulations to compare hamstring musculotendon stretch, loading, and work done during stance and swing phases of high-speed running.
METHODS: Whole-body kinematics, EMG activities, and ground reactions were collected as 12 subjects ran on an instrumented treadmill at speed

Traumas resulting from naso-orbitoethmoidal fractures and tumor surgery of the medial canthal region may cause orbital dystopia. A variety of techniques with many disadvantages, such as detachment and high cost, has been described for reattachment of medial canthal tendon. We present a new technique, namely, unitransnasal canthoplasty, which is easy to apply, cheap, and reliable. Two nonabsorbable sutures were passed through 2 drill holes to the nasal cavity, taken out from the nasal ostium, and were tied up. Upper and lower eyelids were fixed to the ipsilateral nasal bone.

This clinical outcomes study examined factors affecting patient satisfaction after treatment of Achilles tendon rupture. There were 35 patients; 14 treated nonsurgically and 21 treated by open surgical repair. A retrospective chart review was performed to determine the mechanism and type of injury, the time to treatment, any comorbidities, and the length of follow-up. Patients completed a questionnaire that evaluated their pre- and postinjury activity levels, occupation, and overall satisfaction with their function by using a visual analogue scale. The mean follow-up was 2 years. Seventy perce

BACKGROUND: Distal intraarticular radius fractures can be operated on by means of a palmar or dorsal approach. The dorsal approach has been reported as a treatment option that often leads to tendon ruptures. The purpose of this study was to show that a dorsal implant does not lead to a high rate of tendon ruptures.
METHODS: A retrospective study was conducted in which the patient population was analyzed for complaints and complications, follow-up time, and further surgical procedures.
RESULTS: A series of 389 patients with distal intraarticular fractures of the radius treated with the dorsal

Multiple digit amputations in children demand special consideration to make subsequent hand function optimal. Replanted digits in children have a comparatively lower viability rate, but those that do survive usually go on to excellent function. In this case, an amputated thumb was severely mangled and not suitable for replantation. An amputated index finger was transposed to the thumb position. A six-month postoperative follow-up of the transposed digit confirmed satisfactory joint motion, restored sensibility, and unimpaired digit growth. The 11-month-old infant described is the youngest pati

Tendons are made of compact dense collagen fibers with only sparse cellularity and naturally low immunogenicity. Allogenic tendons may be preserved through deep freezing methods and retain excellent mechanical properties after revitalization. Allogenic tendons were used in 22 patients (30 tendons) for second-stage tendon reconstruction in the hand. Preliminary results indicate no observable adverse tissue reactions, and functional recovery after tendon grafting does not seem different from reconstruction using tendon autografts. This type of allogenic graft does not seem to produce serious con

All Swedish male and female elite basketball players were interviewed concerning their injuries during 1981/1982. Fifty-eight percent of the male and 62% of the female players reported injuries. The injury frequency was 2.5 injuries/1,000 activity-hours in male and 2.85 injuries/1,000 hours of activity in female players. This corresponds to 8.6 injuries/male team/season and 7.5 injuries/female team/season. An ankle twist was the most common injury (52%) while a knee injury was involved in 18%. Prophylactic orthoses for knee and ankle are suggested.

Disabling shoulder pain is common in those who engage in athletic activities and there is a wide spectrum of causes. Some entities are secondary to an acute injury such as a shoulder subluxation or dislocation with subsequent disability. More commonly, we see pain resulting from repeated insult, such as chronic tendinitis or attritional tears of the rotator cuff. Each entity has its own distinctive characteristics, so that a thorough history, physical examination, radiograms, and, occasionally, special studies should result in the proper diagnosis. The appropriate treatment can then be institu

The structures of the posterolateral corner (PLC) work cohesively to provide both static and dynamic knee stability. Injury to the PLC disrupts both the static and dynamic properties. Although multiple PLC reconstruction techniques have been described, most address only the static, and not dynamic, function of the PLC. The popliteus tendon provides dynamic stability to the PLC, but its specific function is not addressed in current techniques. We describe an arthroscopic-assisted fibular-based dual reconstruction with popliteus repair to restore static and dynamic stability of the PLC.

Wrist arthroscopy presents a significant learning curve, necessitating various precautions to avoid complications like tendon laceration or nerve injury. Arthroscopic procedures become even more challenging in arthritic wrists due to altered anatomy and reduced joint space, which in turn increase the risk of complications. In this review article, we offer tips and tricks to minimize these risks. Despite the challenges, wrist arthroscopy remains a valuable diagnostic and therapeutic tool when performed by an experienced surgeon with thorough preoperative planning and careful patient selection.

Intra-articular injections are a nonsurgical treatment modality that can be used to manage osteoarthritis, naturally occurring or surgically induced acute synovitis, and intra-articular ligamentous or tendon injury. This option may be assistive for patients in which other conservative modalities are ineffective, or in conjunction with other forms of treatment. It may also be used as the primary treatment. Injectates labeled for use in companion animal joints include corticosteroids and viscosupplements. Additional injectates, that are not specifically approved for use in companion animals are

Distal triceps ruptures are an uncommon injury, and currently, there is a lack of consensus on the optimal surgical technique for repair. The traditional transosseous cruciate repair has been shown to be nonanatomic and to have a higher failure rate and be biomechanically inferior to suture anchor repairs. The use of anchors significantly increases procedure cost and theoretically increases the risk of complications related to drill and anchor placement. The technique described in this article allows anatomic restoration of the triceps tendon while avoiding the increased cost and risk that may

CASE REPORT: A 65-year-old man presented with pain in his left ankle. After a delay, he presented to the orthopaedic clinic where computed tomography scan revealed an intra-articular, locked-lateral calcaneal fracture with dislocation of the peroneal tendons.
CONCLUSIONS: This combination of injuries can produce subtle signs, such as an atypical double-density sign and varus talar tilt, on plain radiographs; however, the consequences of delayed treatment can be catastrophic. Early recognition and treatment are essential to avoid chronic pain and limited function.
