Adhesive Colonic Obstruction After Esophageal Reconstruction: An Uncommon Late Complication.

Adhesive colonic obstruction is a rare but clinically significant late complication following esophageal reconstruction with colon interposition. This case report describes a 68-year-old woman with a history of caustic esophageal injury and colonic interposition, presenting 12 years postoperatively with acute intolerance to oral intake and thoracic colonic distension. Imaging and endoscopy confirmed mechanical obstruction due to an adhesion. Surgical adhesiolysis restored conduit patency, resulting in uneventful recovery. This case highlights the importance of long-term surveillance and prompt

Isolated Right Chylothorax Secondary to Superior Vena Cava (SVC) Obstruction Successfully Treated With Endovascular Stenting.

Chylothorax, an uncommon cause of pleural effusion, typically results from thoracic duct injury or obstruction. Superior vena cava (SVC) obstruction is a rare etiology, often leading to right-sided chylothorax due to disrupted lymphatic drainage. This case report describes a 70-year-old female patient with a history of breast carcinoma who presented with right-sided chylothorax secondary to SVC thrombosis caused by a neglected chemoport. A CT venogram confirmed chronic SVC occlusion. Endovascular intervention, including angioplasty and stenting, successfully restored venous patency, resolving

Current Management of Uncomplicated Type B Aortic Dissection.

Aortic dissection is the most common thoracic aortic emergency and is associated with significant morbidity and mortality. Initial complications are dependent on reduction of sheer stress against the aortic wall to protect against rupture and minimize progression of the aortic wall injury. In patients with dissection starting at or distal to the left subclavian artery (Stanford type B), initial management includes strict blood pressure and heart rate control with monitoring for any complications such as malperfusion, rupture, or hemodynamic instability. Following the acute dissection event, su

Massive hemothorax secondary to internal jugular vein central venous catheter placement in a patient undergoing spinal surgery complicated by chest trauma: a case report.

BACKGROUND: Placement of a central venous catheter (CVC) is a common procedure for spinal surgery and is relatively safe under ultrasound guidance. CASE PRESENTATION: We report the case of a 56-year-old female who underwent ultrasound-guided placement of an internal jugular vein CVC for fluid replacement during spinal surgery for thoracic vertebral burst compression fracture and multiple rib fractures as a result of a high-altitude fall injury. Hemothorax developed intraoperatively. During a thoracotomy, the tip of the CVC was found within the chest cavity. The presence of chest trauma may im

False positive metastatic disease due to combined thoracic and subcutaneous splenosis.

A 56-year-old man presented with dyspnea secondary to pulmonary emboli and dilated cardiomyopathy. His past medical history included a history of emergency laparotomy, splenectomy, and splenic flexure resection following a gunshot injury 30 years ago. CT and MRI imaging demonstrated multiple homogeneously enhancing lobulated lesions at the left-sided pleura and chest wall with an irregular calcified spleen. The aforementioned lesions demonstrated a similar level of tracer uptake to the splenic activity with no evidence of other FDG avid malignancy on the follow-up 18F-FDG PET study. All the ab

Hemoptysis as a warning sign of thoracic aorta pseudoaneurysm: A case report.

Pseudoaneurysm is defined as a reperfused pulsatile hematoma, encapsulated and communicated with the damaged vessel's lumen. It originates when there is a disruption of the arterial wall. Hemoptysis is a very rare sign/symptom of a thoracic aortic aneurysm or pseudoaneurysm. There is little information on hemoptysis associated with aortic aneurysm rupture, whose mechanisms are not explained by the presence of an aortopulmonary fistula. Among the hypotheses to explain this phenomenon, is the ability of the bronchial arteries to become hyperplasic and tortuous in the presence of a lesion that mo

Split Azygos Vein: A Case Report.

The azygos venous system, which comprises the azygos, hemiazygos, and accessory hemiazygos veins, assists in blood drainage into the superior vena cava (SVC) from the thoracic cage and portions of the posterior mediastinum. Routine dissection of a fresh-frozen cadaveric specimen revealed a split azygos vein. The azygos vein branched off the inferior vena cava (IVC) at the level of the second lumbar vertebra as a single trunk and then split into two tributaries after forming a venous plexus. The right side of this system drained into the SVC and, inferiorly, the collective system drained into t

Pneumomediastinum following intubation in COVID-19 patients: a case series.

The number of patients requiring tracheal intubation rose dramatically in March and April 2020 with the COVID-19 outbreak. Our thoracic surgery department has seen an increased incidence of severe pneumomediastinum referred for surgical opinion in intubated patients with COVID-19 pneumonitis. Here we present a series of five patients with severe pneumomediastinum requiring decompression therapy over a 7-day period in the current COVID-19 outbreak. We hypothesise that the mechanism for this is the aggressive disease pathophysiology with an increased risk of alveolar damage and tracheobronchial

Expandable Titanium Cages in the Emergent Treatment of Severe Spinal Deformity Secondary to Osteomyelitis: A Series of Three Complex Cases.

The literature lacks robust evidence on the benefits versus risks of instrumenting and fusing the spinal column in the setting of active osteomyelitis. We report three patients with vertebral osteomyelitis and subsequent severe and complex kyphotic deformities. Patients 1 and 2 had previous instrumentation that required revision because of hardware failure in the thoracic and thoracolumbar regions, respectively. Patient 3 developed a severe cervical kyphotic deformity at 2 months after being diagnosed and treated with antibiotics for osteomyelitis, necessitating emergent instrumentation and fu

Cardiopulmonary resuscitation causing thoracolumbar hyperextension with severe spinal cord injury: A case report.

Thoracic vertebral fractures are extremely rare complications of cardiopulmonary resuscitation (CPR). A morbidly obese 79-year-old female positive for COVID-19 suffered cardiac arrest and received CPR for 18 minutes with return of spontaneous circulation. Post cardiac arrest the patient was unable to be weaned from the ventilator and had decreased lower extremity movement. A computed tomography scan of the chest/abdomen/pelvis demonstrated a widely diastatic spinal separation at the T12/L1 intervertebral disc space with L1 spinous process fracture. The patient ultimately expired from the sever

Thoracic Manifestations of Connective Tissue Diseases.

Connective tissue diseases (CTDs) are a common cause of chronic lung disease, and lung involvement, which can be the initial site of disease, is a major contributor to CTD-related morbidity and mortality. A variety of patterns of acute and chronic lung diseases are associated with CTD including diffuse alveolar damage, organizing pneumonia, diffuse alveolar hemorrhage, usual interstitial pneumonia, nonspecific interstitial pneumonia, and lymphoid interstitial pneumonia. Patients with CTD are also at increased risk for respiratory tract infection and treatment-related complications such as drug

Spontaneous Hemothorax, A Rare Face of Vertebral Osteochondroma.

BACKGROUND: Osteochondroma is the most common benign tumor of the bone. It is usually asymptomatic, but complications may result from mechanical injury to adjacent anatomic structures, such as the diaphragm and lung, when located intrathoracically. CASE REPORT: We report the unusual occurrence of a large hemothorax and lacerated right diaphragm in a 41-year-old woman caused by vertebral osteochondroma affecting the eleventh thoracic vertebra. Thoracoscopic exploration with resection of the osteochondroma and repair of the diaphragm was performed. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF
