Update on Gene Therapy Clinical Trials for Eye Diseases.

Inherited and complex retinal degenerative diseases, such as retinitis pigmentosa, age-related macular degeneration, and glaucoma, represent a significant global burden of irreversible vision loss. Due to immense genetic and clinical heterogeneity and complex underlying mechanisms, these diseases still lack safe and effective disease-modifying treatments. This review summarizes the current landscape of gene therapeutic approaches to develop novel treatments for these blinding conditions. Specifically, we provide an update on several ongoing or completed clinical trials on gene-specific or gene

Genomic Predictive Biomarkers in Breast Cancer: The Haves and Have Nots.

Precision oncology, also known as personalized oncology or precision medicine, is the tailoring of cancer treatment to individual patients based on the specific genetic, molecular, and other unique characteristics of their tumor. The goal of precision oncology is to optimize the effectiveness of cancer treatment while minimizing toxicities and improving patient outcomes. Precision oncology recognizes that cancer is a highly heterogeneous disease and that each patient's tumor has a distinct genetic diversity. Precision medicine individualizes therapy by using information from a patient's tumor

Recent advances of chimeric antigen receptor T-cell therapy for acute myeloid leukemia.

Acute myeloid leukemia (AML) is a heterogeneously primary hematopoietic neoplasm characterized by uncontrolled proliferation of immature myeloid cells, which is characterized with poor outcomes. Despite tremendous advances in the treatment paradigm of AML in the past several decades, the cure and prognosis remain unfavorable. More effective treatments are therefore needed to improve the clinical outcomes. Among newly emerging immunotherapies, chimeric antigen receptor (CAR)-T cell immunotherapy is an exceedingly promising approach that has remarkably improved the overall survival for patients

DRUG AND SINGLE-CELL GENE EXPRESSION INTEGRATION IDENTIFIES SENSITIVE AND RESISTANT GLIOBLASTOMA CELL POPULATIONS.

Glioblastoma (GBM) remains the most common and lethal adult malignant primary brain cancer with few treatment options. A significant issue hindering GBM therapeutic development is intratumor heterogeneity. GBM tumors contain neoplastic cells within a spectrum of different transcriptional states. Identifying effective therapeutics requires a platform that predicts the differential sensitivity and resistance of these states to various treatments. Here, we developed a novel framework, ISOSCELES (Inferred cell Sensitivity Operating on the integration of Single-Cell Expression and L1000 Expression

Premature ovarian insufficiency: decoding oxidative stress, metabolic pathways, epigenetic regulation, and the therapeutic role of yoga.

Premature ovarian insufficiency (POI) is a heterogeneous multifactorial disorder characterized by amenorrhoea, hypoestrogenism and elevated serum gonadotropins in women below 40 years. Genetic predisposition, infections, iatrogenic, autoimmune conditions and endocrinopathies are few of the known causes of POI, but majority cases of POI are idiopathic which is attributed to be due to environmental factors, epigenetic changes, oxidative DNA damage or faulty DNA repair mechanisms. These epigenetic changes and cumulative oxidative DNA damage can pass transgenerationally to subsequent generations.

The Tumor Microenvironment as a Therapeutic Target in Cutaneous T Cell Lymphoma.

Cutaneous T cell lymphomas (CTCLs) are a heterogeneous group of non-Hodgkin lymphomas, with mycosis fungoides and Sézary syndrome being the two common subtypes. Despite the substantial improvement in early-stage diagnosis and treatments, some patients still progress to the advanced stage with an elusive underpinning mechanism. While this unsubstantiated disease mechanism coupled with diverse clinical outcomes poses challenges in disease management, emerging evidence has implicated the tumor microenvironment in the disease process, thus revealing a promising therapeutic potential of targeting t

Clinical Disease Measures in Generalized Pustular Psoriasis.

Generalized pustular psoriasis (GPP) is a rare neutrophilic skin condition characterized by episodes of widespread eruption of sterile macroscopic pustules that can be associated with systemic inflammation. The rarity of GPP and its heterogeneous cutaneous and extracutaneous symptoms pose considerable challenges to the development and adoption of comprehensive accurate disease measures for the routine clinical assessment of disease severity and the evaluation of new treatments in clinical trials. Psoriasis disease measures remain among the most commonly used methods for evaluating patients wit

Physiologically Based Pharmacokinetic Modeling of Nanoparticle Biodistribution: A Review of Existing Models, Simulation Software, and Data Analysis Tools.

Cancer treatment and pharmaceutical development require targeted treatment and less toxic therapeutic intervention to achieve real progress against this disease. In this scenario, nanomedicine emerged as a reliable tool to improve drug pharmacokinetics and to translate to the clinical biologics based on large molecules. However, the ability of our body to recognize foreign objects together with carrier transport heterogeneity derived from the combination of particle physical and chemical properties, payload and surface modification, make the designing of effective carriers very difficult. In t

The Revival of Surgery in Crohn's Disease-Early Intestinal Resection as a Reasonable Alternative in Localized Ileitis.

Crohn's disease (CD) represents a heterogeneous and complex disease with no curative therapeutic option available to date. Current therapy is mainly antibody-based focusing on the immune system while other treatment alternatives such as surgery are considered to be "last options". However, medical therapy for CD results in mild to severe side effects in a relevant amount of patients and some patients do not respond to the medication. Following that, quality of life is often significantly reduced in this patient cohort, thus, therapeutic alternatives are urgently needed. Updated evidence has re

Natural History and Disease Burden of Neurofibromatosis Type 1 with Plexiform Neurofibromas: A Systematic Literature Review.

Neurofibromatosis type 1 (NF1) is an incurable genetic condition that frequently includes the development of plexiform neurofibromas (PNs) in patients. A systematic literature review was conducted to identify data on the natural history, disease burden, and treatment patterns among patients diagnosed with NF1 and PN, as well as to identify evidence gaps in these areas. MEDLINE and MEDLINE In-Process, Embase, and Cochrane Library Searches were searched using predefined terms. Potential references underwent two phases of screening by two independent researchers. The Genetic Evolution of Treatment-Resistant Cutaneous, Acral, and Uveal Melanomas.

PURPOSE: Melanoma is a biologically heterogeneous disease composed of distinct clinicopathologic subtypes that frequently resist treatment. To explore the evolution of treatment resistance and metastasis, we used a combination of temporal and multilesional tumor sampling in conjunction with whole-exome sequencing of 110 tumors collected from 7 patients with cutaneous (n = 3), uveal (n = 2), and acral (n = 2) melanoma subtypes. EXPERIMENTAL DESIGN: Primary tumors, metastases collected longitudinally, and autopsy tissues were interrogated. All but 1 patient died because of melanoma progression.

Systemic Mastocytosis: Clinical Update and Future Directions.

Systemic mastocytosis (SM) is defined as the accumulation of abnormal mast cells (MC) in 1 or more extracutaneous tissues. Symptoms are due to either MC activation or organ infiltration and vary depending on disease subtype. More benign forms of SM, such as indolent SM, result in a life expectancy similar to the general population, while more aggressive subtypes, such as MC leukemia (MCL), have a median survival measured on the order of months. Treatment of indolent SM is directed at controlling the symptoms associated with MC activation. In advanced forms, such as aggressive SM and MCL, agent

Orthostatic hypotension associated with baroreceptor dysfunction: treatment approaches.

Orthostatic hypotension (OH) is a relatively common heterogeneous and multifactorial disorder often caused by autonomic dysfunction. This condition has a deleterious impact on quality of life and contributes to higher mortality rates. Supine hypertension is very common in patients with autonomic failure, limits the use of pressor agents, and can result in end-organ damage. Current recommendations on the optimal management of these patients are based on expert opinion and poor-quality small cross-sectional studies including patients with primary autonomic failure and severe orthostatic hypotens

A challenging diagnosis for potential fatal diseases: recommendations for diagnosing acute porphyrias.

Acute porphyrias are a heterogeneous group of metabolic disorders resulting from a variable catalytic defect of four enzymes out of the eight involved in the haem biosynthesis pathway; they are rare and mostly inherited diseases, but in some circumstances, the metabolic disturbance may be acquired. Many different environmental factors or pathological conditions (such as drugs, calorie restriction, hormones, infections, or alcohol abuse) often play a key role in triggering the clinical exacerbation (acute porphyric attack) of these diseases that may often mimic many other more common acute medi

Optimizing the management of patients with myelofibrosis.

Myelofibrosis (MF) is a rare myeloproliferative neoplasm of the bone marrow associated with shortened survival. The disease is characterized by splenomegaly, cytopenias, and multiple disease-related symptoms that reduce quality of life. The clinical management of MF can be challenging because of its heterogeneous presentation and disease course. Therefore, knowledge of the underlying pathology and clinical manifestations of MF is needed. Ruxolitinib, a Janus kinase (JAK) 1 and 2 inhibitor, is the first therapy to be approved by the U.S. Food and Drug Administration for intermediate- or high-ri

[Importance of local skin treatments during radiotherapy for prevention and treatment of radio-induced epithelitis].

Radio-epithelitis represents a common problem, for which treatments are characterized by a great heterogeneity. The present review of literature focuses on data referenced in Pubmed((c))/Medline((c)) and published in French/English. Despite a real preclinical rationale, aloe vera and trolamine failed to demonstrate any benefit in the prophylactic settings. In a prospective assessment phase III assessment, Calendula Officinalis was shown to be superior to trolamine for the prevention of radio-epithelitis. In the curative settings, sucrafalte failed to demonstrate any benefit. The benefit of der

Selective cord coagulation in acardiac twins.

The twin reverse arterial perfusion (TRAP) sequence is a serious complication of monochorionic multiple pregnancies in which the affected twin is reversely perfused from the healthy co-twin, resulting in severely abnormal or absent cardiac development, severe malformations, and massive hydrops fetalis. The acardiac twin threatens the survival of the pump twin either by increasing the risk of congestive cardiac failure or by the development of polyhydramnios, preterm premature rupture of membranes, preterm labour and premature delivery. A wide spectrum of surgical interventions has been applied

The diagnostic value of ATP testing in patients with unexplained syncope.

A minority of patients with unexplained syncope has an increased susceptibility to adenosine triphosphate (ATP) injection. In these 'hypersensitive' patients, owing to its powerful cardiac and hypotensive effects, endogenous adenosine released under physiological and pathological conditions could trigger bradycardia and/or hypotension and cause syncope. This hypothesis still needs to be proven. However, there is some evidence that the ATP test identifies a group of patients with otherwise unexplained syncope with definite clinical features, absence of structural heart disease and benign progno

Resistant pneumococci: a renewed threat in respiratory infections.

The emergence of penicillin-resistant Streptococcus pneumoniae (PRSP) is an international problem which has been compounded by the development of high-level, multiple resistance to other beta-lactam antibiotics. Resistance develops in a 'step-wise' but unpredictable manner due to the mutation of penicillin-binding proteins (PBP). This results in a high degree of heterogeneity between bacterial strains. Such mutations can result in the rapid emergence of antibiotic resistance, including extended-spectrum cephalosporins, with reported minimum inhibitory concentration (MIC) values of up to 32 mg/

[Neonatal cholestasis: the viewpoint of the pediatrician].

The heterogeneous nature of the diseases causing neonatal cholestasis, and the wide range of overlap among the specific entity frequently create serious challenges in their diagnosis. Moreover, the spectrum of correlated nutritional abnormalities in children with chronic cholestasis dictates a complex approach to their management. Nevertheless, pediatricians should be aware of the importance to recognize rapidly a specific treatable metabolic, infectious or malformative disease, in order to institute an early, specific, and effective treatment. However, despite all the still unanswered questio
