Targeted nanosponge-based treatment for pulmonary hypertension: A novel approach.

Pulmonary Hypertension (PH) is a progressive and potentially fatal condition marked by high pulmonary artery pressure, resulting in heart failure and reduced oxygenation. Despite advancements in treatments, therapeutic options for PH remain limited, particularly in cases resistant to conventional therapies. In biomedical research, nanotechnology has become a potential area of study, presenting novel approaches to drug delivery and tissue targeting. Nanosponges, a class of nanoparticles with porous structures, have gained attention for their ability to encapsulate therapeutic agents, enhance dr

Acute Left Ventricular Dysfunction After Mitral Transcatheter Edge-to-Edge Repair.

BACKGROUND: Mitral transcatheter edge-to-edge repair (M-TEER) is an established treatment for severe mitral regurgitation. However, acute left ventricular (LV) dysfunction may complicate the procedure through mechanisms with distinct implications. CASE SUMMARY: An 81-year-old man with severe primary mitral regurgitation underwent elective M-TEER. Immediately after clip deployment, he developed acute LV dysfunction, initially managed as afterload mismatch (AM) with vasodilators. The clinical evolution was associated with apical ballooning on echocardiography, with electrocardiographic changes

A Rare Case Report of Contrast Media-induced Sympathetic Crashing Acute Pulmonary Edema.

INTRODUCTION: Sympathetic crashing acute pulmonary edema (SCAPE), also known as flash pulmonary edema or hypertensive acute heart failure, is a critical condition characterized by a rapid escalation of sympathetic outflow, excessive afterload, and worsening heart failure. Although rare, contrast media-induced pulmonary edema is a severe adverse reaction, occurring in 0.001-0.008% of patients receiving intravenous contrast and accounting for 10-20% of lethal contrast reactions. CASE REPORT: A 70-year-old male developed acute respiratory distress shortly after undergoing an outpatient, contrast

[History and Current Status of Direct or Combined Revascularization Surgery for Moyamoya Disease].

We reviewed the history and development of revascularization surgery for moyamoya disease, particularly STA-MCA bypass. In the early stages, medical treatments, such as vasodilators, were ineffective. In the 1970s, indirect surgical methods were introduced but have shown limited success. Direct bypass techniques have evolved with advancements in microsurgery, and STA-MCA bypass has become the standard treatment. Surgery improves the collateral blood flow and reduces the risk of stroke, especially in patients with ischemia. Despite being generally safe, perioperative complications, such as cere

Anesthetic Management in Combined Off-Pump CABG and EC-IC Bypass for CAD and Moyamoya Disease.

BACKGROUND: Surgical treatment of moyamoya disease combined with coronary heart disease presents unique anesthetic challenges. CASE SUMMARY: A 56-year-old woman underwent combined off-pump coronary artery bypass grafting and extracranial-intracranial arterial bypass surgery. The challenges in this surgery included maintaining cerebral oxygenation (regional cerebral oxygen saturation: >75% of baseline), controlled heart rate (60-70 beats/min), lung-protective ventilation (partial arterial pressure of carbon dioxide: 40-45 mm Hg), and mean arterial pressure >70 mm Hg. The patient experienced 2

Outcomes of Venovenous Extracorporeal Membrane Oxygenation for Pneumocystis jirovecii Pneumonia: A Multicenter Retrospective Case Series.

Pneumocystis jirovecii pneumonia (PCP) is a life-threatening opportunistic infection increasingly recognized among non-HIV immunocompromised patients. In severe cases progressing to acute respiratory distress syndrome, venovenous extracorporeal membrane oxygenation (VV-ECMO) may serve as a rescue therapy. We conducted a retrospective multicenter review of 10 adult patients with proven or probable PCP who received VV-ECMO between 2017 and 2024. Seven of 10 patients survived to discharge, including all three HIV-positive patients and 4 of 7 non-HIV immunocompromised patients. The mean Respirator

Hydralazine-Induced ANCA Vasculitis Presenting With Pericarditis: A Novel Case and Literature Review.

Hydralazine, a commonly used arterial vasodilator for managing congestive heart failure and hypertension, is known to be associated with drug-induced lupus and, less frequently, antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). Drug-induced AAV typically carries a favorable long-term prognosis and is not commonly linked to cardiovascular or ocular involvement. Pericarditis cases associated with hydralazine have not been previously reported. We present a rare case involving an 85-year-old woman on long-term hydralazine therapy, initially presenting with acute lobar pneumon

Single-cell transcriptomics reveals aberrant skin-resident cell populations and identifies fibroblasts as a determinant in rosacea.

Rosacea is a chronic inflammatory skin disorder, whose underlying cellular and molecular mechanisms remain obscure. Here, we generate a single-cell atlas of facial skin from female rosacea patients and healthy individuals. Among keratinocytes, a subpopulation characterized by IFNγ-mediated barrier function damage is found to be unique to rosacea lesions. Blocking IFNγ signaling alleviates rosacea-like phenotypes and skin barrier damage in mice. The papulopustular rosacea is featured by expansion of pro-inflammatory fibroblasts, Schwann, endothelial and macrophage/dendritic cells. The frequenci

Cracking the code of the cardiovascular enigma: hPSC-derived endothelial cells unveil the secrets of endothelial dysfunction.

Endothelial dysfunction is a central contributor to the development of most cardiovascular diseases and is characterised by the reduced synthesis or bioavailability of the vasodilator nitric oxide together with other abnormalities such as inflammation, senescence, and oxidative stress. The use of patient-specific and genome-edited human pluripotent stem cell-derived endothelial cells (hPSC-ECs) has shed novel insights into the role of endothelial dysfunction in cardiovascular diseases with strong genetic components such as genetic cardiomyopathies and pulmonary arterial hypertension. However,

New and future heart failure drugs.

In the past decade, our understanding of heart failure pathophysiology has advanced significantly, resulting in the development of new medications such as angiotensin-neprilysin inhibitors, sodium-glucose cotransporter-2 inhibitors and oral soluble guanylate cyclase stimulators. Backed by positive findings from large randomized controlled trials, recommendations for their use were recently included in the 2022 AHA/ACC/HFSA guidelines and 2023 ESC guidelines for management of heart failure. Promising drugs for future heart failure treatment include agents that modulate the neurohormonal system,

Dissolving microneedles loaded with nimodipine for prevention of sleep disorders at a high altitude.

Sleep disorders are one of the most common acute reactions on the plateau, which can cause serious complications. However, there is no effective and safe treatment currently available. Nimodipine (NMD) is a dihydropyridine calcium channel blocker with neuroprotective and vasodilating activity, mainly used for the treatment of ischemic brain injury. Commercial oral or injectable NMD formulations are not a good option for central neuron diseases due to their poor brain delivery. In this study, nimodipine dissolving microneedles (NDMNs) were prepared for the prevention of sleep disorders caused b

[Research progress on image-based calculation of coronary artery fractional flow reserve].

Coronary artery fractional flow reserve (FFR) is a critical physiological indicator for assessment of impaired blood flow caused by coronary artery stenosis. The wire-based invasive measurement of blood flow pressure gradient across stenosis is the gold standard for clinical measurement of FFR. However, it has the risk of vascular injury and requires the use of vasodilators, increasing the time and overall cost of interventional examination. Coronary imaging is playing an important role in clinical diagnosis of stenotic lesions, evaluation of severity of lesions, and planning of therapies. In

Functional coronary angiography for the assessment of the epicardial vessels and the microcirculation.

Over the last decade, steady progress has been made in the ability to assess coronary stenosis relevance by merging computerised analyses of angiograms with fluid dynamic modelling. The new field of functional coronary angiography (FCA) has attracted the attention of both clinical and interventional cardiologists as it anticipates a new era of facilitated physiological assessment of coronary artery disease, without the need for intracoronary instrumentation or vasodilator drug administration, and an increased adoption of ischaemia-driven revascularisation. This state-of-the-art review performs

Nitric oxide and thiols: Chemical biology, signalling paradigms and vascular therapeutic potential.

Nitric oxide (•NO) interactions with biological thiols play crucial, but incompletely determined, roles in vascular signalling and other biological processes. Here, we highlight two recently proposed signalling paradigms: (1) the formation of a vasodilating labile nitrosyl ferrous haem (NO-ferrohaem) facilitated by thiols via thiyl radical generation and (2) polysulfides/persulfides and their interaction with •NO. We also describe the specific (bio)chemical routes in which •NO and thiols react to form S-nitrosothiols, a broad class of small molecules, and protein post-translational modificatio

Enteric tube position on preoperative radiographs predicts clinical outcomes in neonatal congenital diaphragmatic hernia with and without prenatal diagnosis.

OBJECTIVE: Congenital Diaphragmatic Hernia (CDH) is diagnosed prenatally in ~60% of cases. Prenatal measures typically guide management and prognostication. Simple postnatal prognosticators are needed when prenatal diagnosis is lacking. We hypothesized that preoperative orogastric tube (OGT) tip position relative to the contralateral diaphragm correlates with defect severity, resource utilization, and clinical outcomes regardless of diagnostic status. STUDY DESIGN: 150 neonates with left-posterolateral CDH were analyzed. Impact of intrathoracic and intraabdominal preoperative tip position on

Chorioretinal venous anastomosis for non-ischemic retinal vein occlusion.

Retinal vein occlusion (RVO) refers to the occlusion of the central retinal vein or primary and secondary branches caused by multiple factors. Clinical treatments for it include intravitreal or systemic vasodilator application, local usage of steroids and NSAID (non-steroidal anti-inflammatory drugs), thrombolysis, hemodilution, retinal laser photocoagulation, vitrectomy with vascular sheath incision, chorioretinal venous anastomosis (CRVA), and so on. At present, most treatments are aimed at RVO complications, while chorioretinal vein anastomosis can fundamentally reflux retinal vein blood th

The PASTIS trial: Testing tadalafil for possible use in vascular cognitive impairment.

INTRODUCTION: There are few randomized clinical trials in vascular cognitive impairment (VCI). This trial tested the hypothesis that the PDE5 inhibitor tadalafil, a widely used vasodilator, increases cerebral blood flow (CBF) in older people with symptomatic small vessel disease, the main cause of VCI. METHODS: In a double-blind, placebo-controlled, cross-over trial, participants received tadalafil (20 mg) and placebo on two visits ≥7 days apart (randomized to order of treatment). The primary endpoint, change in subcortical CBF, was measured by arterial spin labelling. RESULTS: Tadalafil inc

Alterations of sodium-hydrogen exchanger 1 function in response to SGLT2 inhibitors: what is the evidence?

This review summarizes and describes the current evidence addressing how sodium-glucose cotransporter 2 (SGLT2) inhibitors alter the function of sodium-hydrogen exchanger 1 (NHE-1), in association with their protective effects against adverse cardiovascular events. In the heart, SGLT2 inhibitors modulate the function of NHE-1 (either by direct inhibition or indirect attenuation of protein expression), which promotes cardiac contraction and an enhanced energy supply, in association with improved mitochondrial function, reduced inflammation/oxidative/endoplasmic reticulum stress, and attenuated

A Novel Role of Semaphorin 3C in Modulating Systemic and Renal Hemodynamics.

BACKGROUND: Alterations of renal hemodynamics play an essential role in renal homeostasis and kidney diseases. Recent data indicated that semaphorin 3C (SEMA3C), a secreted glycoprotein involved in vessel development, can modulate renal vascular permeability in acute kidney injury, but whether and how it might impact systemic and renal hemodynamics is unknown. OBJECTIVES: The objective of the study was to explore the effect of SEMA3C on systemic and renal hemodynamics. METHODS: SEMA3C recombinant protein was administered intravenously in two-month-old wild-type mice, and the variations of me
