An alternative approach for repair of supracardiac and infracardiac total anomalous pulmonary venous drainage in neonates and infants: superior approach with caval transection.

Recently, the superior approach with transaction of the superior vena cava was introduced for the repair of supracardiac anomalies in adults. We developed a bidirectional right-angle venous cannula and placed it within the innominate vein to make the modified superior approach with the superior caval transection suitable for neonates and tiny infants. We applied this modified superior approach for the repair of infracardiac forms of total anomalous pulmonary venous drainage.

An Unusual Case of Severe Stenosis of the Coronary Sinus Ostium in Association With Double Inlet Left Ventricle.

We present a patient with complex single ventricle physiology who was subsequently diagnosed with atresia of the coronary sinus ostium in the setting of myocardial dysfunction following operative palliation. Although a rare cardiac defect, awareness is important as the coronary venous system will often drain to a left superior vena cava (LSVC). If the LSVC is ligated without knowing of this defect, cardiac dysfunction and death can occur.

Symptomatic deep venous thrombosis of the lower extremity in a pregnant woman successfully treated with endovascular procedures.

We report a pregnant woman with symptomatic deep venous thrombosis of the lower extremity distributing in the left iliac, femoral, popliteal and calf veins. Systemic anticoagulation therapy failed; however, treatment was successful through a combination of various endovascular procedures, including catheter-directed thrombolysis. During the therapies, an optional inferior vena cava filter was implanted to protect against the development of pulmonary embolism.

Normal Drainage, Abnormal Connection: Partial Anomalous Pulmonary Venous Connection and Sinus Venosus Atrial Septal Defect With a Net Right-to-Left Shunt.

A seven-month-old girl with partial anomalous pulmonary venous connection and atrial septal defect underwent cardiac magnetic resonance for further evaluation. Anatomical images and flow quantification confirmed the diagnosis and demonstrated a commitment of the superior vena cava to the left atrium, resulting in a right-to-left shunt and pulmonary/systemic blood flow ratio of 0.85. The findings were confirmed during surgery.

Off-pump Kawashima operation.

We describe the technique of Kawashima repair without using cardiopulmonary bypass in 6 consecutive patients of single ventricle morphology with interrupted inferior vena cava and pulmonary stenosis. No patient had central nervous system disorder or chylothorax. The off-pump technique is feasible in Kawashima repair. This technique avoids use of cardiopulmonary bypass, thereby preventing its deleterious effects, which is also economical.

Epithelioid angiomyolipoma: imaging appearances.

Epithelioid angiomyolipoma is a recently described rare variant of renal angiomyolipoma. It can occur in patients with or without tuberous sclerosis, and may potentially be malignant. We report the imaging findings from two cases of epithelioid angiomyolipoma: the first in a patient with tuberous sclerosis complex, arising in a horse-shoe kidney and growing into the inferior vena cava and right atrium; the second in a 62-year-old hypertensive man.

[Fatal hydatid pulmonary embolism. Report of two cases].

Hydatid pulmonary embolism is rare but serious. The cause is usually the rupture of a cyst in the right cardiac chambers and less frequently the opening of an hepatic cyst into the venous circulation. We report two cases of hydatid pulmonary embolism following the rupture of a cyst in the right atrial cavity in one case and fistulisation of an hepatic cyst into the inferior vena cava in the second case. The course is dramatic in the two cases.

Unusual case of right-sided May-Thurner syndrome and review of its management.

May-Thurner syndrome is a phenomenon commonly described as an acquired stenosis of the left common iliac vein as a result of right common iliac artery compression. We report an unusual case of right-sided May-Thurner syndrome in a patient found to have a left-sided inferior vena cava. We also review the management of this patient using angioplasty, intraoperative thrombolysis, and endoluminal stent placement.

[Thoracic involvement in Behçet's vasculitis].

Thoracic involvement of Behcet's disease is unusual but serious. It is related to the well known vascular tropism of the disease. It may involve the superior vena cava, pulmonary arteries, aorta and subclavian vessels. Imaging is useful for diagnosis and assess the degree of thoracic involvement. CT scan and MRI are obviously more accurate than angiography. The spectrum of thoracic manifestations of the disease is presented based on a review of 22 cases.

Direct reimplantation technique via a right minithoracotomy for scimitar syndrome: a case report.

Scimitar syndrome (SS) is a rare congenital cardiac anomaly defined by an anomalous right pulmonary vein draining of the right lung into the inferior vena cava. We describe a direct reimplantation technique and atrial septal defect closure using cardiopulmonary bypass via a right minithoracotomy on a 24 year-old female SS patient who had an accompanying sinus venous atrial septal defect.

[Pheochromocytoma with thrombus in cava].

Pheochromocytoma, a paraganglioma of suprarenal location, is a catecholamine-secreting chromaffin cell tumour. Spread of these tumours to the vena cava is rare and the thrombus only reaches the right atrium in exceptional cases. We present the case of a patient who, without previous symptomatology, presented with a clinical picture of multiorganic dysfunction with primary manifestation of a suprarenal tumour with vascular spread to the right atrium affecting the right suprahepatic vein.

Combined aortic and inferior vena caval coarctation --an unknown association.

Atypical coarctation of the aorta refers to narrowing (discrete or diffuse) that involves the thoracic or abdominal segments. Such a phenomenon has never been reported in the inferior vena cava. This is a report of combined coarctation of the abdominal aorta and inferior caval vein in a still birth with sirenomelia, ambiguous genitalia, and other extracardiac anomalies. The maternal serum was positive for Toxoplasma and CMV. Evidence of phlebitis was present in the inferior caval vein.

Successful treatment of acute Budd-Chiari syndrome with percutaneous transluminal angioplasty.

A 26-year-old man developed progressive, massive ascites and hematemesis due to rupture of esophageal varices. Combination diagnostic modalities of color doppler ultrasonography, enhanced computed tomography, and magnetic resonance imaging led to the case being diagnosed as acute Budd-Chiari syndrome with severe stricture of the intrahepatic inferior vena cava. Percutaneous transluminal angioplasty this resulted in great improvement of the clinical manifestations.

Lumbar azygos vein producing an azygos lobe in the right lung.

The continuation of the azygos vein with the inferior vena cava and a well demarcated azygos lobe of the right lung were detected during routine dissection of a 65-years-old white male cadaver. The azygos arch was found dilated and malpositioned to create an aberrant azygos fissure in the right superior lobe which, was occupied by a fold of parietal pleura. The developmental and clinical significance of the azygos lobe is discussed.

Pulmonary aplasia: MR angiography findings.

A 26-year-old woman presented with absence of the left lung. A diagnosis of pulmonary aplasia (main bronchus ending in a blind pouch) was established by bronchoscopy. Although a CT scan and MR imaging suggested the correct diagnosis, MR angiography enabled optimal demonstration of the vascular changes due to the anomaly (single main pulmonary artery and vein) and an associated anomaly (azygous continuation of the inferior vena cava).

Detachment of balloon in an infant: a rare complication of balloon pulmonary valvuloplasty.

A 3-month-old infant with severe tetralogy of Fallot underwent percutaneous balloon valvuloplasty. After the successful dilatation, the balloon could not be deflated completely and the partial deflated balloon was totally peeled off from the catheter at the junction of inferior vena cava and right common iliac vein when we attempted to move it out. The skin of the balloon was successfully retrieved with a snared guidewire without sequel.

Short report: a case of fibrosing mediastinitis caused by Wuchereria bancrofti.

Wuchereria bancrofti is a mosquito-borne filarial nematode that commonly invades lymphatic vessels. Common clinical manifestations include elephantiasis, orchitis, epididymitis, and chyluria. This report concerns an Egyptian man who developed superior vena cava syndrome secondary to a mediastinal mass that was found to contain a filarial nematode consistent with W. bancrofti. This is the first case, to our knowledge, of this parasite causing fibrosing mediastinitis.

[Intraoperative pulmonary embolism of a hydatid membrane].

Only four cases of per-operative pulmonary embolus of parasites during surgery for hydatic cysts of the liver have been reported. Embolus usually occurs in patients who already have lung metastasis of hepatic cysts, suggesting a hepatic cyst in direct communication with the vena cava or suprahepatic vein. We report the first case which occurred in a patient in which there was no evidence of such communication.
