What is the best method of treatment for labyrinthine fistulae caused by cholesteatoma?

A series of 19 patients with a labyrinthine fistula caused by cholesteatoma was analysed. Two patients presented with acute suppurative labyrinthitis and meningitis. A canal wall down procedure was performed in all but one patient. Of the 17 patients with preserved inner-ear function, the cholesteatoma matrix over the fistula was removed in 10 resulting in severe sensorineural hearing loss in one of these. The matrix was left in situ in 7 patients and hearing was maintained or improved in all of them. Post-operatively, 2 patients suffered from vestibular disturbances, the matrix covering the f

Recovery mechanism of postural disturbance after vestibular neurectomy.

Sectioning of the unilateral vestibular nerve of the bull frog resulted in tilting of the head and the body. The angle of head tilt was periodically measured. The angle returned to 0 degree in an average of 7.3 weeks. A morphological study demonstrated normal structure of the sensory epithelia and a well-regenerated nerve bundle. Also, discrete action potentials could be recorded from the nerve centrally to the cut site of the vestibular nerve. These results indicate that recovery from the tilting involves two mechanisms, one being central compensation and the other reactivation of the sensory

Effects of lidocaine infusion in cats after unilateral labyrinthectomy.

In the auditory/vestibular system, intravenous lidocaine hydrochloride administration has been reported to provide transient relief from severe tinnitus, reduce dizziness and emesis accompanying Meniere's disease, and sometimes improve audiometric thresholds in sensorineural hearing loss. In this study, the labyrinth was destroyed unilaterally in a series of cats. Animals constantly fell and demonstrated prominent contralateral nystagmus and a rotary motion of the head. Within four hours of a 4-mg/kg intravenous lidocaine hydrochloride injection, the cats were able to ambulate freely without f

Clinical manifestations of otological syphilis.

One patient with early otological syphilis and five patients with late otological syphilis are described including audiological and serological findings. In patients with late syphilis poorer than expected speech discrimination scores relative to the pure tone audiograms were noted. Profound hypo-activity of vestibular responses to caloric stimulation was present in the cases of late otological syphilis. Routine non-treponemal serological tests were not sensitive enough to establish the diagnosis. It is recommended that more sensitive treponemal serological tests such as the FTA-ABS should be

The role of the psychiatrist in the general hospital--II. Psychiatric patients with somatic complaints.

In part I, one of the important roles of the psychiatrist in the general hospital was pointed out with respect to the management and treatment of psychiatric patients with somatic complaints (PPSC) referred from other nonpsychiatric departments of the hospital. Part II deals with the way in which a PPSC is brought to the psychiatrist, and the basic principles concerning the initial interview which is crucial to the management and treatment of such patients. The principles for the initial interview are to clarify: (1) the processes through which the patient came to the psychiatrist, (2) the pat

Comparison of side effects with coated lithium carbonate tablets and lithium sulphate preparations giving medium-slow and slow-release.

Side effects with coated, rapidly dissolving lithium carbonate tablets, medium-slow-release and slow release lithium sulphate tablets, all administered twice a day, were compared in a double-blind, cross-over study in 27 patients who had only used lithium carbonate tablets. The relation between medication and meals was not standardized. The repidly dissolving tablets and the medium-slow-release tablets had a similar tendency to produce side effects, while slow-release tablets more often produced purgative side effects than the other two preparations did.

Therapy in hereditary angioneurotic oedema.

Two branches of a family suffering from hereditary angioneurotic oedema underwent trials of therapy of ε-amino caproic-acid (EACA) to ascertain the optimum dosage required to alleviate symptoms without giving rise to unpleasant side effects. It was found that children under 11 years tolerated 3 g/day and patients over 11 years 6 g/day without side effects, but with incomplete control of symptoms in some of the patients. However, if the dose was doubled for a period of less than 4 days during times of expected and experienced trauma, better control was achieved and unpleasant muscle cramps were
