To date, only one group has reported its clinical experience with colchicine therapy. This study included a randomized clinical trial of 22 patients and an open study of 53 patients. The beneficial effect of 1 mg colchicine daily for five days a week has been promising but not convincing. Corticosteroid therapy for the treatment of alcoholic hepatitis was evaluated in nine short-term randomized clinical trials that included 150 treated and 161 control patients with a mortality rate of 37 and 44%, respectively. The propensity to develop alcoholic cirrhosis is probably, at least in part, genetically determined. A striking similarity exists histologically between perhexiline and alcohol-related hepatitis and both are potentially precirrhotic lesions. Liver damage due to perhexiline is associated with impaired drug oxidation capacity which is genetically determined and tested by use of debrisoquine. Peripheral blood T lymphocytes and T lymphocytes subsets have been quantified by an indirect immunofluorescence technique using monoclonal antibodies, in 10 patients with fatty liver, 8 with acute alcoholic hepatitis (AAH), 10 with inactive cirrhosis and 7 with cirrhosis and AAH. Twenty normal subjects were studied as controls. As compared to controls (1.81 +/- 0.56 10(9)/l), we found a reduced number of peripheral T lymphocytes (OKT3+) in patients with inactive cirrhosis (0.98 +/- 0.45, p less than 0.001) and in patients with cirrhosis and AAH (1.22 +/- 0.51, p less than 0.02). The Michigan Alcoholism Screening Test (MAST) was administered to 214 admissions to a maximum security ward of a prison hospital. Over 70.7% of the patients had scores greater than 5 and 57.8% had scores greater than 10, indicating that they engaged in problem drinking or alcohol misuse before admission. Older patients had higher scores, and a small number of patients with no criminal involvement had lower scores.

When processed by the rapid Golgi method, a significant reduction (P less than 0.002) of the dendritic arborization of Purkinje cells located at the tips of the folia of the rostral vermis was demonstrated in four human cases of cerebellar atrophy, related to chronic alcohol consumption. Hypophosphatemia is frequently found in patients with alcoholic cirrhosis, and its postulated causes include inadequate phosphorus (PO4) intake, reduced gastrointestinal absorption, and inappropriate PO4 loss in the urine. Keratometry was performed on 30 fetal alcohol syndrome (FAS) and fetal alcohol effect (FAE) children. Horizontal keratometric measurements that are 46.00 diopters or greater appear to be an objective landmark of FAS. Keratometry is recommended as a routine test in evaluating suspected FAS children and appears to have value in screening the general school population for FAS.

Cytotoxicity of peripheral blood lymphocytes for autologous hepatocytes has been studied in 18 patients with different histological types of alcoholic liver disease. Cytotoxicity was significantly increased in five of ten patients with cirrhosis and/or alcoholic hepatitis but in only one of eight with fatty infiltration or minor histological changes. Autopsy was performed on a total of 48 motorcycle accident victims (39 riders and 9 passengers) in Cape Town during 1982. Thirty-nine (81%) were under 30 years of age, while 61,3% of the riders tested were found to have alcohol in their blood, 42% having a blood alcohol level of more than 0,10 g/dl.

Ejaculates from 30 patients with alcohol toxic delirium have been tested. We found a decreased number of hypospermia (n = 8) and a reduced sperm density of 18 mill.spermatozoa/ml. Judging sperm density, morphology and motility, 29 sperm-counts are severe pathological. The disturbance of spermiogeneses is most pronounced (2 = 0.01), when increased FSH-values (n = 10) and acute alcohol toxical hepatitis (n = 4) were found, but is less dependent on the drinking habits according to Jellinek or on increased oestradiol values (n = 6) or increased CK-values. Three-month-old male Wistar rats exposed intermittently (5 days weekly, 6 hr/day) to furfuryl alcohol vapor at 1 (25 ppm), 2 (50 ppm), or 4 mumol/liter (100 ppm) for 4 to 16 weeks were lighter than controls at the end of the experiment at the two higher concentration levels. Interaction between alcohol and zopiclone or flunitrazepam as well as the residual effects of both these hypnotics were studied in 20 normal male volunteers who received each 3 of 6 different drug and drink combinations according to a balanced incomplete block design as follows: placebo, zopiclone (7.5 mg), or flunitrazepam (2 mg) was administered double-blind in identical capsules at 23.00 h, and 0.5 g/kg body weight of alcohol or placebo alcohol was given at 08.30 h the following morning. Psychomotor skills of the volunteers were measured before drinking and 30 min, 1.5 h and 2.5 h after it.
