Contour uncertainty assessment for MD-omitted daily adaptive online head and neck radiotherapy.

BACKGROUND AND PURPOSE: Daily online adaptive radiotherapy (DART) increases treatment accuracy by crafting daily customized plans that adjust to the patient's daily setup and anatomy. The routine application of DART is limited by its resource-intensive processes. This study proposes a novel DART strategy for head and neck squamous cell carcinoma (HNSCC), automizing the process by propagating physician-edited treatment contours for each fraction. MATERIALS AND METHODS: This study retrospectively analyzed 24 HNSCC patients treated with DART, encompassing 810 fractions. Both weekly and daily off

The terminal ileal vein approach: a novel and simple technique for locating the superior mesenteric vein and en bloc resection of the ileal mesentery in laparoscopic right hemicolectomy.

Laparoscopic right hemicolectomy (Lap-RHC) presents technical challenges due to the complex vascular anatomy of the mesentery, which increases the risk of intraoperative bleeding and complicates surgical navigation. Accurate identification of the superior mesenteric vein (SMV) is crucial for maintaining surgical safety and achieving optimal oncological outcomes. To address these challenges, this study proposes the terminal ileal vein (TIV) approach, a novel technique designed to facilitate precise SMV identification and enable en bloc resection of the ileal mesentery while preserving mesenteri

Assessing Pediatric Pelvic Fracture Patterns: New Insights and Unique Fracture Characteristics.

BACKGROUND: Pediatric pelvic ring fractures are different in biomechanics and anatomy compared with adults. Existing classification systems are insufficient in assessing the mechanical stability of these fractures due to a variety of reasons, leading to a potential underestimation of the injury severity, resulting in suboptimal treatment with the risk of long-term dysfunctions. This study aims to address this problem by comprehensively describing a cohort of patients with pediatric pelvic fractures, identifying specific pediatric pelvic fractures and patterns. METHODS: A retrospective cohort

Cerebrovascular Pressure Restoration Rates during Recanalisation with Aspiration and Stent-Retriever Thrombectomy for Acute Ischaemic Stroke: An in Vitro Study.

OBJECTIVE: Mechanical thrombectomy alters cerebrovascular hemodynamics in ways that remain poorly characterized. This in vitro study investigated the pressure restoration rate (PRR), a quantitative measure of how quickly pressure returns to baseline in an occluded vessel, during thrombectomy using aspiration and stent-retriever devices in different cerebrovascular anatomical configurations. METHODS: A cerebrovascular glass model with pulsatile flow was constructed to simulate good (symmetrically connected) and poor (unilateral) collateral anatomy. Pressure sensors were integrated into the sys

From Coronary Arteries to Lower Limbs: Advances in Drug-Eluting Bioresorbable Scaffolds for Treating Chronic Limb-Threatening Ischemia.

As the most severe manifestation of peripheral arterial disease, patients with chronic limb-threatening ischemia (CLTI) are at significant risk for adverse outcomes, such as amputation and mortality. A standard endovascular therapy for below the knee (BTK) intervention, percutaneous transluminal angioplasty (PTA), has shown high rates of restenosis and frequent re-interventions. Alternative technologies, such as drug-eluting stents (DES) and drug-coated balloons, have not consistently demonstrated superiority over PTA and have not received US regulatory approval. Esprit BTK, recently approved

Contemporary Outcomes of Pancreatic Head Resection for Chronic Pancreatitis.

OBJECTIVE: To evaluate perioperative morbidity and mortality outcomes in a large, contemporary series of patients undergoing pancreas head resection for chronic pancreatitis. SUMMARY BACKGROUND DATA: Select chronic pancreatitis (CP) patients benefit from pancreatic head resection, but contemporary data are sparse. Anatomy dictates selection of duodenum-preserving pancreatic head resection (DPPHR) or pancreatoduodenectomy (PD). We hypothesized that both DPPHR and PD are safe in select patients. METHODS: CP patients undergoing pancreas head resection from 2007-2023 at a high-volume institution

Physiologically Based Pharmacokinetic Modeling for Predicting Drug Levels After Bariatric Surgery: Vardenafil Exposure Before vs. After Gastric Sleeve/Bypass.

Bariatric surgery involves major changes in the anatomy and physiology of the gastrointestinal tract, which may alter oral drug bioavailability and efficacy. Phosphodiesterase-5 inhibitor (PDE5i) drugs are the first-line treatment of erectile dysfunction, a condition associated with a higher BMI. In this paper, we examine the PDE5i vardenafil for possible post-bariatric changes in solubility/dissolution and absorption. Vardenafil solubility was determined in vitro, as well as ex vivo using aspirated gastric contents from patients prior to vs. following bariatric procedures. Dissolution was tes

Effect of portal pressure gradient reduction on outcomes after transjugular intrahepatic portosystemic shunt in portal hypertension patients.

BACKGROUND: Portal hypertension (PHT), a complication of liver cirrhosis, is sometimes managed with transjugular intrahepatic portosystemic shunt (TIPS) to reduce portal pressure. Although effective, TIPS poses risks, including hepatic encephalopathy (HE). This study investigates whether a significant reduction in the portal pressure gradient (PPG) after TIPS improves outcomes in PHT patients. AIM: To evaluate the impact of post-TIPS PPG reduction on clinical outcomes and explore the relationship between PPG reduction and portal vein diameter. METHODS: This retrospective cohort study include

Anatomy of the sellar barrier: From magnetic resonance imaging to the operating room.

BACKGROUND: The sellar barrier concept concerns the correlation between the components of the pituitary fossa roof and the risk of intraoperative cerebrospinal fluid (CSF) leak during pituitary tumor surgery. Our team previously classified the sellar barrier according to its thickness on contrast-enhanced T1-weighted magnetic resonance imaging (MRI) sections into three subtypes: strong, mixed or weak. The purpose of this study is to complement the preoperative analysis of the sellar barrier with T2-weighted MRI sections to enhance our knowledge of the anatomical configuration of the sellar bar

A bias-resilient client selection analysis for federated brain tumor segmentation.

Brain tumor segmentation is difficult because of a number of technical problems, including its complex morphology, individual differences in anatomy, irregular shapes, overlapping, homogeneous gray matter and white matter intensity values, abnormalities that might not contrast with normal tissues, and the possibility of additional complications from various modalities. Expert radiologists may make different conclusions as a result of these difficulties. Regarding this, deep learning techniques, particularly CNN models, can be trained to handle these MRI artifacts and automatically extract feat

The Alternative Approach to the Lumbosacral Segment: The Right-Sided Oblique Lumbar Interbody Fusion Compared with Anterior Lumbar Interbody Fusion.

BACKGROUND: Anterior lumbar interbody fusion with dorsal percutaneous pedicle screw fixation (ALIF+D) is a well-described technique treating lumbosacral degenerative diseases. Mobilizing the common iliac arteries and veins during the ALIF+D approach may increase the risk of bleeding when the bifurcations are low. This study demonstrates that in such cases, the right-sided oblique lumbar interbody fusion with dorsal percutaneous pedicle screw fixation (OLIF+D) offers a novel alternative to the ALIF+D approach. METHODS: Twenty-one patients were operated on with the ALIF+D approach, and 20 patie
