Nitric oxide inhibition accelerates hypertension and induces perivascular inflammation in rats.

1. Inflammatory changes in peripheral arteries have been reported in animal models of hypertension. Whether they occur in cerebral arteries (CA) with hypertension induced by deprivation of endogenous nitric oxide (NO) remains unknown. 2. In the present study, we compared the arteriolar injury score (AIS) and perivascular inflammation in CA between hypertensive and normotensive rats following NO deprivation with the NO synthase inhibitor N(omega)-nitro-L-arginine methyl ester (L-NAME). Five-week-old male spontaneously hypertensive rats (SHR) and Wistar -Kyoto (WKY) rats were fed with L-NAME (1

Low-dose methotrexate as a risk factor for Pneumocystis carinii pneumonia.

Low-dose methotrexate is a commonly prescribed medication for the treatment of rheumatoid arthritis and other rheumatic diseases. Its effectiveness in ameliorating the clinical symptoms and findings has been well established, and adverse effects have been relatively infrequent, particularly compared to other immunosuppressive agents. Pneumocystis carinii pneumonia is most often seen in severely immunosuppressed patients related to the acquired immunodeficiency syndrome or treatment of malignancies with potent cytotoxic agents. However, P. carinii pneumonia can occur in patients receiving low-d

Arthritis-associated syndromes.

There are a number of diseases characterized by inflammatory arthropathy that, although not as commonly seen as rheumatoid arthritis, often present to the family physician as difficult diagnostic problems. The diagnosis is frequently most difficult during the early course of these diseases. During recent years, new and altered concepts have arisen regarding both diagnostic and therapeutic management of this challenging group of arthropathies. This article presents a review of the more common arthritis-associated syndromes with emphasis on the differential diagnosis and medicinal therapeutics.

Diagnosis and treatment of headache in the elderly.

The diagnosis and management of some causes of headache in the elderly are reviewed. Etiologic theories have been presented for each condition. Treatment modalities include pharmacologic, nonpharmacologic, and surgical therapies. The treatment course set forth in this article can help the clinician provide proper relief for the patient in pain.

Connective tissue disease in the elderly.

The prevalence of the inflammatory disorders of connective tissue in the older age group is quite likely underestimated at the present time; and, in most instances, age-related factors influencing clinical expression and autoreactivity have not been intensively studied. The array of connective tissue disease that usually targets the young adult emerges in the older age group as well; and, selectively in the elderly, GCA and PMR are found. The association of malignancy with PM/DM is increased with advanced age. In view of the therapeutic implications, early diagnosis is essential, but this can

A syndrome of arterial-occlusive retinopathy and encephalopathy.

An analysis of two new cases and four previously reported cases produced evidence for a syndrome of arterial-occlusive retinopathy and encephalopathy. All six patients were women; they ranged in age from 21 to 40 years. The clinical features of this condition include multiple branch retinal arterial occlusions and encephalopathy in which behavioral and memory disturbances predominate early. Hearing loss is frequent. Except for cerebrospinal fluid pleocytosis and an increased cerebrospinal fluid protein level, there are few laboratory or radiographic abnormalities. The disease may be responsive

Geriatric headaches.

Headaches, although common occurrences in the elderly, can be the symptoms of a serious problem. Specific headaches found in this population need careful consideration and diagnosis. When the diagnosis is elusive, or the headache is sudden, severe, and incapacitating, a neurologist or neurosurgeon should be consulted.

The clinical characteristics and significance of novel inflammatory biomarkers in patients with polymyalgia rheumatica: A single-center study from China.

OBJECTIVE: Polymyalgia rheumatica (PMR) is an inflammatory rheumatic disorder predominantly affecting older adults, characterized by proxima … nts than in male patients. 12 out of 87 (13.8 %) patients with PMR were complicated with giant cell arteritis (GCA). Anorexia was more commonly seen in patients with GCA (P = 0.024). IL-6 was the most frequently elevated cytokine and was significantly associated with fever. The systemic immune-inflammation index (SII), platelet-to-lymphocyte ratio (PLR) and …

The association between inflammation, arterial stiffness, oxidized LDL and cardiovascular disease in Finnish men with metabolic syndrome - a 15-year follow-up study.

BACKGROUND: All-cause mortality and cardiovascular disease are increased in subjects with metabolic syndrome (MetS). Risk scores are used to predict individual risk of heart disease. We performed a long-term follow-up study to investigate whether risk scores and cardiovascular risk factors such as arterial stiffness, high-sensitive C-reactive protein (hs-CRP) and oxidized LDL (OxLDL) can be used to predict cardiovascular events in Finnish men with MetS. METHODS: After baseline measurements we followed 105 Finnish men aged 30 to 65 years with MetS for a mean period of 16.4 years. The primary o

Abnormalities at three musculoskeletal sites on whole-body positron emission tomography/computed tomography can diagnose polymyalgia rheumatica with high sensitivity and specificity.

PURPOSE: To evaluate the sensitivity and specificity of PET/CT findings in PMR and generate a diagnostic algorithm utilizing a minimum number of musculoskeletal sites. METHODS: Steroid-naïve patients with newly diagnosed PMR (2012 EULAR/ACR classification criteria) were prospectively recruited to undergo whole-body 18F-FDG PET/CT. Each PMR case was age- and sex-matched to four PET/CT controls. Control scan indication, diagnosis and medical history were extracted from the clinical record. Qualitative and semi-quantitative scoring (maximum standardized uptake value [SUVmax]) of abnormal 18F-FDG

Critical left coronary main trunk stenosis, chronic occluded right coronary artery, left subclavian artery occlusion, severe aortic regurgitation and porcelain aorta in a patient with aortitis.

BACKGROUND: Coronary involvement is rare but can be critical in patients with aortitis. Although cardiac ischemia can be resolved by coronary artery bypass grafting (CABG), patients complicated with cardiac ischemia, calcified aorta, and valve insufficiency pose difficult problems for surgeons. CASE PRESENTATION: A 71-year-old woman was referred to our institution because of unstable angina. She had been previously diagnosed with aortitis and left subclavian artery occlusion. Contrast-enhanced computed tomography revealed severe left coronary main trunk stenosis, right coronary artery occlusi

Combination of HIV-1 and Diabetes Enhances Blood Brain Barrier Injury via Effects on Brain Endothelium and Pericytes.

Despite combined antiretroviral therapy (ART) achieving efficient HIV replication control, HIV-associated neurocognitive disorders (HAND) continue to be highly prevalent in HIV-infected patients. Diabetes mellitus (DM) is a well-known comorbidity of HAND in HIV-infected patients. Blood brain barrier (BBB) dysfunction has been linked recently to dementia development, specifically in DM patients. BBB injury exists both in HIV and DM, likely contributing to cognitive decline. However, its extent, exact cellular targets and mechanisms are largely unknown. In this report, we found a decrease in per
