The four ulnar metacarpal bones are connected to each other at their proximal and distal ends by strong ligaments. The second and fifth metacarpal bones are the frame of the metacarpus. Surgically, this is of great importance. A fracture of these two bones often requires stabilization, whereas a fracture of the third and fourth metacarpal bones with intact ligaments seldom requires surgery except in cases of severe fragment dislocation. Using external fixation for these purposes it seems to be of utmost interest to define secure pintracks through the soft tissue. In an anatomical study using f

BACKGROUND CONTEXT: Osteoporosis and osteopenia are common among patients undergoing posterior spine fusion surgery, presenting challenges such as pseudarthrosis, screw loosening, and poor patient outcomes. While pharmacological interventions are available, no consensus exists regarding the optimal perioperative treatment for these patients. Furthermore, the effectiveness of various treatment options in improving fusion rates and minimizing complications remains uncertain.
PURPOSE: To compare the effects of teriparatide, bisphosphonates, denosumab, and romosozumab in patients with posterior s

The temporalis myofascial flap (TMF) is a reliable option for midfacial defect reconstruction, benefiting from robust vascularisation and anatomical proximity. However, its use in palatal defect coverage has been limited by the presence of dentition. This article describes a novel TMF transposition technique through a trans-antral pathway, with immediate correction of temporal contour deformity with a titanium mesh. A case of type 2A maxillectomy in a fully dentate patient with a malignant neoplasm shows the technique's feasibility. Key steps include raising the TMF via a hemicoronal approach,

OBJECTIVE: This study proposes a novel combined intraosseous-subperiosteal hybrid implant to rehabilitate severely atrophic edentulous mandibles. We aimed to elucidate its biomechanical behavior through finite element analysis.
METHODS: Using CBCT data from an edentulous patient, we reconstructed a 3D mandibular model. We designed intraosseous fixtures tailored to the residual bone volume and fused them with superstructure abutments and a customized subperiosteal titanium mesh using 3-matic software. Four finite element models were assembled, varying in implant design and abutment count. Thes

BACKGROUND/AIM: Giant cell tumors of the bone (GCTB) involving the sacroiliac joint are uncommon and pose significant surgical challenges due to their proximity to major neurovascular structures and the complexity of stabilizing the pelvic. We describe a case of sacroiliac joint GCTB successfully managed with preoperative denosumab, piecemeal tumor resection, and spinopelvic reconstruction.
CASE REPORT: A 34-year-old woman presented with progressive gluteal pain and swelling. Imaging demonstrated an osteolytic lesion extending from the left sacrum to the ilium with extraosseous soft-tissue in

INTRODUCTION: Distal radius fractures are common among adults. Despite being a rare complication, occurring in only 0.2 % of the cases, non-union poses significant treatment challenges.
PRESENTATION OF CASE: In this article, we report the case of a 43-year-old male with a history of distal radius fracture resulting from a motorcycle accident. His initial treatment consisted of external fixation followed by a distraction plate and internal fixation with volar plate and screws. Due to plate breakage, he underwent refixation with a volar plate and autologous bone graft six months after the accid

Double-layered patella (DLP) is an osseous disorder associated with multiple epiphyseal dysplasia (MED). A thorough investigation should be conducted prior to surgery to assess the role of each layer and their vascularization in order to establish the best surgical strategy. We present the case of a woman in her late 50s with MED, bilateral DLP, and secondary osteoarthritis treated by a left total knee arthroplasty (TKA). To plan the surgical procedure, bilateral knee magnetic resonance imaging was obtained to study the anatomy of the two bony layers and their relationship to the extensor mech

Volarly displaced distal radius fractures (VD-DRFs) with small thin volar marginal fragments (VMFs) are challenging to treat with conventional volar locking plates (VLPs) alone. Several alternative surgical options have been reported to treat these fractures, including wire-loop fixation, spring-wire fixation, hook plates, headless compression screws, and tension-band wiring, but the optimal method is yet to be determined. We have devised a surgical treatment for these fractures, entitled the "loop-wiring method" in which VMFs are anatomically reduced and 2 0.043-inch K-wires are drilled throu

BACKGROUND: Basal joint arthritis is a common disease which is especially present in post-menopausal women. A variety of procedures have been described once conservative treatment has failed. The purpose of our study is to present a novel surgical technique based on a clinical case with three-year follow-up which is highly effective, even after failure of previous procedures.
CASE SUMMARY: We present a case of a 39 year-old-woman who suffered from recurring, persistent, sharp left thumb pain for several years After two operative procedures, she presented again with intolerable, sharp pain ove

BACKGROUND: The facial bone has a complex structure compared to other bones, and various types of fractures can occur due to its characteristics. Among them, in comminuted fractures of anterior maxillary wall, multiple depressed and impacted bony segments cannot be reduced easily when performing internal fixation using plates and screws or wires, and inadequate restoration leads to a range of complications. This paper introduces an alternative technique using a resorbable mesh with fibrin glue to restore comminuted fractures of anterior maxillary wall.
METHODS: Thirteen patients were diagnose

Unicameral bone cysts are rare in adults and are most often found incidentally on radiographs. However, they can persist from the adolescent period and may be present in locations that predispose to or exacerbate fractures. This article describes a case of a healthy 40-year-old woman who sustained a proximal humerus trauma that involved a large unicameral bone cyst, resulting in a 3-part head-splitting fracture. The epiphyseal location of the cyst contributed to the severity and extent of the fracture that resulted from a simple fall. Given the age of the patient, open reduction and internal fi

Vertebrectomy and instrumentation only via the posterior approach has been increasingly used in sagittal, frontal plane and combined deformities. The aim of this retrospective study is to evaluate the clinical and radiological results of hemivertebrectomy and instrumentation only via the posterior approach in congenital spinal deformities. Between 1998 and 2003, we performed hemivertebrectomy and interbody fusion using posterior instrumentation with titanium mesh cage via the posterior approach in 19 patients (three scoliosis, five kyphosis and 11 kyphoscoliosis). The age of the patients range

Hydroxyapatite/collagen (HAp/Col) composites having a bone-like nanostructure were synthesized and shaped into implants. This study was designed to develop an artificial vertebra system using this novel implant for anterior fusion of the cervical spine. Anterior fusion was carried out on 6 beagle dogs with the implants adsorbing rhBMP-2 (400 microg/ml). and 9 dogs with the implants without rhBMP-2. In 3 dogs of the rhBMP-treated group, as well as 6 dogs of the non-rhBMP-treated group, the implant was fixed with a poly-L-lactide plate and 2 titanium screws. Implants were taken out after 13 week
