Pharmacometric Model-Based Sample Size Allocation for a Region of Interest in a Multi-Regional Phase 2 Trial: A Case Study of an Anti-Psoriatic Drug.

Phase 2 trials have historically focused on characterizing the dose-exposure-response relationship in relatively homogeneous patient populations before proceeding to confirmatory trials. However, with the rise of multi-regional Phase 2 trials, it is important to strike a balance between this goal and the requirement to make sure that the optimal doses are chosen for patients from various geographic areas. This study uses a dose-ranging trial for an anti-psoriatic drug, featuring a typical design with a total sample size of N = 175, to highlight key considerations regarding sample size in multi

Imaging in clinical trials of axial spondyloarthritis: what type of imaging should be used.

Axial spondyloarthritis (axSpA) is a chronic inflammatory condition predominantly affecting the sacroiliac joints and spine. Early and accurate diagnosis is crucial to prevent structural damage and improve patient outcomes. Imaging plays a pivotal role in axSpA diagnosis, monitoring, and clinical trials, offering insights into both inflammatory activity and structural progression. Conventional radiography has been foundational for detecting structural changes, such as syndesmophytes and erosions, but it is limited by poor sensitivity for early disease detection and significant interobserver va

[Expert opinions on the technical guiding principles for clinical trials of drugs in the treatment of chronic hepatitis B virus infection].

Chronic hepatitis B virus (HBV) infection remains one of the major public health challenges facing our country. There are approximately seventy-five million chronic HBV-infected cases in our country, according to the latest epidemiological data. Current first-line antiviral drugs, including nucleos(t)ide analogs and interferons, can effectively suppress viral replication and delay disease progression, but they still struggle to provide a functional cure for chronic hepatitis B or completely eliminate HBV infection. This unmet clinical need has become the core driving force behind the developme

State-of-the-art in oncolytic virotherapy using adenoviruses other than the commonly applied adenovirus type 5.

In most clinical trials, oncolytic viruses (OAds) based on human adenovirus type 5 (HAdV-C5) were explored, and only rarely a complete switch to another adenovirus type was evaluated. This review highlights the broader diversity of human adenoviruses and discusses advances in engineering non-HAdV-C5 OAds. We will discuss ongoing research to refine adenoviral constructs derived from alternative adenovirus species, including chimeric viruses, to optimize viral delivery and enhance antitumor immune responses. We summarize translational and clinical studies using these alternative OAds, emphasizin

Methodology of drug trials in migraine: History and suggestions for the future.

There is a multitude of scientific papers and guideline papers on the methodology of drug trials in migraine. Here, we try to condense this into a single paper and to make proposals for future consideration. Literature known by the authors and from reference lists of relevant publications was used for the history. Relevant literature for our proposals was searched on PubMed. The main headings in published guidelines, namely, Patient selection, Trial design, Evaluation of results and Statistics, have remained unchanged over the years. Most of the methodology has remained unchanged but the chang

Sharing is caring: a network collaborative approach to identify and address barriers in accessing clinical trials in adolescents and young adults with leukemia and lymphoma.

Leukemia and lymphoma are 2 common hematologic cancers in adolescents and young adults (AYAs, age 15-39 years at diagnosis); however, this population has historically had lower clinical trial enrollment and less dramatic improvements in overall survival compared to other age populations. Several unique challenges to delivering care to this population have affected drug development, clinical trial availability, accessibility, and acceptance, all of which impact clinical trial enrollment. Recently, several national and institutional collaborative approaches have been utilized to improve trial av

Bispecific antibodies for multiple myeloma: past, present and future.

Despite the development of various therapeutic agents, multiple myeloma remains incurable. Recently, T-cell redirected immunotherapy has become a promising strategy for the treatment of refractory myeloma. Clinical trials using chimeric antigen receptor (CAR)-T cells and bispecific antibodies have demonstrated successful anti-myeloma responses in triple-class-refractory patients. However, unique and unwanted immune effects associated with on-target/off-target reactivity of activated immune cells need to be considered and properly managed. This review summarizes recent advances in bispecific an
