Ketone Supplementation: Meeting the Needs of the Brain in an Energy Crisis.

Diverse neurological disorders are associated with a deficit in brain energy metabolism, often characterized by acute or chronic glucose hypometabolism. Ketones serve as the brain's only significant alternative fuel and can even become the primary fuel in conditions of limited glucose availability. Thus, dietary supplementation with exogenous ketones represents a promising novel therapeutic strategy to help meet the energetic needs of the brain in an energy crisis. Preliminary evidence suggests ketosis induced by exogenous ketones may attenuate damage or improve cognitive and motor performance

[Cognitive symptoms in an elderly patient--maintaining the functional capacity is always important].

In primary health care, elderly patients having problems in their ability to deal with information are increasingly encountered as a result of lessened institutional care and altered population structure. The most common causes of cognitive problems in the elderly include delusional disorder, various progressive memory diseases and delirium. Treatment of cognitive problems is multidisciplinary collaboration, and maintaining the functional capacity is important at all stages of the disease. Memory disorder medications clearly increase the time of functional capacity, provided that their use is

[The characteristics of non-verbal learning disorder].

INTRODUCTION: Non verbal learning disorder (NLD) appears to be the consequence of an alteration in the cognitive functions linked to the right hemisphere. It is characterised by the coexistence of difficulties in carrying out social skills, poor academic achievement, low visuospatial skills and motor clumsiness. DEVELOPMENT: In this paper, information from the medical literature and the authors' own experience are taken as the basis to describe the most relevant clinical manifestations and the cognitive behavioural profile presented by children with NLD. Bearing in mind the similarity between

Integrating Hypnosis with Other Therapies for Treating Specific Phobias: A Case Series.

There is a high prevalence of anxiety disorders including specific phobias and panic disorder in the United States and Europe. A variety of therapeutic modalities including pharmacotherapy, cognitive behavioral therapy, systematic desensitization, hypnosis, in vivo exposure, and virtual reality exposure therapy have been applied. No one modality has been entirely successful. There has been only a limited attempt to combine psychological therapies in the treatment of specific phobias and panic disorder and what has been done has been primarily with systematic desensitization or cognitive behavi

Postoperative cognitive dysfunction: toward the Alzheimer's disease pathomechanism hypothesis.

Alzheimer's disease (AD), a chronic and progressive deterioration of memory and other cognitive domains, is the most common form of dementia. Because of related health and social impact, there is growing interest in assessing potential relationship between anesthesia and the onset and progression of chronic neurodegenerative disorders, including AD. Currently, preclinical and clinical research is addressed to identify underlying pathomechanisms, patient risk factors, and the use of the least provocative drugs and techniques, to minimize the incidence of chronic neurodegenerative disorders. Pre

[Gait disturbances and normal pressure hydrocephalus].

Normal pressure hydrocephalus must be considered when gait disturbance, cognitive impairment and sphincter disorders are associated. Symptoms of normal pressure hydrocephalus, especially gait disturbance, are potentially curable by surgery. Our paper provides a summary review of gait disorders associated with normal pressure hydrocephalus detailing their characteristics and the best assessment methods. Although the pathogenic mechanisms underlying normal pressure hydrocephalus remain poorly understood, advances in imaging have enabled considerable progress in our fundamental knowledge of the c

The neuroprotective effects of estrogen on the aging brain.

The population of the western world is ageing. This increase in the elderly population will inevitably mean a rise in the prevalence of age-related cognitive decline and late-onset neuropsychiatric disorder, such as Alzheimer's disease (AD). There are sex differences in the incidence and age of onset of these disorders. Sex steroids and sex chromosomes are therefore implicated in their pathophysiology. We have identified relevant past and current literature using a Medline search and from the references of relevant papers. These were then reviewed and relevant articles have been summarized and

Getting comfortable with physical discomfort: A scoping review of interoceptive exposure in physical and mental health conditions.

Interoceptive exposure (IE) involves the use of exercises, activities, or tasks to intentionally induce (or exacerbate) physical symptoms in the body, to challenge misconceptions about the harmful nature of the physical symptoms that maintain fear and problematic avoidance. IE was originally developed for the cognitive behavioral treatment and prevention of panic disorder. Bodily sensations and concern about physical symptoms are common features in many conditions, not limited to panic disorder. For this reason, IE could be theoretically relevant to cognitive behavioral intervention for many p

First episode of psychiatric and neuropsychiatric disease among patients infected with COVID-19: A scoping review.

This scoping review aims to examine the frequency and prevalence of neuropsychiatric disorders reported in patients infected with coronavirus disease 2019, and the mechanisms by which these develop during and post infection. A systematic search using relevant search terms and key words was done on six electronic databases of literature on neuropsychiatric conditions post-coronavirus disease 2019 infection from 2020 to 2023. Data were extracted following Joanna Briggs Institute guidelines, focusing on key findings, intervention details, and outcomes. We included 333 studies in the review. Studi

Female Sexual Dysfunction: Common Questions and Answers.

Symptoms of female sexual dysfunction are common, affecting 40% to 50% of women. Most patients would like their physician to actively inquire about sexual dysfunction. When symptoms of female sexual dysfunction are chronic and cause the patient distress, they can be diagnosed as female sexual disorders. This clinical diagnosis is based on a thorough history and physical examination; understanding the patient's symptoms and associated distress is essential. The condition is further categorized into disorders of sexual desire and arousal, orgasmic disorders, and genito-pelvic pain disorders. The

Avoidant personality disorder: current insights.

Avoidant personality disorder (AVPD) is a relatively common disorder that is associated with significant distress, impairment, and disability. It is a chronic disorder with an early age at onset and a lifelong impact. Yet it is underrecognized and poorly studied. Little is known regarding the most effective treatment. The impetus for research into this condition has waxed and waned, possibly due to concerns regarding its distinctiveness from other disorders, especially social anxiety disorder (SAD), schizoid personality disorder, and dependent personality disorder. The prevailing paradigm subs

Commonalities and Divergences in the Cognitive Profiles of Autism and Dementia: Protocol for a Narrative Literature Review.

BACKGROUND: Literature has shown an increase in research relating to autism and aging, and more specifically, autism and healthy aging. Within the literature, there is a clear lack of knowledge and understanding of the impacts of dementia and autism as co-occurring experiences. More specifically, there is a lack of clinical knowledge about the ways in which the cognitive profiles of autism and dementia do and do not overlap, and how this overlap might affect dementia assessments for this population. This is likely to result in challenges with diagnosis and may lead to misdiagnosis or a lack of

Association of Sarcopenia With Global, Motor, Cognitive, and Swallowing Function Prognosis in Stroke Patients: A Systematic Review and Meta-Analysis.

OBJECTIVE: Sarcopenia has been demonstrated to correlate with the prognosis of stroke. This study aimed to systematically evaluate the impact of sarcopenia on global, motor, cognitive, and swallowing function outcomes in stroke patients. METHODS: Relevant studies investigating the association between sarcopenia and multidimensional prognostic outcomes in stroke were retrieved from the PubMed, Embase, Web of Science, Cochrane Library, and Scopus databases. The primary outcome was the risk of poor global functional prognosis assessed by the modified Rankin Scale (mRS). Secondary outcomes includ

Cognitive Dysfunction in Migraine: Underlying Mechanisms and Clinical Implications.

PURPOSE OF REVIEW: Cognitive dysfunction (CD) is a common and disabling, yet under-recognized, symptom of migraine. Difficulty with receptive and expressive language, attention, and memory are reported ictally, while memory-related deficits are common interictally. Furthermore, migraine-related comorbidities can contribute independently to CD. RECENT FINDINGS: Both subjective patient-reported and objective neuropsychological testing cognitive changes occur in migraine. The pathophysiology of CD in migraine involves altered subcortical activity affecting the hypothalamus, hippocampus, and thala

Insomnia phenotypes and neurocognitive biomarkers.

Insomnia is increasingly understood as a heterogeneous disturbance of sleep-wake regulation shaped by interacting cognitive, physiological, circadian, neurobiological, and perceptual mechanisms. This chapter develops a phenotype-informed framework for insomnia that distinguishes descriptive clinical presentations from underlying mechanistic dimensions and measurable biomarker domains. Core phenotypes reviewed include cognitive-emotional hyperarousal, objective short sleep duration, paradoxical insomnia and sleep-state misperception, circadian misalignment, autonomic dysregulation, sleep-depth

State of the Science: Hoarding Disorder and Its Treatment.

This state-of-the-science review describes the relatively new diagnosis of hoarding disorder (HD), characterized by difficulty discarding possessions and resulting clutter in living spaces. We review current theoretical models of HD, including a cognitive-behavioral model, a biopsychosocial model, an attachment model, and an addictions model. We then describe interventions for HD, focusing largely on cognitive-behavioral therapy for hoarding disorder (CBT-HD), the only treatment with conclusive evidence of efficacy. We review the components of CBT-HD and their rationale, noting that clinical r

Plasma biomarkers in neuropsychiatric syndromes: A narrative review.

Neuropsychiatric symptoms (NPS) are common features of neurodegenerative disease (NDD) but are relatively understudied compared to cognition, especially regarding biomarkers. Further, emerging evidence describes the utility of systematic assessment of NPS across the cognitive continuum, even in advance of dementia. In this narrative review, we discuss the role of plasma biomarkers in relation to NPS across the cognitive continuum of unimpaired, subjective cognitive decline, mild cognitive impairment, and dementia. While Alzheimer's disease is the primary focus, vascular, Lewy body, and frontot

A hypothesis to explain malaria-induced neurocognitive sequelae.

The development of malaria-induced neurocognitive and behavioral sequelae is not entirely understood. We hypothesize that liver dysfunction caused by Plasmodium infection is responsible for malaria-induced neurocognitive and behavioral sequelae. Our metabolic hypothesis not only explains neurocognitive sequelae after cerebral malaria (CM) but also after other severe, non-severe, and asymptomatic malaria infections.

[Anxiety].

In this article, we will address 10 questions about anxiety that are relevant to doctors who encounter this in their practice. This often occurs in the primary care setting, where individuals with anxiety frequently present with somatic complaints. A focused medical history, including questions about the use and withdrawal of psychoactive substances, can assist in the diagnostic process. Psychoeducation may be sufficient, otherwise cognitive-behavioral therapy can be conducted. In cases of non-response, serotonergic antidepressants represent a treatment option. Half of the individuals with anx
