The evidence behind the World Health Organization guidelines that BCG vaccination should not be used in HIV-infected children is compelling. We report three cases of severe BCG-associated immune reconstitution syndrome in HIV-infected infants and describe measures available in developing countries that prevent inadvertent BCG immunization in these infants.

BACKGROUND: Fabry disease (FD), the second most prevalent lysosomal storage disorder, is classified as a rare disease. It often leads to significant quality of life impairments and premature death. Many cases remain undiagnosed due to the rarity and heterogeneity. Further, costs related to treatment often constitute a substantial financial burden for patients and health systems. While its epidemiology is still unclear, newborn screenings suggest that its actual prevalence rate is significantly higher than previously suspected.
METHODS: Based on well-established methodologies, this study gives

Micronutrient deficiencies are common consequences of the plant-based diet in children from developing countries which may affect their linear and ponderal growth. The aim of the study was to investigate the association between micronutrient status and growth indices in Indian girls. In cross-sectional studies (2006-2010), data on weight, height and diet were collected on 1302 girls (2-18 years) from Pune city, India. Fasting hemoglobin was measured on 1118 girls and serum zinc was measured on 695 girls. Height-for-age Z-scores (HAZ) and body mass index for age Z-score (BMIZ) were computed usi…

Introduction In developing countries, sepsis and associated mortality rates in neonatal patients is a serious concern. To improve the outcomes and mortality posed by sepsis, physicians need to know the local epidemiology of the microbial pathogens and their resistance patterns to antimicrobial agents. Therefore, our aim was to determine the frequency of early-onset neonatal sepsis (EONS) following prolonged rupture of membranes (PROM). Materials and methods After approval from the ethical review committee, this cross-sectional study was conducted at a tertiary care hospital of a developing cou…

While vitamin A excess is more common in developed countries, deficiency is most prevalent in developing countries. With proper maintenance, regulation, and education about VAD and excess, a pregnant mother can diminish potential harm to her fetus and potential teratogenic risks.

Rotavirus is the leading cause of acute diarrhea in children younger than five years of age around the world. Severe dehydration and mortality rates are higher in developing countries, especially those from Latin America, Africa, and Asia. The vaccine has been introduced in the national immunization programs of more than half of Latin American countries, and impact data from some of these nations has been already published. The two rotavirus vaccines, the 2-dose monovalent (RV-1) and the 3-dose pentavalent (RV-5) vaccine, have been available in the market to all Central American countries. Rot…

Pneumocystis jirovecii pneumonia (PcP) is a well-recognized major opportunistic infection in HIV-infected patients. During the 1980s, the HIV pandemic turned PcP into a major worldwide medical and public health problem. With the introduction of Pneumocystis chemoprophylaxis and the development of highly active antiretroviral therapy (ART) for the treatment of HIV infection, there has been a decrease in PcP incidence in developed countries. However, the prevalence of AIDS-related PcP in developing countries remains high because a lot of people do not have access to ART or ignore their HIV infec…

Rotavirus gastroenteritis is one of the most important diseases in young children. There are 400,000 to 600,000 deaths annually among children under 5 years old in the world, especially in developing countries. Rotavirus vaccine has been developed since 1980s. However vaccine development and trial took a long time before the commercial available. One reason is the genetic variability of rotavirus, there are more than 10 serotypes (G types) in group A rotavirus. Another reason is side reaction such as intussusception which was recognized at the time of development of RotaShield. At present, Rot…

Uganda is one of only two countries in the world that has successfully reversed the course of its HIV epidemic. There remains much controversy about how Uganda's HIV prevalence declined in the 1990s. This article describes the prevention programs and activities that were implemented in Uganda during critical years in its HIV epidemic, 1987 to 1994. Multiple resources were aggregated to fuel HV prevention campaigns at multiple levels to a far greater degree than in neighboring countries. We conclude that the reversed direction of the HIV epidemic in Uganda was the direct result of these interve

Children responsible for their own survival exist in all countries. Despite social and cultural differences between street youth in developing countries versus homeless youth in developed countries, the predictors and correlates of homelessness are similar among youth. The AIDS pandemic is inextricably linked to homelessness and is a particularly devastating threat to the welfare of the world's disenfranchised youth, as they are continually forced into multiple HIV-related high risk situations and behaviors. Specific recommendations regarding clinical care, prevention programs, research, and t…

Myotonic dystrophy is an autosomal dominant multisystem disorder rarely reported in sub-Saharan Africa. We report a 30-year-old Ethiopian female with progressive weakness, myotonia, positive family history, and characteristic electromyography findings. As there is no curative treatment, management is supportive. This case highlights its rarity and rehabilitation challenges in developing countries.

Organophosphate poisoning is the most common poisoning in developing countries. Although the acute muscarinic and nicotinic side-effects of organophosphate poisoning are well known and easily recognized, but neuropsychiatric changes are rarely reported. We are reporting a case of a 33-year-old female who developed manic episode following acute organophosphate poisoning.

If patients are planning to travel to developing countries, their primary care physicians can counsel them on various medical risks, especially traveler's diarrhea, and offer to update their immunizations. However, travelers to areas where there is a risk of malaria, yellow fever, or other tropical diseases should be referred to a specialist.

Survival of preterm neonates in developing world has improved. Developing countries lag behind in nutritional management in NICU especially parenteral nutrition (PN). This randomized controlled trial was done to evaluate the effect of aggressive parenteral nutrition on nitrogen retention of sick VLBW and extremely low birth weight (ELBW) babies. From September 2009 to February 2010, total 34 babies were randomized to receive aggressive parenteral nutrition (APN)(n=17) or standard parenteral nutrition (SPN) (n=17). The average daily total and PN calory intake of babies in APN group was signific…

The year 2023 marks the 100th anniversary of the first successful valvotomy for mitral valve stenosis by Elliott C. Cutler in 1923. Closed-chest mitral valve commissurotomy developed further before being replaced by an open procedure after the advent of the heart-lung machine. Currently, because of the almost complete disappearance of rheumatic disease in the Western World, mitral commissurotomies are infrequently performed in those countries, although the procedure-either closed or open-is still performed in developing countries and select patients. This review retraces the 100-year journey f…

Vesicovaginal fistulas (VVF) represent a detrimental condition causing psychological, physical and social strain on patients. In developed countries they are predominantly the result of pelvic surgery or radiation therapy, whilst obstetric VVF are common in developing countries due to prolonged and complicated births. The majority of VVF require surgical therapy, thus a comprehensive diagnostic workup is needed. Depending on diagnostic characteristics fistula repair can be performed by a transvaginal, transabdominal or minimally invasive approach. Timing of surgery, appropriate interposition o…

Hepatitis E virus (HEV) is a common infection worldwide and is an emerging infectious disease in the developed countries. The unique characteristics of HEV is that it displays different epidemiological and clinical characteristics between developing and developed countries. Neurological disorders are emerging extra hepatic manifestations of both acute and chronic Hepatitis E virus infection. We report a 17 year old sportsman presenting with acute encephalitic Parkinsonism concurrent with acute hepatitis. Serology was positive for Hepatitis E virus (HEV) and HEV RNA was confirmed.
