The gastric emptying of a liquid test meal containing 99Tcm-sulphur colloid was studied with a gamma camera in 24 duodenal ulcer patients. Studies were made before surgery, randomly chosen between truncal vagotomy with pyloroplasty and parietal cell vagotomy, and six weeks and one year after. The results of a randomized, prospective clinical trial of highly selective vagotomy (HSV) versus truncal vagotomy and pyloroplasty (TVP) in 126 male patients undergoing elective surgery for chronic duodenal ulceration are presented. The operations were performed by surgeons of all grades of experience. Graded antrum distension by 50-, 100-, and 150-ml balloon volumes significantly stimulated gastric acid secretion in 6 duodenal ulcer patients, but had no stimulatory effect in 6 healthy subjects. In a necropsy series from Malmö of 2,218 subjects above 20 years of age, and composing 88.2% of all Malmö residents above 20 who died during the period of investigation, the frequency of gastric and duodenal peptic ulcer disease (ulcer, scar or surgically treated peptic ulcer) was studied. The total frequency of peptic ulcer disease was 17.9% (20.9% in men and 14.7% in women). The condition was most common (25.4%) in men in the 50-59 year age group, after which the frequency declined. A consecutive series of 100 men with uncomplicated duodenal ulcer was randomly divided into two groups: one group of 52 underwent proximal gastric vagotomy (PGV), the other group (48) underwent PGV with pyloroplasty (PGVP). Preoperative peak acid output (PAOP) was measured in all patients. Those with a higher preoperative PAOP were significantly more likely to develop recurrent ulceration. The case of a 31 year old male mongoloid patient is reported, where a congenital duodenal membrane, asymptomatic in childhood, had led to hiatal hernia, stricture of the oesophagus, and finally to carcinoma of the stricture (Fig. 1). Observation time until death was 26 months. In a first operation, the membrane was resected (Fig. 2) and Nissen's fundoplication performed. 31/2 months after that stricture, carcinoma was diagnosed. In the second operation, carcinoma was resected and continuity reestablished by oesophagogastrostomy. The patient died 8 months after the second intervention.

Relations between gastric acid secretion measurements and recurrence of duodenal ulcer within 1 to 4 years after selective vagotomy and pyloroplasty were evaluated in a series of 117 men. The discriminatory ability of spontaneous, histamine- and insulin-activated acid secretion measurements was significant and similar to that after truncal vagotomy and drainage. The clinical results of truncal vagotomy and drainage for duodenal ulcer in 500 patients are evaluated by a personal 6-8 year follow up. Pyloroplasty was used unless pyloric stenosis made a gastrojejunostomy necessary. Forty-three patients had recurrence (39 were reoperated) and gastric ulcer was seen in five.
