The purpose of this study was to obtain an overview of acute injuries and chronic overuse syndromes due to rock climbing on artificial climbing walls. Strategies for prevention should then be developed based on these experiences. From October 1995 to December 1996 314 climbers of both sexes and all degrees of climbing abilities were individually interviewed at five different indoor climbing arenas with the help of a special questionnaire. Type, quantity and cause of typical injuries and overuse syndromes on artificial climbing walls were determined. Injuries (n = 204) as well as overuse syndro

A system for evaluation of physical impairment in the hand and upper extremity was developed and has been tested and used by many hand surgeons around the world. It was approved for international application by the International Federation of Societies for Surgery of the Hand. A method for evaluating amputation and sensory and motion impairments has been devised for the hand and upper extremity. Amputation impairment percentage values were determined for each digit or position thereof for the hand, wrist, elbow, and shoulder. By a principle of progressive multiplication of percentage values, i

BACKGROUND: The optimal fixation strategy for complex humeral shaft fractures, especially those with severe comminution, remains controversial. This study compared clinical and radiological outcomes after open reduction and internal fixation (ORIF), minimally invasive plate osteosynthesis (MIPO), or intramedullary (IM) nailing for AO/OTA type 12-C humeral shaft fractures.
PATIENTS AND METHODS: This multicenter retrospective comparative study included 75 adults with AO/OTA type 12-C humeral shaft fractures treated surgically between 2015 and 2023. Patients were grouped by the fixation method,

AIMS: This study aimed to determine the minimal detectable change (MDC), minimal clinically important difference (MCID), and substantial clinical benefit (SCB) under distribution- and anchor-based methods for the Mayo Elbow Performance Index (MEPI) and range of movement (ROM) after open elbow arthrolysis (OEA). We also assessed the proportion of patients who achieved MCID and SCB; and identified the factors associated with achieving MCID.
METHODS: A cohort of 265 patients treated by OEA were included. The MEPI and ROM were evaluated at baseline and at two-year follow-up. Distribution-based MD

Reverse total shoulder arthroplasty (RTSA) in very young adults with rheumatoid arthritis (RA) is uncommon because of concerns regarding implant longevity and lifetime revision burden, and outcomes data in patients younger than 30 years remain limited. We report the case of a 25-year-old woman with seropositive RA who developed rapid, destructive monoarticular involvement of the right shoulder despite treatment with disease-modifying antirheumatic drugs (DMARDs) and biologic therapy. Imaging demonstrated complete glenohumeral joint space obliteration, humeral head collapse, erosive changes, an

Proprioception plays a critical role in motor control and is often impaired following orthopedic or neurological damage. Non-invasive brain stimulation methods, such as transcranial direct current stimulation (tDCS), could potentially be used to modulate proprioceptive processing and help recovery. Among cortical areas, the premotor cortex (PMC) and posterior parietal cortex (PPC) are key regions involved in proprioceptive processing. We investigated the effects of tDCS delivered with the anode positioned over the right PMC and PPC on active joint position sense (AJPS) in healthy adults. Forty

The lack of reliable sensory input from prosthetic limbs limits transfemoral amputees' ability to perceive limb movement without visual monitoring. This study evaluated design parameters of a proposed forearm-based vibrotactile system in a pre-clinical, design-level perceptual evaluation, conveying prosthetic joint positions through patterned vibrations to provide non-invasive proprioceptive feedback. Healthy participants completed two experiments assessing detection of tactile cues from dual-actuator bands on the wrist and elbow representing assumed ankle and knee positions. The effects of te

Chronic Monteggia fracture-dislocations in children, defined by presentation beyond four weeks, pose significant surgical challenges. While ulnar osteotomy is central to reconstruction, the optimal technique for ensuring stable radiocapitellar reduction remains a topic of debate. We present the cases of two nine-year-old patients with chronic Bado type I Monteggia lesions. The first patient underwent transverse ulnar osteotomy with annular ligament repair and temporary radiocapitellar fixation but developed recurrent radial head dislocation after K-wire removal. The second patient was treated

BACKGROUND: Osteochondritis dissecans (OCD) is an articular disorder characterized by focal subchondral bone involvement that may progress to cartilage instability and fragment detachment. Although OCD most commonly affects the knee, ankle, and elbow, shoulder involvement is rare, and glenoid lesions are particularly uncommon. The limited literature consists primarily of isolated case reports, most frequently describing male overhead athletes. Optimal management strategies for glenoid OCD remain poorly defined, particularly in adolescent athletes with big chondral lesions.

INTRODUCTION: Osteosarcoma, the most prevalent malignant primary bone tumor in children and adolescents, can present formidable challenges when involving long segments of the humerus. Reconstruction options must balance oncological clearance with long-term limb function, particularly in skeletally immature patients.
CASE REPORT: We report a case of a 15-year-old female diagnosed with high-grade osteosarcoma involving the proximal epiphysis and most of the diaphysis of the right humerus, sparing only the distal 3 cm. Following neoadjuvant chemotherapy, the patient underwent wide tumor excision

Cubital tunnel syndrome, a common neuropathy of the upper limb, presents with sensory and motor symptoms, impacting hand function and grip strength. Surgical interventions, such as medial epicondylectomy (ME), aim to alleviate symptoms by relieving pressure on the ulnar nerve at the elbow. Historical attempts to treat this condition date back to the early 19th century, with procedures evolving over time. Modern techniques, including in situ decompression, transposition, and various forms of epicondylectomy, offer different approaches to address nerve compression. This article examined the evol

Myopathic Ehlers-Danlos syndrome (RmEDS) is an emerging hybrid phenotype that combines connective and muscle tissue abnormalities. It has been associated with variants of the COL12A1 gene, which are known as Ullrich congenital muscular dystrophy-2 (UCMD2; 616470) and Bethlem myopathy-2 (BTHLM2; 616471). Here, we report two splicing mutations of COL12A1 identified in three patients from two unrelated families who present a combination of joint hypermobility and axial, distal, and proximal weakness. The muscular strength of their neck and limb muscles was assessed at 4/5 (MRC); however, when mea

A 26-year-old man was admitted to our hospital due to the replantation of the severed right upper arm and right thigh injury 6 days before. The patient received emergency treatment at a local hospital. He underwent amputation and replantation for the right upper arm and right thigh. After surgery, he experienced fever, limb swelling, and wound pain. At the time of admission to our hospital, the patient exhibited stable vital signs. An infection was found at the replanted wound, and the culture results showed Pseudomonas aeruginosa. After admission, the patient underwent symptomatic antiinflamm

(1) Background: Extravasation injuries in pediatric patients can lead to significant harm if they are not promptly diagnosed and treated. However, evidence-based standardization on extravasation management remains limited, particularly for extensive wound necrosis. This case report presents the management of an 8-week-old premature patient with an extensive extravasation injury involving the right forearm and dorsum of the hand. (2) Methods: The patient was evaluated by a multidisciplinary team in our Neonatal Intensive Care Unit. Surgical intervention involved the debridement of necrotic tiss

Individuals with neurological disorders, such as stroke or spinal cord injury, often have weakness and/or spasticity in their hand and wrist muscles, which can lead to impaired ability to extend their fingers and wrists. Functional electrical stimulation can help to restore these motor functions. However, the conventional stimulation method can lead to fast muscle fatigue and limited movements due to a non-physiological recruitment of motor units and a limited recruitment of deep muscles. In this paper, we investigated the feasibility of eliciting various hand opening and wrist extension movem
