Nine patients with tremor on writing and one patient with tremor only on swinging a golf club were investigated. None of the patients had any other neurological symptoms or signs. The frequency of the tremor ranged from 5 to 6 Hz. Rapid passive supination or pronation of the forearm by a torque motor evoked a short burst of alternating tremor in seven patients. The tremor was improved by alcohol or propranolol in six patients. These characteristics of writing tremor (and of other isolated action tremors) suggest that it is a variant of benign essential tremor.

Complete ascertainment of adult onset limb-girdle muscular dystrophy in the Lothian Region of Scotland was attempted. Ten index cases were identified giving a prevalence of 1.3 per 100 000 (0.9 per 100 000 for cases where the diagnosis of muscular dystrophy was supported by both electromyographic and muscle biopsy findings). In these 10 sibships there had been 11 affected subjects, significantly less than the 16.5 cases expected for autosomal recessive inheritance. Rhythmic myoclonus in an arm began abruptly following an injury and persisted continuously for six years. Topographical EMG showed abnormal activity confined to muscles innervated by the axillary and radial nerves from the posterior cord of the brachial plexus. Abduction of the arm above horizontal level stopped myoclonus and EMG discharges. EEG was normal. It is suggested that the myoclonus was caused by mechanical irritation of the posterior cord of the brachial plexus.

Eight patients with essential blepharospasm were given five consecutive sessions of electromyogram biofeedback from cutaneous electrodes placed over the frontalis muscle. Four of the eight patients demonstrated a 60% or greater reduction in blepharospasm frequency. Posttreatment electromyogram significantly predicted improvement of blepharospasm. These data suggest that electromyogram biofeedback may provide a therapeutic alternative to the treatment of this disabling condition.

Dogs exposed to topical organophosphate (fenthion) developed decreased plasma and muscle cholinesterase activities. After 2 doses were applied (1 week between doses), plasma concentrations declined 80% and muscle cholinesterase activity was reduced by 56%. Twenty-five cases from the literature and four personal cases with dystonia musculorum deformans are described. The polysomnographic EEG findings of severely involved patients were characterized by over abundance of stage 2 sleep, with a characteristic pattern of spindle activity, increased latency to sleep, and reduced sleep efficiency. The similarities between these patients may underscore the importance of sleep spindles as a feature of the dystonia process and may be useful in the investigation of other basal ganglia disorders.

In 3 patients with the syndrome of Unverricht-Lundborg's myoclonus-epilepsy electropolygraphic examinations were carried out during night sleep.
