Hospital Factors, Performance on Process Measures After Transient Ischemic Attack, and 90-Day Ischemic Stroke Incidence.

BACKGROUND AND PURPOSE: We determined the association between hospital factors, performance on transient ischemic attack (TIA) process measures, and 90-day ischemic stroke incidence. METHODS: Longitudinal analysis of retrospectively obtained data on 9168 veterans ≥18 years with TIA presenting to the emergency department or inpatient unit at 69 Veterans Affairs hospitals with ≥10 eligible patients per year in fiscal years 2015 to 2018. Process measures were high/moderate potency statin within 7 days of discharge, antithrombotic by day 2, and hypertension control (<140/90 mm Hg) at 90 days. The

The Application of GSCM in Eliminating Healthcare Waste: Hospital EDC as an Example.

Eliminating unnecessary healthcare waste in hospitals and providing better healthcare quality are the core issues of green supply chain management (GSCM). Hence, this study used a hospital's emergency department crowding (EDC) problem to illustrate how to establish an emergency medicine service (EMS) simulation system to obtain a robust parameters setting for solving hospitals' EDC and waste problems, thereby increasing healthcare quality. Inappropriate resource allocation results in more serious EDC; more serious EDC results in increasing operating costs. Therefore, in the healthcare system,

Disparities in postoperative surveillance and use of emergency health services following endovascular abdominal aortic aneurysm repair among Medicare beneficiaries.

OBJECTIVE: Routine imaging surveillance following endovascular aortic aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA) is critical for the timely diagnosis of late postoperative complications. Compliance with recommended EVAR surveillance is variable, and disparities in post-EVAR surveillance remain unclear. This study examines variability in EVAR surveillance and emergency health service use across several sociodemographic populations. METHODS: All Medicare fee-for-service beneficiaries who underwent infrarenal EVAR for intact abdominal aortic aneurysm between January 2011 and Dece

Hospital and physician-based mental healthcare during 12 months of opioid agonist treatment for opioid use disorder: Exploring costs and factors associated with acute care.

BACKGROUND: Individuals with opioid use disorder (OUD) have a high prevalence of co-occurring mental health disorders; however, there exists little information on mental health service use for this population. We aimed to determine the prevalence of non-substance use-related mental health emergency department (ED) visits, hospitalizations, and outpatient physician visits for individuals receiving treatment for OUD over one year. We also explored individual-level characteristics associated with mental health care service use and estimated the costs of this care. METHODS: We linked observationa

Emergency Medical Service Identification and Prehospital Care of Acute Coronary Syndrome Among Culturally and Linguistically Diverse Immigrants: Statewide Linkage Study.

BACKGROUND: Timely recognition and response to acute coronary syndrome (ACS) by emergency medical services is critical to reducing delays and improving outcomes. This study examines whether emergency medical services identification, care, and times (the interval from emergency medical services call to hospital arrival) differ by culturally and linguistically diverse (CALD) background among patients with ACS. METHODS: We conducted a retrospective cohort study using ambulance data linked with the Department of Health's hospital data sets (January 2015-June 2019). The CALD group comprised indivi

Emergency dermatology: Three-month experience from an Italian academic outpatient clinic during lockdown for COVID-19 pandemic.

Dermatology is a field of medicine where urgent cases occur commonly. However, access to specialized emergency dermatology services is very limited. Following the declaration of the COVID-19 pandemic, the cessation of all elective dermatology visits was widely urged. Accordingly, in Italy, a country severely affected by the pandemic, various measures were applied and the care at university clinics was limited to urgent cases. Here we retrospectively analyzed data of patients who presented at an Italian academic outpatient clinic reserved only for emergency cases. In total, 252 patients (109 ma

Isolated Penile Edema After Diagnostic Paracentesis.

Diagnostic paracentesis is a routinely practiced, typically safe procedure performed in the emergency department. Genital swelling post-paracentesis is a rare complication with few documented case reports. We report a case of isolated penile edema after a diagnostic paracentesis performed in the emergency department. The patient is a 63-year-old male who came to the emergency department with a two-day history of isolated penile swelling after undergoing a diagnostic paracentesis in the emergency department as part of his workup during a recent hospital admission. On exam, the paracentesis site

Care requirements of people with ARC/AIDS in Rome: non-hospital services.

In Italy, care for people with AIDS (PWA) is centred on hospital services. However, other services are beginning to develop, both in the form of residential facilities and of home care. It seems, therefore, important to define the type of non-hospital services that should be supplied on the basis of AIDS patients' real needs. The aim of this paper is to contribute to the definition of care requirements by administration to 92 persons with ARC/AIDS of a questionnaire on Functional Multidimensional Evaluation to assess the demand for services in relation to the state of health and socio-economic

Disposition at Equipoise: A Qualitative Study of Emergency Physicians' Decision-Making About Hospitalizing People With Dementia.

BACKGROUND: People with dementia are often hospitalized from the emergency department (ED) for conditions that could potentially be treated in the outpatient setting. In scenarios of clinical equipoise-where either admission or discharge may be appropriate-little is known about how emergency physicians make disposition decisions. METHODS: We conducted a qualitative study with attending emergency physicians at a single academic medical center. Interviews included open-ended questions about disposition decision-making in general as well as structured case scenarios using a think-aloud protocol.

Impact of strategies to improve flow and lower hospital admissions for low back pain in the emergency department: an interrupted time-series analysis.

BACKGROUND: Low back pain (LBP) is a common, disabling condition that often results in presentation to the emergency department (ED). There is currently a paucity of research examining how to optimise care for this patient group and concurrently improve service efficiency. We assessed the impact of a suite of co-designed strategies to improve the flow and reduce admission rate of people with LBP presenting to the ED. METHODS: A quasi-experimental implementation science study in a 30-bed ED at an Australian tertiary referral hospital. We implemented four improvement strategies: (1) co-design a

Safety of antibiotic audit and feedback: secondary analysis of a randomized controlled trial.

BACKGROUND: We previously demonstrated in a randomized controlled trial that antibiotic prescribing feedback to family physicians reduced antibiotic use. However, that trial did not evaluate for patient harms from potential under-prescribing of antibiotics. OBJECTIVES: To evaluate the safety of an antibiotic audit and feedback intervention. METHODS: We performed a post hoc secondary analysis of a randomized controlled trial that compared an intervention group, who received a mailed antibiotic prescribing peer comparison feedback letter, compared with a control group who did not receive a let

The Epidemiological and Clinical Profiling of Heart Failure-A Retrospective and Comparative Analysis of Cases Before, During, and After the COVID-19 Pandemic in a Romanian Emergency County Clinical Hospital.

Background and Objectives: Heart failure (HF) represents a clinical syndrome characterized by symptoms and signs such as fatigue, dyspnea, edema of the lower limb, or pulmonary rales. It usually occurs in elderly individuals due to decreased cardiac pumping function and/or increased diastolic ventricular filling pressures. The COVID-19 pandemic deeply altered many daily life habits, and one of the most affected groups of people were those with chronic diseases because of their need for regular medical follow-up. Furthermore, SARS-CoV-2 infection itself has been shown to exacerbate cardiovascul

Pulse oximetry beyond oxygen saturation: Early waveform characteristics in sepsis patients with adverse outcomes - A proof-of-concept study.

BACKGROUND: Sepsis is a life-threatening disorder with an in-hospital mortality rate of 10-40 %. Identifying patients at risk for deterioration is critical in guiding clinical decisions for patients with infection at the emergency department (ED). While pulse oximetry is known for estimating oxygen saturation, we hypothesize that photoplethysmography (PPG) can reflect early cardiovascular changes in sepsis. This study explores the potential use of pulse oximetry - PPG as a tool for predicting clinical deterioration in patients with early sepsis. METHODS: We conducted a secondary analysis of p
