Predicting 10-Year Diabetes Risk Through Physiological Acceleration: A Longitudinal Deep Learning Ensemble Approach.

Background/Objectives: Type 2 diabetes (T2D) develops gradually over many years through a prolonged preclinical phase, yet traditional static risk scores often fail to capture these dynamic metabolic trajectories. We propose a longitudinal deep learning framework to predict the 10-year risk of Type 2 diabetes onset defined by comprehensive ADA criteria by modeling the physiological acceleration of routine clinical biomarkers. Methods: Utilizing an 18-year longitudinal dataset from the community-based Korean Genome and Epidemiology Study (KoGES) cohort, we selected N=4354 participants with comp

Prenatal adipose tissue estimation using early second trimester anthropometry.

BACKGROUND/OBJECTIVES: Prenatal visceral (VAT) and total (TAT) adipose tissue are associated with gestational diabetes and preeclampsia risk, yet cannot be routinely assessed in clinical or epidemiologic settings. Although anthropometric measures reflect adiposity in non-pregnant individuals, their utility in early pregnancy remains unclear. We examined associations between anthropometrics and TAT and VAT in the early second trimester and evaluated differences by prepregnancy BMI. SUBJECTS/METHODS: Among pregnant women (n = 61) in the Mother and Infant NuTrition (MINT) cohort, TAT and VAT wer

Analysis of factors influencing hemorrhagic fever with renal syndrome and its prediction in Weifang, China from 2013 to 2021.

BACKGROUND: This study aimed to analyze the epidemiology and trends of hemorrhagic fever with renal syndrome (HFRS) in Weifang, China (2013-2021) and to guide prevention strategies. METHODS: The study examined the prevalence and incidence trends of HFRS in Weifang (2013-2021). Spearman correlation and wavelet analysis were employed to explore variable relationships and their associations with HFRS incidence. Generalized additive models (GAMs) were used to identify key risk factors, while structural equation modeling (SEM) quantified direct and indirect pathways influencing HFRS transmission.

Exposure to aircraft noise exacerbates cardiovascular and oxidative damage in three mouse models of diabetes.

AIMS: Epidemiology links noise to increased risk of metabolic diseases like diabetes and obesity. Translational studies in humans and experimental animals showed that noise causes reactive oxygen species (ROS)-mediated cardiovascular damage. The interaction between noise and diabetes, specifically potential additive adverse effects, remains to be determined. METHODS AND RESULTS: C57BL/6 mice were treated with streptozotocin (i.p. injections, 50 mg/kg/day for 5 days) to induce type 1 diabetes mellitus, with S961 (subcutaneous osmotic mini-pumps, 0.57 mg/kg/day for 7 days) or fed a high-fat die

Temporal, Seasonal, and Patient Demographic Trends Among Culture-Confirmed Human Salmonellosis Cases Identify Regional Differences in Salmonella Serotypes Reported in Virginia, USA From 2012 through 2022.

The U.S. federal government established a national goal of reducing the salmonellosis incidence to ≤11.5 cases/100,000 persons by 2030. To assess progress toward this goal in Virginia, we used data from culture-confirmed salmonellosis cases reported to the Virginia Department of Health during 2012-2022 (N = 11,411). A Bayesian negative binomial splines model was implemented to estimate changes in incidence/100,000 persons during 2012-2022 statewide and for specific localities (for example: county or independent city). Incidence varied substantially by locality and over time. Output from modele

Impact of human papillomavirus age-related prevalence and vaccination levels on interpretation of cervical screening modalities: a modelling study.

OBJECTIVE: Cervical screening is a life-saving intervention, which reduces the incidence of and mortality from cervical cancer in the population. Human papillomavirus (HPV) based screening modalities hold unique promise in improving screening accuracy. HPV prevalence varies markedly by age, as does resultant cervical intraepithelial neoplasia (CIN), with higher rates recorded in younger women. With the advent of effective vaccination for HPV drastically reducing prevalence of both HPV and CIN, it is critical to model how the accuracy of different screening approaches varies with age cohort and

Machine learning-driven survival prediction in gestational trophoblastic neoplasms: a focus on PSTT and ETT prognosis.

INTRODUCTION: The clinicopathological characteristics and prognosis of placental site trophoblastic tumor (PSTT) and epithelioid trophoblastic tumor (ETT) have not been well summarized. Consequently, we conducted the largest to date series of samples of both types and employed machine learning (ML) to assess treatment effectiveness and develop accurate prognostic models for patients with GTN. Gestational choriocarcinoma (GCC) was used as the control group to show the clinical features of PTSS and ETT. METHODS: The Surveillance, Epidemiology, and End Results (SEER) database provided the data u

Investigating the dose-dependency of the midgut escape barrier using a mechanistic model of within-mosquito dengue virus population dynamics.

Arboviruses can emerge rapidly and cause explosive epidemics of severe disease. Some of the most epidemiologically important arboviruses, including dengue virus (DENV), Zika virus (ZIKV), Chikungunya (CHIKV) and yellow fever virus (YFV), are transmitted by Aedes mosquitoes, most notably Aedes aegypti and Aedes albopictus. After a mosquito blood feeds on an infected host, virus enters the midgut and infects the midgut epithelium. The virus must then overcome a series of barriers before reaching the mosquito saliva and being transmitted to a new host. The virus must escape from the midgut (known

Evaluating the health and health economic impact of the COVID-19 pandemic on delayed cancer care in Belgium: A Markov model study protocol.

INTRODUCTION: Cancer causes a substantial burden to our society, both from a health and an economic perspective. To improve cancer patient outcomes and lower society expenses, early diagnosis and timely treatment are essential. The recent COVID-19 crisis has disrupted the care trajectory of cancer patients, which may affect their prognosis in a potentially negative way. The purpose of this paper is to present a flexible decision-analytic Markov model methodology allowing the evaluation of the impact of delayed cancer care caused by the COVID-19 pandemic in Belgium which can be used by research

Proposal for a descriptive and differentiated presentation of the longitudinal impact of the new organized cancer screening guideline and HPV vaccination in Germany.

INTRODUCTION: Since 01/01/2020, the cervical cancer screening in Germany has been carried out due to the organized early cancer diagnosis guideline (oKFE-RL). In 2007, HPV vaccination was initiated in Germany. The main goal of both initiatives is to further reduce the incidence of invasive cervical cancer. To assess the effect of the new screening strategy in a timely manner, monitoring of short-term changes need to be considered. Ideally, the effects of both prevention methods would be presented together in one model. MATERIALS AND METHODS: Because no change in the incidence of invasive cerv

Optimal Timing for Cancer Screening and Adaptive Surveillance Using Mathematical Modeling.

Cancer screening and early detection efforts have been partially successful in reducing incidence and mortality, but many improvements are needed. Although current medical practice is informed by epidemiologic studies and experts, the decisions for guidelines are ultimately ad hoc. We propose here that quantitative optimization of protocols can potentially increase screening success and reduce overdiagnosis. Mathematical modeling of the stochastic process of cancer evolution can be used to derive and optimize the timing of clinical screens so that the probability is maximal that a patient is s

Downregulation of metabotropic glutamate receptor 5 inhibits hepatoma development in a neurotoxin rotenone-induced Parkinson's disease model.

Clinical epidemiological studies have shown that there is a link between Parkinson's disease (PD) and cancer, but how PD regulates cancer development remains unknown. In our study, the effect of metabotropic glutamate receptor 5 (mGlu5) on hepatoma was explored in a rotenone-induced PD model both in vitro and in vivo. We found that conditioned media derived from MN9D dopaminergic neuronal cells by rotenone-induced toxicity inhibited the growth, migration, invasion and promoted apoptosis of Hepa1-6 cells, which corresponded with decreased expression of mGlu5. Furthermore, treatment with 2-methy

Clinical Score to Differentiate Scrub Typhus and Dengue: A Tool to Differentiate Scrub Typhus and Dengue.

BACKGROUND: Dengue and scrub typhus share similar clinical and epidemiological features, and are difficult to differentiate at initial presentation. Many places are endemic to both these infections where they comprise the majority of acute undifferentiated febrile illnesses. MATERIALS AND METHODS: We aimed to develop a score that can differentiate scrub typhus from dengue. In this cross-sectional study, 188 cases of scrub typhus and 201 cases of dengue infection who presented to the emergency department or medicine outpatient clinic from September 2012 to April 2013 were included. Univariate

Estimation and Short-Term Prediction of the Course of the HIV Epidemic Using Demographic and Health Survey Methodology-Like Data.

BACKGROUND: Mathematical models have played important roles in the understanding of epidemics and in the study of the impacts of various behavioral or medical measures. However, modeling accurately the future spread of an epidemic requires context-specific parameters that are difficult to estimate because of lack of data. Our objective is to propose a methodology to estimate context-specific parameters using Demographic and Health Survey (DHS)-like data that can be used in mathematical modeling of short-term HIV spreading. METHODS AND FINDINGS: The model splits the population according to sex

Reconstruction of the natural history of metastatic cancer and assessment of the effects of surgery: Gompertzian growth of the primary tumor.

This work deals with retrospective reconstruction of the individual natural history of solid cancer and assessment of the effects of treatment on metastatic progression. This is achieved through a mathematical model of cancer progression accounting for the growth of the primary tumor, shedding of metastases, their dormancy and growth at secondary sites. To describe dynamics of the primary tumor, we used the Gompertz law, a parsimonious model of tumor growth accounting for its saturation. Parameters of the model were estimated from the age and volume of the primary tumor at surgery and volumes

Latency estimation for chronic disease risk: a damped exponential weighting model.

Identifying the susceptible period when environmental factors affect disease risk is essential for understanding disease etiology. Most existing epidemiologic studies use oversimplified summaries of time-dependent exposures such as baseline or most current exposure, or the cumulative average of exposure over available follow-up periods. In this paper, we introduce a damped exponential weighting model for estimating optimal exposure weights for different time intervals. This model can accommodate flexible patterns of weights and can be fit using standard software. We applied the model to assess
