Modulation of ipsilateral motor cortex in man during unimanual finger movements of different complexities.

To understand the role of the ipsilateral motor cortex in the control of unimanual movements, we evaluated changes in cortical motor evoked potentials (MEP) from the left abductor pollici brevis (APB) to transcranial magnetic stimulation (TMS) of the right hemisphere in nine normal subjects during execution of right finger movements of different complexities. The motor tasks were (a) repetitive opposition movement (thumb tapping the 3rd finger); (b) isolated finger movements in a 'usual' sequence (thumb tapping fingers 2, 3, 4 and 5) and (c) in an 'unusual' sequence (thumb tapping fingers 3, 5…

Hereditary chin trembling or hereditary chin myoclonus?

Hereditary chin trembling is a rare autosomal dominant disease often considered as an "essential tremor variant". The clinical and neurophysiological data obtained in a new white family lead to the suggestion that this abnormal involuntary movement is a focal variant of hereditary essential myoclonus.

Do visual neurophysiological tests reflect magnocellular deficit in dyslexic children?

To address the question of a possible magnocellular visual deficit in children with reading problems (dyslexia), we examined pattern ERG and VEP responses to stimulation with checks of 24', 49' and 180' in size and of 5%, 42% and 100% contrast level. Neurophysiological difference between children with reading problems and those without them was found confined to VEP which showed a significant prolongation of P100 wave in dyslexic children at highest contrast (100%) and smallest checks (24'). Pattern ERG was normal. These results support the assumption of a visual deficit in dyslexic children.

[Regeneration of the cauda equina after transection and end-to-end anastomosis in the dog].

This study was designed to test surgical techniques for reconstruction of an injured cauda equina. After transection of the intradural lumbar nerve root, end-to-end anastomosis was performed under surgical microscope in the dog. To preserve accurate adaptation of the transected nerve root, the root sheath and the funiculi were carefully sutured with a minimal number of stitches. Regenerated axons in the ventral root appeared distal from the suture at five weeks postoperatively, whereas those in the dorsal root appeared at seven weeks after anastomosis. The histogram and regrowth rate of the ve

Statomotor system in the marine mollusk Clione limacina.

1. In the marine mollusk Clione limacina the "statomotor system" (named by analogy with the oculomotor system) has been found. This system includes a muscle that is directly attached to the statocysts connecting them with each other and with the inner surface of the body. 2. The statocyst muscle consists of four electrically coupled, mononuclear cells. Statocyst muscle cells do not generate spike-like potentials but only excitatory junctional potentials. 3. The motor input to the statocyst muscle correlates with the activity of the locomotor generator. This suggests that in the soft-bodied Cli

Multiple sclerosis in children: report of two cases.

Multiple sclerosis is a disease of white matter in the central nervous system characterized by clinically relapsing and remitting course. It is rare in children and the mean age of onset is about 30 years. To establish diagnosis, one must note that white matter lesions are disseminated in time and place. We report two female patients with recurrent episodes of headaches, seizures, hemiparesis, and optic neuritis since childhood. Four of the five CT scans done during the relapse attacks in these two patients showed negative findings. The only one abnormal finding was decreased density in the wh

Magnetisation transfer ratios and transverse magnetisation decay curves in optic neuritis: correlation with clinical findings and electrophysiology.

Conventional MRI sequences do not permit the distinction between the different pathological characteristics (oedema, demyelination, gliosis, axonal loss) of the multiple sclerosis plaque. Magnetisation transfer imaging and transverse magnetisation decay curve (tMDC) analysis may be more specific. These techniques have been applied to the optic nerves in 20 patients with optic neuritis and the results correlated with clinical and visual evoked potential (VEP) findings. tMDC analysis failed to identify separate intracellular and extracellular water compartments within the optic nerve but gave a

Medial femoral cutaneous nerve conduction.

Medial femoral cutaneous nerve (MFCN), a sensory branch of the femoral nerve, supplies the skin on the anterior medial aspect of the thigh and knee. A conduction study for this nerve is described. A recording electrode was placed anterior-medially in the thigh and 14 cm distal to the stimulating cathode that was placed immediately lateral to the femoral artery just below the inguinal ligament. Sixty-four nerves of 32 adults with a mean age of 40 yr (range, 21-56) were tested. The latencies to the onset and peak were 2.4 +/- 0.2 and 2.9 +/- 0.2 ms, respectively. The amplitude of baseline to neg

Cytoarchitecture and responsiveness of the medial ansate region of the cat primary somatosensory cortex.

To understand its relationship to somatosensory areas in other species, we studied the rostral bank of the medial ansate sulcus in adult cats. Neurons in the shoulder and upper part of the sulcal wall responded to low-threshold cutaneous stimuli much like neurons on the crown of the gyrus, whereas neurons in some deeper portions of the sulcus required more intense but innocuous somatic stimuli. Because we found much of the body surface re-represented in this area, we suggest that, besides the representation in area 3b, there is another cutaneous representation of the hindlimb and trunk located

Nerve, plexus and spinal cord: possible targets for non-invasive neuromagnetic measurements in man.

Somatosensory evoked neuromagnetic fields are recorded from peripheral nerves in the upper arm, from plexus brachialis at the ventral and dorsal thorax and from the dorsal horn of the cervical spinal cord ('P13m') at the upper lateral neck. Some perspectives for clinical applications are suggested.

Interpretation of electrocochleography in Menière's disease and normal subjects.

Electrocochleographic recordings were obtained from 20 subjects with normal hearing and 10 subjects with Menière's disease by using an eardrum electrode. Stimuli included clicks and 6,000-Hz tone bursts. Results were not significantly different between the two groups for summating potential (SP) amplitude, action potential (AP) amplitude, or the SP/AP amplitude ratio. Interpreting the results in light of symptoms on the date of assessment or hearing threshold did not appear to improve separation between the two groups. Various SP/AP amplitude ratio criteria have been proposed in order to separ

Neurodiagnostic testing in patients with closed head injury.

This brief review summarizes the current understanding of mild closed head injury with postconcussion syndrome. It is to be emphasized that objective evaluation be performed because it is critical to the assessment of the patient. Increased awareness of patients with symptoms in association with heightened sensitivity and caring will go a long way in helping the millions of patients yearly with mild to moderate head injury. Only with objective scientific data will the understanding of the causes of the varied symptomatology of the postconcussion syndrome be thoroughly understood.

Brainstem auditory evoked responses during the recovery of Wernicke's disease.

Serial brainstem auditory evoked responses (BAERs) during the acute and recovery phase of Wernicke's disease were studied in two patients. Both morphologic abnormalities and prolongation of absolute and interpeak latencies of wave I through V were noted during the acute and recovery periods. Normalization was found in one patient and incremental improvements in the other. This study suggests that the BAER may provide documental evidence of auditory tracts involvement in Wernicke's disease. The diagnostic value of the BAER is for establishing the presence of brainstem involvement when features

Effects of famotidine at the mouse neuromuscular junction compared with those of cimetidine and ranitidine.

The effects of some histamine H2-receptor antagonists on the cholinergic system have been evaluated at the mouse end-plate. Previous data revealed interactions at both pre- and postsynaptic sites for cimetidine and ranitidine. The present work shows the effects of the same two drugs, and of famotidine, on some parameters related to the neuromuscular transmission. Ranitidine potentiates the amplitude of the spontaneously occurring miniature end-plate potentials. Famotidine reduces the same parameter. A common effect achieved with higher concentrations of the three drugs is the reduction of the

Neonatal seizures: a commentary on selected aspects.

Despite the immaturity of the newborn brain, the neonatal period is reported to have a very high frequency of seizures. This review concludes that many of the neonatal events that are called seizures probably originate from subcortical structures, have little in common with cortical seizures seen in older individuals, and may not benefit from conventional anticonvulsant treatment. Many studies of anticonvulsants in the newborn have important methodologic problems, compounded by the fact that the seizures tend to spontaneously remit with the resolution of the acute hypoxic-ischemic encephalopat
