Scaling surgical care capacity in Nigeria: a narrative review of current practices and innovations in rural surgical workforce development.

BACKGROUND: Low- and middle- income countries face a surgical workforce shortage amidst a high burden of diseases amenable to surgery. Rural Nigeria, which is mainly supported by primary health care, lacks access to timely, affordable and safe surgical care, leading to high morbidity and mortality. This review aims to assess the burden of lack of personnel, evaluate current innovative strategies, and provide future directions for rural surgical workforce development in Nigeria. METHODS: A total of 79 articles were included in this review. A comprehensive literature search was conducted in Pub

Value-Based Pricing of Resmetirom for Metabolic Dysfunction-Associated Steatotic Liver Disease.

IMPORTANCE: Resmetirom can slow fibrosis progression and improve resolution of metabolic dysfunction-associated steatohepatitis (MASH). As the first Food and Drug Administration-approved pharmacologic treatment for MASH and fibrosis stages F2 and F3, resmetirom is very expensive, potentially creating a financial barrier to patient access. OBJECTIVE: To estimate the cost-effectiveness of resmetirom vs standard of care (SoC) over a lifetime and to determine a price threshold at which resmetirom would be cost-effective. DESIGN, SETTING, AND PARTICIPANTS: This economic evaluation used an agent-b

Real-World Evidence for Faricimab in Neovascular Age-Related Macular Degeneration and Diabetic Macular Edema: A Scoping Review.

PURPOSE: Since faricimab (Vabysmo) was approved for the treatment of neovascular age-related macular degeneration (nAMD) and diabetic macular edema (DME), a growing body of real-world data has been reported, forming an important source of evidence for faricimab in a heterogeneous population. Scoping reviews are an effective approach to comprehensively assess the state of evidence on areas yet to be well characterized, allowing for the inclusion of a wide range of study designs and methodologies. This scoping review aimed to assess the current breadth and nature of real-world evidence (RWE) for

Cost Comparison and Spending on Tobacco Products: Evidence From A Nationally Representative Sample of Adult E-cigarette Users.

INTRODUCTION: Over 20 states and local jurisdictions in the United States have imposed e-cigarette taxes. It is important to evaluate how adult vapers, including those who also smoke respond to e-cigarette taxation. The purpose of this study is to examine factors associated with adult vapers' cost comparison between e-cigarettes and cigarettes (ie, which one is perceived to be more expensive) and expenditures (ie, dollars spent) on these two products. AIMS AND METHODS: We recruited a nationally representative sample of 801 adult e-cigarette users in the United States., who participated in an

Deep Learning with Disc Photos or OCT Scans in Glaucoma Detection.

OBJECTIVE: To determine whether a deep learning (DL) model using retinal nerve fiber layer thickness (RNFLT) maps from OCT scans can detect glaucoma, defined by functional visual field (VF) impairment, more accurately than a DL model using disc photos (DPs). A secondary objective was to assess the diagnostic performance of these DL models across demographic groups (race, sex, and ethnicity). DESIGN: Retrospective cohort study at a tertiary glaucoma center utilizing OCT and DP datasets collected between 2011 and 2022. PARTICIPANTS: Out of the 16 936 DP and OCT image sets, patients with Cirrus

Hinge craniotomy versus decompressive craniectomy for the neurosurgical management of traumatic brain injury and stroke: A systematic review and meta-analysis.

BACKGROUND: Traumatic brain injury (TBI) and stroke are major contributors to global morbidity and mortality, particularly in low- and middle-income countries. Decompressive craniectomy (DC) is an established neurosurgical intervention for reducing elevated intracranial pressure and preventing secondary neurological injury in the context of trauma and stroke. However, DC is a financially cumbersome two-stage procedure that requires specialised infrastructure, post-operative intensive care, and access to delayed cranioplasty, rendering it largely inaccessible to patients in resource-limited set

Expressed Emotions in Families of Patients With Bipolar Disorder.

Introduction Bipolar affective disorder (BPAD) is a chronic psychiatric illness characterized by recurrent episodes of mania, hypomania, and depression, significantly impacting both patients and their caregivers. Expressed emotion (EE), which includes criticism, hostility, and emotional over-involvement, plays a crucial role in caregiver burden and patient outcomes. High EE environments have been linked to increased relapse rates and hospitalization, while low EE settings promote better adherence and stability. In India, where mental healthcare is predominantly family-driven, caregivers experi

A systematic review of cost-effectiveness analyses of gene therapy for hemophilia type A and B.

BACKGROUND: In 2022-2023, the US Food and Drug Administration approved 2 novel gene therapies, valoctocogene roxaparvovec and etranacogene dezaparavovec, for hemophilia A and B, respectively. These one-time-administered gene therapies have been marketed at prices that create financial challenges for payers and patients. Understanding the magnitude and uncertainties around the long-term value of these therapies and how they can potentially relate to managed care practices is of high interest to the payer and patient community. OBJECTIVE: To conduct a systematic review of cost-effectiveness ana

Oversight May Reduce Military Health System Leakage: Preventing Inappropriate Interhospital Transfers.

INTRODUCTION: The Military Health System (MHS) offers an example of a socialized healthcare model, operating within a larger "purchased care" civilian healthcare market. This arrangement has facilitated a trend wherein MHS clinicians often transfer moderate-to-complex patients to surrounding civilian hospitals, despite having the capability to care for such patients in-house. In an effort to stem this behavior, two initiatives were introduced at Carl R Darnall Army Medical Center (CRDAMC): A Transfer Policy Statement and Transfer Rounds. The Transfer Policy Statement emphasized that patients o

Digitally-supported patient-centered asynchronous outpatient follow-up in rheumatoid arthritis - an explorative qualitative study.

OBJECTIVE: A steadily increasing demand and decreasing number of rheumatologists push current rheumatology care to its limits. Long travel times and poor accessibility of rheumatologists present particular challenges for patients. Need-adapted, digitally supported, patient-centered and flexible models of care could contribute to maintaining high-quality patient care. This qualitative study was embedded in a randomized controlled trial (TELERA) investigating a new model of care consisting of the use of a medical app for ePRO (electronic patient-reported outcomes), a self-administered CRP (C-rea

Efficient stabilization of therapeutic hepatitis B vaccine components by amino-acid formulation maintains its potential to break immune tolerance.

BACKGROUND & AIMS: Induction of potent, HBV-specific immune responses is crucial to control and finally cure HBV. The therapeutic hepatitis B vaccine TherVacB combines protein priming with a Modified Vaccinia virus Ankara (MVA)-vector boost to break immune tolerance in chronic HBV infection. Particulate protein and vector vaccine components, however, require a constant cooling chain for storage and transport, posing logistic and financial challenges to vaccine applications. We aimed to identify an optimal formulation to maintain stability and immunogenicity of the protein and vector components

Chance of live birth: a nationwide, registry-based cohort study.

STUDY QUESTION: Does the sequence of prior pregnancy events (pregnancy losses, live births, ectopic pregnancies, molar pregnancy and still birth), obstetric complications and maternal age affect chance of live birth in the next pregnancy and are prior events predictive for the outcome? SUMMARY ANSWER: The sequence of pregnancy outcomes is significantly associated with chance of live birth; however, pregnancy history and age are insufficient to predict the outcome of an individual woman's next pregnancy. WHAT IS KNOWN ALREADY: Adverse pregnancy outcomes decrease the chance of live birth in th

A proposed bio-panel to predict risk for spontaneous preterm birth among African American women.

Preterm birth (PTB), or birth prior to 37 weeks gestation, impacts 11.5% of U.S. deliveries. PTB results in significant morbidity and mortality among affected children and imposes a large societal financial burden. Racial disparities in PTB are alarming. African American women are at more than 1.5 times the risk for PTB than white women. Unfortunately, the medical community's ability to predict who is at risk for PTB is extremely limited. History of a prior PTB remains the strongest predictor during a singleton gestation. Cervical length and fetal fibronectin measurement are helpful tools. How

Effect of serum nicotine level on posterior spinal fusion in an in vivo rabbit model.

BACKGROUND CONTEXT: Cigarette smoking has a deleterious effect on spinal fusion. Although some studies have implied that nicotine is primarily responsible for poor fusion outcomes, other studies suggest that nicotine may actually stimulate bone growth. Hence, there may be a dose-dependent effect of nicotine on posterior spinal fusion outcomes. PURPOSE: The purpose of this study was to determine if such a relationship could be shown in an in vivo rabbit model. STUDY DESIGN/SETTING: This is a prospective in vivo animal study. METHODS: Twenty-four adult male New Zealand white rabbits were rand
