[A case of syphilitic uveitis].

A 43-year-old woman with alcoholism developed hearing loss, dizziness, unsteady walking, frequent urination, memory disorders, and later a manifest drop of bilateral vision. The patient denied syphilis. Examinations revealed bilateral panuveitis, bilateral neurosensory amblyacusia, vestibulopathy, and imperative urges to urination. Serological tests were positive with both Treponema and other than Treponema antigens. The cerebrospinal fluid was normal. Ampicillin therapy was ineffective. The clinical picture, diagnosis, and treatment of syphilitic uveitis are discussed; the authors point out t

[Hypersomnia in multiple sclerosis].

A 13-year-old girl presented with hyersomnia and incontinence of urine. The clinical course was complicated by strabism, muscle weakness and coordination and balance disturbances. VEP recording were pathologic. Myelin basic protein in the cerebrospinal fluid was elevated. Nuclear resonance imaging of the brain revealed multiple areas of demyelination. These findings supported the diagnosis of multiple sclerosis. There was a marked disturbance of the sleep-waking-pattern during the acute phase of the illness. The hypnogram resembled the pattern of polysymptomatic narcolepsy. Regression of sympt

[Antihypertensive therapy in renal failure].

Hypertension can be a cause as well as a consequence of renal disease. Fluid overload or activation of the renin angiotensin system by glomerular ischemia leads to an elevation of systemic blood pressure in acute as well as chronic renal disease. Furthermore in the latter disorder an increased activity of the sympathetic nervous system as well as metabolic disorders contribute to hypertension. Accordingly antihypertensive therapy in patients with renal insufficiency is diverse. Additionally in chronic kidney disease one has to consider the variable effects of different antihypertensive drugs o

New perspectives in the management of adults with severe head injury.

Traditional treatment options for the adult with severe head injury have included hyperventilation, elevation of the head of the bed, sedation, chemical paralysis, fluid restriction, diuresis, anticonvulsant therapy, and barbiturate coma. These interventions for severe head injury are reviewed and updated with current research. Neuroprotective drugs (including glutamate antagonists, oxygen free radical scavengers, and high dose steroids) are introduced, along with monitoring of jugular venous oxygen saturation and the role of the transcranial Doppler. The role of the critical care nurse in the

Nosocomial epiglottitis associated with penicillin- and cephalosporin-resistant Streptococcus pneumoniae bacteremia.

We cared for a 4-year-old male with nosocomially acquired epiglottitis caused by Streptococcus pneumoniae. He had been receiving ceftazidime therapy when this infection was recognized. The S. pneumoniae isolate was of serotype 15B and was resistant to beta-lactam antibiotics, cephalosporins (including those with extended spectra), and trimethoprim-sulfamethoxazole. Clinicians and clinical microbiologists must be aware that cephalosporin susceptibility may no longer be assumed for penicillin-resistant S. pneumoniae isolates and that susceptibility testing for the extended-spectrum cephalosporin

Cryptococcal meningitis with severe visual and hearing loss and radiculopathy in a patient without immunodeficiency.

Cryptococcosis is an important cause of lymphocytic meningitis, especially but not necessarily in immunocompromised patients. We present the case of a 23-year-old man with a severe and rapid course of a cryptococcal meningoencephalitis, which led to visual and hearing loss, psychotic illness and radiculopathy. There was no evidence of immunodeficiency. Treatment with amphotericin B and flucytosine led to improvement of the symptoms but did not eradicate the micro-organisms from the cerebrospinal fluid (CSF). Maintenance therapy with fluconazole was necessary and led to improvement of the CSF p

[Mycotic sinusitis. Contribution of x-ray computed tomography].

The authors present three cases of fungal sinusitis. Fungal sinusitis is a relatively rare disease that affects more commonly immunosuppressed and diabetic individuals. The radiographic signs are non-specific. Nevertheless, some CT findings are highly suspicious: unilateral lesions of one or more sinuses; nodular mucoperiosteal thickening, and, or focal areas of bone destruction; very dense, intrasinus concretions. Air-fluid levels are very uncommon, and when present suggest a nonfungal bacterial infection. In most cases, a combination of these findings suggests fungal disease. The clinical ha

Clinical, ultrasonographic and CT evaluation of orbital rhabdomyosarcomas with management.

Fourteen cases of orbital rhabdomyosarcoma have been studied. Clinical presentation was by way of short history, fast growing proptosis, inflammatory signs and epistaxis. Computerised tomography showed only a mass lesion without bony involvement and was very useful in follow up. Ultrasonography revealed pseudo-encapsulation of the tumour which is postulated as possibly specific of Rhabdomyosarcoma due to its fast growth, cellular content and oedema fluid. A survival rate of 21% over a 3 year period has been achieved by combined mode therapy consisting of surgery, radiation and chemotherapy.

Treatment of cardiovascular disease in cattle.

Cardiac diseases of cattle may involve valvular structures, myocardium, pericardium, or blood vessels and are manifested by the clinical signs of cardiac dysrhythmias, cardiac murmurs, generalized edema, muffled heart sounds, jugular venous distention, jugular venous pulsations, pulmonary edema, pleural effusion, or ascites. Digoxin, quinidine, and furosemide can be used effectively to control signs of CHF and cardiac arrhythmias. Combination antimicrobial therapy can be successful for cows with infective endocarditis and thrombophlebitis. Pericardial fluid drainage may temporarily improve cat

First report of chronic meningitis caused by Trichosporon beigelii.

Trichosporon beigelii (Trichosporon cutaneum) was identified as the causative agent of chronic meningitis in a 15-year-old boy with acute lymphocytic leukaemia. After a neutropenic episode following cytostatic treatment and itraconazole therapy as prophylaxis, cerebrospinal fluid (CSF) samples yielded growth of Trichosporon beigelii. Treatment with amphotericin B, flucytosine and high doses of fluconazole was followed by clinical improvement, although CSF pleocytosis remained. The cross-reactivity between Cryptococcus neoformans and Trichosporon beigelii in a cryptococcal antigen latex test wa

Treatment of severe cryptosporidium-related diarrhea with octreotide in a patient with AIDS.

Cryptosporidiosis commonly causes severe diarrhea in immunosuppressed patients. There currently are no antiparasitic drugs consistently effective for this infection. This case describes a 26-year-old hemophiliac patient with acquired immunodeficiency syndrome and cryptosporidiosis whose diarrhea improved with continuous intravenous administration of a long-acting somatostatin analog, octreotide. Somatostatin has a variety of inhibitory effects on gastrointestinal hormones as well as a possible nonspecific effect on gastrointestinal mucosal fluid and electrolyte secretion. The somatostatin anal

[Embryopathic-dysontogenetic syndromes in pathocharacterological disorders in the progeny of alcoholic parents].

In a clinical and follow-up study of the progeny of alcoholic parents the author investigated embryopathic/dysontogenetic disorders ("oligosymptomatic" forms of alcoholic embryopathy, mental infantilism, hyperdynamic syndrome and the syndrome of cerebrospinal fluid hypertension) exerting a pathoplastic effect on the deviant behaviour of adolescents. The author has developed approaches toward a multiple modality therapy and rehabilitation of these patients in conditions of a special school for adolescents with deviant behaviour. A hypothesis is advanced as to the relationship between embryopath

Systemic infections with Trichosporon beigelii (cutaneum). Report of three new cases.

Three new cases of systemic mycosis due to Trichosporon cutaneum are reported and compared with the 23 previous reports. Two patients had acute leukemia and one patient had a lymphoblastic lymphoma. Blood cultures in two patients and cerebrospinal fluid in the third patient were positive for T. cutaneum. Only one patient recovered after antimycotic therapy and concomitant remission of his leukemia. At autopsy, the two other patients showed widespread infection with T. cutaneum. The authors conclude that diagnosis and management of such infection in the immunosuppressed host are difficult and t

Velopharyngeal insufficiency after adenoidectomy: an 8-year review.

The characteristics of 137 patients referred to the Speech Clinic at The Hospital for Sick Children, Toronto, Canada during the past 8 years for the investigation of persistent hypernasality after adenoidectomy are reported. Over 30% of the patients had preoperative factors that are thought to increase the risk, such as submucous cleft palate, fluid regurgitation through the nose, a family history of velopharyngeal insufficiency or clefting, or hypernasality. Fifty percent of the patients required pharyngoplasty to correct hypernasality, 37% required speech therapy alone, and 13% improved with

Demonstration of uronic acid capsular material in the cerebrospinal fluid of a patient with meningitis caused by mucoid Pseudomonas aeruginosa.

A 39-year-old female with insulin-dependent diabetes mellitus developed Rhizopus infection of the maxillary sinus. Subsequent to successful treatment with amphotericin B and surgical debridement, she developed purulent meningitis due to a mucoid strain of Pseudomonas aeruginosa. Analysis of cerebrospinal fluid documented the presence of a uronic acid polymer at a concentration of 40 micrograms/ml. In spite of parenteral and intrathecal antibiotic therapy, the patient died. This case illustrates that mucoid strains of P. aeruginosa may result in fatal infection and that alginate capsule is prod

Tuberculoma mimicking a pinealoma. Case report.

Computerized tomography (CT) revealed obstructive hydrocephalus and a pineal mass in a 14-year-old girl who presented with headaches and a Parinaud's syndrome. Although there was no major evidence of extracranial tuberculosis, and cerebrospinal fluid obtained during third ventriculostomy contained no leukocytes, suboccipital transtentorial biopsy of the lesion revealed it to be a tuberculoma. Serial CT scans showed resolution of the lesion following subtotal resection and antituberculous therapy. The implications of this case with regard to difficulties in the diagnosis of cerebral tuberculoma

Neutropenic typhlitis. A plea for conservatism.

Neutropenic typhlitis is an acute inflammation of the cecum occurring in neutropenic patients. Two cases are described, both presenting typical features of the disease. It is our experience that the treatment approach should consist of bowel rest, fluid therapy, and massive administration of broad spectrum antibiotics. In some cases, full recovery will follow. If abdominal signs persist, surgery should be deferred until hematologic convalescence occurs. At this time, cecostomy and drainage should be adequate. Only in very severe cases, with wide-spread necrosis, should a right hemicolectomy be…

Reproductive parameters in prolactinaemic men.

The sexual behavior, hypothalamic-pituitary-testicular axis and seminal fluid parameters in three different models of hyperprolactinaemia were studied: patients with prolactin secreting pituitary adenomas, normal volunteers before and during metoclopramide, and psychotic patients on chronic neuroleptic therapy. Hyperprolactinaemia is always accompanied by a reduced seminal volume but there is no clearcut evidence that it can induce spermatogenesis modifications. Impotence is constantly present in the hyperprolactinaemic syndromes. The mildly impaired androgen production by the Leydig cells dur

Surgical monitoring of spinal cord function: cauda equina stimulation technique.

Spinal cord and subcortical brain stem evoked potentials had an amplitude at least 2 times higher when the cauda equina rather than bilateral peripheral nerves was stimulated. Cauda equina stimulation is indicated when potentials to peripheral nerve stimulation are absent or are too low in amplitude to permit reliable surgical monitoring. The technique is essentially without risks, but should be performed with a small lumbar puncture needle (21 to 22 gauge), and is contraindicated in patients with general infections, increased cerebrospinal fluid pressure, or a hemorrhagic tendency (thrombocyt

Bacterial prostatitis caused by Staphylococcus saprophyticus.

Staphylococcus saprophyticus has been shown to be an important uropathogen in urinary tract infections in young women. Thirty-five isolates from 27 patients with staphylococci in the prostatic fluid of men with bacterial prostatitis were evaluated for the presence of S. saprophyticus. Three patients (11 per cent) with this organism were identified by novobiocin resistance (disk diffusion test), absence of hemolysis, and coagulase. These patients tended to be younger, more symptomatic, and more responsive to appropriate antibiotic therapy than those with staphylococcus epidermidis. S. saprophyt
