BACKGROUND: Aim of our study was to evaluate the use of Locking Compression Plate (LCP) in patients with long standing nonunions of the clavicle with osteoporotic bones.
MATERIAL AND METHODS: There were a total of 11 patients. Surgery was performed for painful nonunion with shoulder dysfunction. Average time since injury was 6.9 months (3-39 months). Nonunions were stabilized by a 3.5-mm LCP, applied on the antero-inferior surface of the clavicular shaft. A six-hole LCP was used in eight cases. The results following nonoperative treatment of displaced, medial end clavicle fractures is often unsatisfactory; but no study has yet reported the outcome of operative fixation of these fractures. This study reports the results of open reduction and internal fixation on displaced, medial end clavicle fractures, in five adult patients (aged 25-52 years, mean 43) including 1 patient with a nonunion. This paper reports an isolated dorsal fracture-dislocation of the scaphoid at its waist with the proximal fragment dislocated dorsally. Such a fracture-dislocation is extremely rare. We believe the pathomechanics of this injury to have been a flexion and radial deviation with an axial force on the wrist.

PURPOSE OF THE STUDY: Fractures of a tibial pylon are serious problems in traumatology. The methods of their therapy have not been unified yet and treatment outcomes are not satisfactory. This is often due to extensive comminution of the distal tibia, bone defects or a thin coverage of soft tissue. The objective of this study is to compare the literature data and, on the basis of this and our own experience, to present guidelines for the treatment of fractures of tibial pylons.

Malignant hyperthermia (MH) is a potentially fatal disease triggered by general anaesthesia. Four cases of compartment syndrome complicating MH have been reported. We report here a case of transient talipes equinus deformity of bilateral lower limbs, a condition similar to compartment syndrome, following MH in a previously healthy pediatric patient. MRI revealed high intensity in the bilateral soleus muscles but not in the gastrocnemius muscles. We discuss the possible mechanisms of the deformity of the lower limbs after the MH crisis.

We recommend a treatment algorithm for tibial eminence fracture management (Fig.11). Displaced and irreducible fractures require arthroscopic or open treatment based on surgeon preference. Objective sagittal plane laxity does not translate into long-term clinical or subjective instability. Every effort should be made to obtain the best possible reduction with stable fixation, when needed, to maximize function.

Late results of operatively treated malleolar fractures were analyzed. 101 patients were operated on, 58 (57.4%) were available for follow-up. In 1993-1999 in the Traumatological Hospital in Brno 141 syntheses DHS Medin were made in patients with fractures of the proximal femur. (Eighteen DHS syntheses of other manufacturers were also made. OBJECTIVE: To study the surgical approach to the orbital floor and infraorbital rim.
METHOD: 15 cases of fractures of the orbital floor and infraorbital rim were treated with the transconjunctival approach.
RESULT: All fractures of the orbital floor and infraorbital rim were repositioned and fixed through transconjunctival approach. Except for 1 case with mild canthal malposition, no other complication was observed.
CONCLUSION: The transconjunctival approach can provide adequate exposure of the infraorbital rim and orbital floor without scar formation in the face.

Twenty-two displaced talar neck fractures were treated by open reduction and internal fixation. Four open fractures were operated within 8 hours and 18 closed injuries were treated at an average interval of 13.8 hours after injury. The authors have studied one case of pathological fracture of the femoral shaft related to hydatid disease. The diagnosis was based on the presence of whitish vesicles at the surgical procedure. A direct approach to the zygomatic arch can be a useful alternative for primary or secondary deformities in which simple well known technics are ineffective. This approach is very limited because of the anatomical elements located directly over the zygomatic region. The exposure of the fracture site is performed by an unusual surgical approach which allows visualization of both zygomatic arches simultaneously, to correct all deformities and to perform osteosynthesis. Four cases using this technique are reported and the value of this approach is assessed.

Traditional three-point reduction may not restore proper projection of the malar prominence following a fracture dislocation of the zygoma if two of the three anterior points of realignment are comminuted. In such cases, reconstruction of the fourth or posterior projection, the zygomatic arch, increases the accuracy of the multidimensional reconstruction. Although the extended access approach required to repair the arch increases operative time and possibly length of hospitalization, its use is justified by the improved results in selected patients with severe injuries of the zygoma.
