Cardiovascular responses to rhythmic handgrip exercise in heart failure with preserved ejection fraction.

Although the contribution of noncardiac complications to the pathophysiology of heart failure with preserved ejection fraction (HFpEF) have been increasingly recognized, disease-related changes in peripheral vascular control remain poorly understood. We utilized small muscle mass handgrip exercise to concomitantly evaluate exercising muscle blood flow and conduit vessel endothelium-dependent vasodilation in individuals with HFpEF (n = 25) compared with hypertensive controls (HTN) (n = 25). Heart rate (HR), stroke volume (SV), cardiac output (CO), mean arterial pressure (MAP), brachial artery b

Pericardial, But Not Hepatic, Fat by CT Is Associated With CV Outcomes and Structure: The Multi-Ethnic Study of Atherosclerosis.

OBJECTIVES: The study sought to determine the associations between local (pericardial) fat and incident cardiovascular disease (CVD) events and cardiac remodeling independent of markers of overall adiposity. BACKGROUND: The impact of pericardial fat-a local fat depot encasing the heart-on myocardial function and long-term CV prognosis independent of systemic consequences of adiposity or hepatic fat is an area of active debate. METHODS: We studied 4,234 participants enrolled in the MESA (Multi-Ethnic Study of Atherosclerosis) study with concomitant cardiac magnetic resonance imaging and compu

Seizure-Induced Sympathoexcitation Is Caused by Activation of Glutamatergic Receptors in RVLM That Also Causes Proarrhythmogenic Changes Mediated by PACAP and Microglia in Rats.

UNLABELLED: Cardiovascular autonomic dysfunction in seizure is a major cause of sudden unexpected death in epilepsy. The catecholaminergic neurons in the rostral ventrolateral medulla (RVLM) maintain sympathetic vasomotor tone and blood pressure through their direct excitatory projections to the intermediolateral (IML) cell column. Glutamate, the principal excitatory neurotransmitter in brain, is increased in seizures. Pituitary adenylate cyclase activating polypeptide (PACAP) is an excitatory neuropeptide with neuroprotective properties, whereas microglia are key players in inflammatory respo

Clinical outcome after triple-valve operations in the modern era: are elderly patients at increased surgical risk?

BACKGROUND: Despite modern advances in surgical care, triple-valve surgery (TVS) remains a challenge and carries a mortality of 10% to 20%. No validated risk score is available for TVS, and the effect of advanced age is unknown. This study examined our results in the modern era with the aim of identifying perioperative predictors of adverse outcomes. METHODS: Between 1997 and 2013, 131 patients (mean age, 67.2±13.4 years) underwent TVS at our institution. Sixty-eight patients (51.9%) were aged 70 years and older. The most common etiology for aortic and mitral disease was degenerative (77.1%),

Feasibility, safety, and intensity of Frame Running for people with multiple sclerosis with moderate-to-severe walking impairments.

PURPOSE: Physical activity is a key symptom management strategy for people with Multiple Sclerosis (pwMS). Yet, pwMS with advanced disability remain less active than the general population, possibly reflecting the dearth of adapted exercise opportunities for this population. This study therefore aimed to evaluate the feasibility of Frame Running, an adapted physical activity, for pwMS with moderate-to-severe walking impairments, its physiological demands, and the feasibility of conducting a definitive trial. METHODS: A single-arm, pre-post design was employed, with physical function and self-

Reference Intervals for Conventional Transthoracic Echocardiography and Two-Dimensional Speckle Tracking Echocardiography-Derived Strain Values in the Dutch Sheepdog ('Schapendoes').

Echocardiographic values can vary between dog breeds, making breed-specific reference intervals (RIs) preferable. Two-dimensional speckle tracking echocardiography (2-D STE) is an advanced imaging technique that enables the measurement of myocardial deformation parameters, contributing to the assessment of systolic function. The objective was to determine breed-specific RIs for 2-D, M-mode, and Doppler-derived echocardiographic parameters for Dutch Sheepdogs, and to obtain 2-D STE-derived strain and strain rate values in this breed. Apparently healthy, purebred Dutch Sheepdogs (1-7 years) were

A mixed-methods study to investigate feasibility and acceptability of an early warning score for preterm infants in neonatal units in Kenya: results of the NEWS-K study : Neonatal early warning scores in Kenya.

Preterm birth (< 37 weeks gestation) complications are the leading cause of neonatal mortality. Early-warning scores (EWS) are charts where vital signs (e.g., temperature, heart rate, respiratory rate) are recorded, triggering action. To evaluate whether a neonatal EWS improves clinical outcomes in low-middle income countries, a randomised trial is needed. Determining whether the use of a neonatal EWS is feasible and acceptable in newborn units, is a prerequisite to conducting a trial. We implemented a neonatal EWS in three newborn units in Kenya. Staff were asked to record infants' vital sign
