In vitro investigation of prosthetic heart valves in magnetic resonance imaging: evaluation of potential hazards.

BACKGROUND AND AIM OF THE STUDY: Magnetic resonance (MR) imaging is used in an increasing number of patients, and not only after cardiac valve replacement. However, ferromagnetic biomedical implants are often considered a contraindication for MR imaging because of the potential hazards with respect to their movement, dislodgement, or heating effects during the procedure. The purpose of this study was to assess ferromagnetism, attraction forces, heating effects, and artifacts associated with prosthetic heart valve implants. METHODS: Seventeen common heart valve prostheses (12 mechanical, five

Management of a patient with a mechanical aortic valve and antibodies to both thrombin and factor V after repeat exposure to fibrin sealant.

We describe a patient who developed a markedly prolonged PT, PTT, and thrombin time 13 days after repeat exposure to fibrin sealant during coronary artery bypass grafting and aortic valve replacement. Evaluation revealed an inhibitor to bovine thrombin that cross-reacted with human thrombin. In addition an inhibitor to human coagulation factor V was identified. Despite coagulation abnormalities there was no evidence of bleeding. Nevertheless, effective anticoagulation was required to minimize the thrombotic complications associated with the patient's prosthetic valve. We elected to take a cons

[The use of the results of current instrumental methods of cardiological examination in the sociomedical expert assessment of patients with rheumatic heart defects].

Altogether 103 patients with rheumatic heart disease were examined within different times after prosthetics of heart valves (mitral or aortal). The purpose was to elaborate the principles of applying the data of instrumental methods of cardiological examination to the improvement of disability criteria. The examination included echocardiography, tetrapolar chest rheography and ECG exercise test. The data allowed one to estimate myocardial reserves and to specify the intensity of functional disorders of the cardiovascular system. The expert medical conclusions on the working ability were revise

Giant right atrial myxoma with rheumatic mitral valve disease.

A patient with a 456 Gram right atrial myxoma is described. Associated rheumatic mitral valvular heart disease obscured the presence of the tumor and only with presentation of bradyarrhythmias leading to pacemaker implantation was the diagnosis suspected. Successful surgical removal of the tumor and correction of the intracardiac valvular dysfunction were performed. The role of echocardiography in the early detection of these tumors is reinforced.

[Severe acute cardiac failure caused by the combination of the aortic stenosis and sudden onset of the mitral chordae rupture].

The patient, who had an aortic stenosis, suddenly complicated with severe acute cardiac failure. The rupture of the mitral chordae tendineae was detected by the echocardiogram. Double replacements (aortic and mitral) were done immediately after the onset of chordae rupture. This condition, combination of aortic stenosis and sudden onset of chordae rupture, always results in severe heart failure which is explained by the combination of pressure and volume loading. Finally the emergent operation is believed to be an only solution for this situation.

Evidence against a myocardial factor as the cause of left ventricular dilation in active rheumatic carditis.

OBJECTIVES: The aim of this study was to determine whether left ventricular dilation and congestive heart failure in patients with acute rheumatic fever with carditis are accompanied by left ventricular contractile dysfunction. BACKGROUND: Acute rheumatic fever with carditis involves both the myocardium and endocardium, with consequent valvular regurgitation. The relative contribution of volume overload induced by valvular regurgitation and myocardial dysfunction due to rheumatic myocarditis to the overall degree of left ventricular dilation and congestive heart failure in these patients is u

Value of echocardiography and nuclear techniques in assessment of valvular heart lesions.

Assessment of the severity of valvular regurgitation is possible by nuclear techniques, but overlap with mild, moderate, or severe degrees of valvular regurgitation has been reported. For the individual patient, if a baseline correlation is established between RNA measurements and catheterization derived measurements, RNA may then be used serially to follow the patients. Controversy still persists in defining the optimum time for aortic valve replacement. We do not recommend aortic valve replacement in an asymptomatic patient whose end-systolic dimension is greater than 55 mm and fractional sh

Valvular heart disease in congenital heart disease: a narrative review.

Patients with congenital heart disease (CHD) are one of the fastest growing populations in cardiology, and valvular pathology is at the center of many congenital lesions. Derangements in valvular embryology lead to several anomalies prone to dysfunction, each with hemodynamic effects that require appropriate surveillance and management. Surgical innovation has provided new treatments that have improved survival in this population, though has also contributed to esotericism in patients who already have unique anatomic and physiologic considerations. Conduit and prosthesis durability are often m

Thrombotic and haemorrhagic complications in patients with mechanical heart valve prostheses attending the Singapore General Hospital Anticoagulation Clinic.

INTRODUCTION: This study evaluated the intensity of anticoagulation required for prevention of thromboembolic episodes with minimal haemorrhagic complications in patients with mechanical heart valve prostheses. MATERIALS AND METHODS: The incidence of thrombotic and haemorrhagic events was retrospectively assessed in a series of 143 patients attending the anticoagulation clinic over a period of 67 months. Risk factors for haemostatic events were also analysed. RESULTS: All embolic events occurred when the International Normalised Ratio (INR) was less than 2.2, suggesting an INR of 2.5 to 3.0

Contemporary trends in mitral valve surgery in East China: a 10-year experience.

BACKGROUND: Contemporary nationwide data from mainland China on trends in mitral valve surgery are scarce. The purpose of the present study was to review a single-center experience with mitral valve surgery over a 10-year period in East China. METHODS: Between July 2009 and June 2019, 3238 consecutive patients who underwent mitral valve surgery in our center were retrospectively reviewed. The patients were evenly divided into three periods: July 2009 to October 2012 (period I), November 2012 to February 2016 (period II), March 2016 to June 2019 (period III). RESULTS: The 3238 patients includ

[A case of quadricuspid aortic valve associated with mitral insufficiency].

We report a case of a 70-year-old woman who had congenital quadricuspid aortic valve. She had undergone Manouguian's operation because of the narrow annulus associated with aortic steno-insufficiency and mitral insufficiency. The aortic valve was found to have 4 equal-sized cusps. The noncoronary cusp was divided into two equal parts. This congenital quadricuspid valve was extremely rare anomaly and we have nerve seen the case which had been corrected surgically by Manouguian's procedure. The aortic valve was replaced by a 23 mm St. Jude Medical prosthesis and the mitral valve was replaced by

Five-year outcomes after surgical versus transcatheter aortic valve replacement with new generation devices from the prospective OBSERVANT studies.

The efficacy and durability of transcatheter (TAVR) over surgical aortic valve replacement (SAVR) for severe aortic stenosis (AS) has been demonstrated in randomized studies, but these findings were not confirmed in several observational studies. This is an analysis of 5706 AS patients who underwent SAVR from 2010 and 2012, and 2989 AS patients who underwent TAVR from 2017 and 2018 from the prospective OBSERVANT I and II studies. TAVR procedures were performed with new-generation devices. Five-year all-cause mortality was the primary outcome of this analysis. Propensity score matching yielded

Transcatheter aortic valve implantation with SAPIEN 3 versus surgical aortic valve replacement in patients with symptomatic severe aortic stenosis at low risk of surgical mortality: a cost-utility analysis for Switzerland.

AIMS OF THE STUDY: The 2021 European Society of Cardiology Guidelines on valvular heart disease recommend transcatheter aortic valve implantation (TAVI) for patients with symptomatic severe aortic stenosis at low surgical risk and age ≥75 years who are suitable for a transfemoral approach (recommendation class IA) based on two large randomised controlled trials (PARTNER 3 and Evolut Low Risk) comparing transcatheter aortic valve implantation with surgical aortic valve replacement (SAVR). Whether such an approach is cost-effective in Switzerland remains unclear. The aim of this cost-utility ana
