Monitor lizard bite-induced acute kidney injury--a case report.

Envenomations by venomous lizards are rare. Monitor lizard bite-induced acute kidney injury (AKI) is a previously unreported complication in humans. A 55-year-old female was bitten on her right leg during farming activity by a monitor lizard (Varanus bengalensis). The patient experienced severe local pain and bleeding from the wound, coagulopathy, hemolysis, rhabdomyolysis, sepsis, and AKI. Patient was treated with supportive care and peritoneal dialysis but succumbed to a sudden cardiac arrest. Post mortem kidney biopsy revealed pigment induced-acute tubular injury. AKI after monitor lizard e

The threshold of human endurance: 60 years of a congenital heart defect.

Long term survival in patients with partial atrioventricular septal defect (PAVSD) without adequate surgical management is very rare. The authors present the case of a 60 year-old woman who was referred to our hospital with dyspnea and worsening of general condition. A heart defect was diagnosed at the age of 25 years, but at the time the patient declined surgery. The patient underwent echocardiographic and haemodynamic evaluation prior to corrective surgery. The present report highlights the complications of chronic volume overload due to a congenital cardiac defect characterized by left--to-

Ovarian Hyperstimulation Syndrome with pleural effusion: a case report.

BACKGROUND: To report a case of severe ovarian hyperstimulation syndrome (OHSS) with right pleural effusion following controlled ovarian hyperstimulation. CASE PRESENTATION: A 24-year-old woman had severe OHSS as a complication of gonadotropin stimulation. The clinical picture showed enlarged ovaries, massive ascites, pleural effusion, abdominal pain, and dyspnea. Beside the medical treatment, abdominal paracentesis for the drainage of the massive ascites and tube thoracostomy were performed, resulting in expansion of the lung. CONCLUSION: Physicians can reduce the risk of OHSS by monitoring

[Association between WAGR syndrome and diaphragmatic hernia].

Anomalies in WT-1 (Wilms' tumor gene), mapped to 11p13, cause Denys-Drash, Frasier and WAGR syndromes. WAGR syndrome is characterized by Wilms' tumor (W), aniridia (A), genitourinary anomalies (G) and mental retardation (R). In the early human fetus, WT-1 is expressed in the pleural and abdominal mesothelium, and consequently this gene may play a role in diaphragm development. The first report of an association between WAGR syndrome and congenital diaphragmatic hernia has recently been published. We present another infant with aniridia, left cryptorchidism with testicular dysgenesis, right-sid

[Pulmonary dirofilariasis resected by video-assisted thoracoscopy; report of a case].

A 35-year-old female was admitted for biopsy of abnormal shadow on chest X-ray. She was operated on for partial wedge resection of the right lower lobe by video-assisted thoracoscopy without complication. Pathological findings in operation showed inflammatory benign tissues and suggested pulmonary dirofilariasis. Serologic examination was negative after operation, however histological diagnosis supported pulmonary dirofilariasis because a pulmonary artery embolism of calcified tissues consisted of non-human cells. There may be many cases due to old infections like this one, so it is important

Neisseria meningitidis native valve endocarditis. A case report.

The so-called nonpathogenic neisseriae are common inhabitants of the upper respiratory tract in humans and are not usually regarded as pathogens. Neisseria meningitidis on the contrary may cause severe disease. These organisms are an uncommon cause of infective endocarditis. The authors report a case of a 64 year-old male, type II diabetic, previously asymptomatic, admitted to hospital because of fever, aphasia and right hemi-paresis. A systolic murmur was heard at the cardiac apex, and three blood cultures were positive for Neisseria meningitidis. The echocardiogram showed a vegetation on the

Selective activation of human cortical area V5A by a rotating visual stimulus in fMRI; implication of attentional mechanisms.

The human homologue of area V5A of rotation-selective cells in the monkey medial superior temporal area (MST) was identified using functional magnetic resonance imaging (fMRI). It was located within the border region of occipito-temporo-parietal cortex, in four of 10 subjects on both sides, and on the right or left side in three subjects each. The stimulus was a black-and-white sine-modulated windmill presented either stationary or in rotation phases of 1 s duration. Areas V1-V3 did not show up with this paradigm. Focusing attention by mentally counting the number of rotation phases ensured hi

Surviving the impossible: the long march from Srebrenica. An investigation of the possible use of chemical warfare agents.

In July 1995 about 15,000 people attempted to walk from the enclave of Srebrenica to free territory in Bosnia. Two-thirds were captured or killed. Many of the remainder experienced hallucinations on the march, leading them to believe they were the victims of chemical weapons. This paper reports extended structured interviews with 35 survivors, including three doctors, carried out a year later on behalf of Human Right Watch. The literature on the likeliest CW agent, 3-quinuclidinyl benzilate (BZ), and on stress as a cause of hallucinations, is reviewed. While CW exposure cannot be ruled out, it

Crohn's-like colitis in glycogen storage disease Ib: a case report.

Association between glycogen storage disease type Ib and Crohn's like disease is rare. We present one such case. A young woman with glycogen storage disease type Ib underwent a right hemicolectomy for stenosis and a blind fistulous tract. The histological picture of the resected bowel was consistent with the diagnosis of Crohn's like disease. A leak in the entero-colostomy occurred during medical treatment with steroids, sulfasalazine, and methronidazole. Partial ileal and colonic resection and a new anastomosis were then performed. Recombinant human colony-stimulating factor (300 mcg/die subc

MRI evidence of labyrinthine and eighth-nerve bundle involvement in mumps virus sudden deafness and vertigo.

Enhanced MRI has recently become an even more useful diagnostic tool in the study of the labyrinth in sudden deafness and vertigo. A 22-year-old male who experienced a right-sided sudden deafness and vertigo due to mumps virus underwent labyrinthine MRI examination with gadolinium, and both labyrinthine and nerve bundle involvement was demonstrated. The relationship between mumps virus and the sudden deafness in humans has not been completely established and the path of labyrinthine infection and the possible eighth-nerve involvement is discussed. In vivo evidence of labyrinthine and cochleove

Convergence paralysis after lesions of the cerebellar peduncles.

We studied vergence eye movements in a patient with limited hemorrhage affecting the right cerebellar peduncles and the hemisphere. The patient showed exotropia of 30 prism dptr during near vision. Some vergence paradigms were investigated in this patient. We studied fusional vergence evoked by placing a wedge prism before one eye. Accommodative vergence was studied by blur stimulus using a servocontrolled optometer. Lens accommodation was also recorded during blur stimulus. Although accommodative responses were normal, accommodative vergence was almost absent as well as fusional convergence i

[A case of sepsis caused by Edwardsiella tarda complicated panophthalmitis and pyogenic spondylitis].

Edwardsiella tarda (E. tarda) is gram negative enterobacteriaceae which has been found generally in animal hosts and occasionally in human feces. We have reported a case of sepsis caused by E. tarda, complicated panophthalmitis and pyogenic spondylitis. A 39-year old patient suffered from fever, polyarthralgia and lumbago. We performed blood culture, from which E. tarda was isolated. Spinal CT scan showed destruction and osteogenesis of the fourth and fifth lumbar vertebral body and cranial CT scan showed destruction of the right lens. So we diagnosed sepsis with pyogenic spondylitis and panop

Phonagnosia: a dissociation between familiar and unfamiliar voices.

A dissociation between facial recognition and facial discrimination is well known, but investigations of "phonagnosia" (impairment of voice recognition and discrimination) have not been pursued. Using familiar and unfamiliar voices as stimuli, a marked difference between the ability to recognize familiar voice and the ability to discriminate between unfamiliar voices was identified in five patients, and a sixth showed a severe impairment in both tasks. Clinical and radiologic findings in these cases suggest that recognition of familiar voices is impaired by damage to inferior and lateral parie

Balanced double complex translocations [46,XX,t(1p;6p;7p;3q;11p)(11q;22p;21q)] in an infant with multiple congenital anomalies.

We describe a 1-month-old female infant with balanced double complex translocations [46,XX,t(1p;6p;7p;3q;11p) (11q;22p;21q)]. She had a multiple congenital anomalies syndrome of microcephaly, unilateral corneal staphyloma, hypoplastic thumbs, ectrodactyly of the right foot, brachydactyly of the left third toe, and syndactyly between the left fourth and the fifth toes. The complex chromosome rearrangements of the patient involve 8 breakpoints, the greatest number of breakpoints in complex chromosome rearrangements of humans known to us. The 8 breakpoints are 1p22, 3q27, 6p21, 7p15, 11p15, 11q11

Antemortem magnetic resonance imaging examination of an intracardiac bony mass in a cynomolgus monkey (Macaca fascicularis).

Ectopic ossification is defined as bone formation at a site where no bone tissue typically exists and usually presents a trabecular bone structure. Rare cases of ectopic ossification in cardiac masses have been previously reported. In general, cardiac masses are diagnosed by echocardiography, computed tomography, and magnetic resonance imaging (MRI). MRI, in particular, has been used in studies to evaluate cardiometabolic function and tumors in various non-human primates (NHPs). Here, we report a 15-year-old male cynomolgus monkey with significant weight loss and arrhythmia observed during a r

Atypical Presentation of an Odontogenic Brain Abscess Due to Eggerthella lenta and Bilophila wadsworthia Coinfection.

Odontogenic brain abscesses are uncommon. Eggerthella lenta and Bilophila wadsworthia are anaerobes that are normal components of the human intestinal microbiota. Both can also be found in the oral cavity. We report the case of an 81-year-old man with a brain abscess caused by coinfection of E. lenta and B. wadsworthia associated with periodontitis. The patient underwent a right frontal craniotomy with excision of the abscess. Ceftriaxone and metronidazole were then started, along with levetiracetam. Ceftriaxone and metronidazole were maintained for four weeks, and the antibiotic treatment was

Human Epidermal Growth Factor Receptor 2 (HER2/Neu)-Positive Ovarian High-Grade Serous Carcinoma Metastasizing to the Breast: A Case Report.

Metastasis to the breast from malignancies is rare and often poses a diagnostic challenge due to its clinical and radiologic similarity to primary breast cancer. We report the case of a 47-year-old woman who presented with abdominal discomfort, a palpable abdominal mass, and bilateral inguinal lymphadenopathy. Imaging and biopsy revealed high-grade serous carcinoma of Müllerian origin with human epidermal growth factor receptor 2 (HER2/neu) overexpression, which had metastasized to the breast and right axillary lymph nodes. She was treated with a combination chemotherapy regimen of Taxotere, c

Paradoxical Tuberculosis Immune Reconstitution Inflammatory Syndrome Complicated by Large Bilateral Psoas Abscess: A Case Report.

Immune reconstitution inflammatory syndrome is a common manifestation in human immunodeficiency virus (HIV)-positive patients infected with tuberculosis (TB). One of the unusual complications of this condition is the development of psoas abscess. We describe a case of immune reconstitution inflammatory syndrome (IRIS) in a patient with disseminated TB under treatment, HIV-positive with a low CD4 cell count, complicated by bilateral psoas abscess. Our patient presented with fever, abdominal pain, and lymphadenopathy, typical symptoms of TB-IRIS. Diagnosis was confirmed by surgical drainage of t

A Case Report of Uterine Body Constriction Precluding Normal Parturition Leading to Dystocia in a Mare.

A 13-year-old multiparous Quarter Horse mare was presented to the Michigan State University's, Large Animal Emergency service for dystocia. Clinical evaluation revealed a minimally dilated cervix on vaginal examination, with a palpable deceased fetus. Postmortem evaluation following owner-elected humane euthanasia revealed a circumferential, tan, fibrous band at the base of the uterine body that constricted the uterus and was adhered to the left and right ovaries. A routine histologic section of the incarcerating cord attached to the ovary consisted predominately of dense fibrous connective ti
