The authors report the cases of two patients in whom strontium-89 (89Sr) was used to relieve diffuse metastatic bone pain. The type of cancer involved, thymic carcinoid tumor, is itself rare and the risk of its metastasizing to the bone is very low. Both patients showed a measure of response to treatment, suggesting that this analgesic method has value for some patients. The marked benefit of one patient for a total of 9 months was attributable to two 89Sr injections, whereas the other patient improved for only 5 weeks after one injection.

A method is described for the measurement of 5,5-diphenylbarbituric acid in plasma using high-performance liquid chromatography with UV detection. Briefly, the compounds are separated on a C18 reversed-phase column using a mobile phase of 50 mM sodium acetate (pH 4.5) and methanol. The flow-rate is 1.0 ml/min and 25 microl are injected and detected at 215 nm. The method is specific and sensitive in the range of concentrations tested, with a limit of quantification of 0.25 microg/ml. We report the youngest known case with Baló's concentric sclerosis (Baló's disease), a variant of multiple sclerosis. This 4-year-old Japanese boy was diagnosed by clinical manifestations and by characteristic findings on magnetic resonance imaging (MRI). A patient with sickle cell trait who presented with gross hematuria and was subsequently found to have renal papillary necrosis is presented. The hematuria resolved with conservative therapy consisting of bed rest and hydration with hypotonic intravenous fluids. The pathophysiology of renal abnormalities associated with sickle cell trait is described. The management of the primary clinical manifestations of this disorder, hematuria and papillary necrosis, are discussed.

Therapeutic cerclage placement may be complicated by prolapsing of the fetal membranes. A gravida presented with incompetent cervix and prolapse of fetal membranes. The membranes were not reduced by Trendelenburg position, decompressive amniocentesis, and spinal anesthesia. Intravenous nitroglycerin promptly reduced the prolapse and allowed cerclage placement. This is the first report of intravenous nitroglycerin tocolysis used to facilitate cerclage placement.
