The effect of mode of delivery and anaesthesia on neonatal blood pressure.

BACKGROUND: Many factors may effect blood pressure (BP) in the early neonatal period, including mode of delivery and anaesthesia, on which there is little reported. AIMS: To determine whether the mode of delivery, anaesthesia and maternal age have an effect on neonatal BP in the first three days of life. METHODS: Healthy, term neonates from August 2003-2005 were enrolled in the study. Infants of mothers with hypertension of any cause, diabetes of any cause, illicit substance use, congenital or chromosomal anomaly, and admission to the neonatal intensive care unit were excluded. Information o

Candidal versus bacterial late-onset sepsis in very low birthweight infants in Israel: a national survey.

Candidal infections are one of the common causes of late-onset sepsis (LOS) among very low birthweight (VLBW) infants, and are associated with substantial morbidity and mortality. The aim of this study was to evaluate the perinatal and neonatal risk factors for fungal LOS compared with bacterial LOS in VLBW infants. This was a population-based observational study of VLBW infants in 28 neonatal intensive care units across Israel, with information on 11,830 infants born between 1995 and 2002 from the Israeli National VLBW infant database. The study population comprised 3054 infants with one or m

Routine cervical length in twins and perinatal outcomes.

A retrospective review of twin gestations was undertaken to evaluate whether routine cervical lengths (CLs) in such instances change pregnancy outcome. Data were collected from the ultrasound database and chart review. Exclusion criteria included twins reduced to singletons, twins not delivering at our institution, and incomplete information. Twin gestations with a CL were compared with those without a CL. Outcomes of interest included gestational age (GA) at delivery, preterm delivery (PTD), antepartum admissions, antepartum length of stay (LOS), cerclage placement, birthweight, neonatal inte

Neonatal complications of term pregnancy: rates by gestational age increase in a continuous, not threshold, fashion.

OBJECTIVE: The purpose of this study was to determine whether, when, and how rates of short-term neonatal complications increase beyond 37 weeks of gestation. STUDY DESIGN: A retrospective cohort study was conducted of all low-risk, term, cephalic, and singleton births that were delivered at the University of California, San Francisco, between 1976 and 2001. Primary outcomes included neonatal umbilical artery pH, umbilical artery base excess, the presence of meconium, macrosomia, 5-minute Apgar scores, and admission to the intensive care nursery. Multivariate analyses were performed that cont

Comparison of outcomes in uncomplicated term and post-term pregnancy following spontaneous labor.

This study was designed to compare outcomes of spontaneous labor in uncomplicated term and post-term pregnancy. It comprised of a case matched study of 285 women with uncomplicated singleton post-term pregnancy (290-301 days of gestation) and 855 women with uncomplicated singleton term pregnancy (259-287 days of gestation). Term controls were matched to post-term cases on a 3:1 basis. The main outcome measures were cesarian section, Apgar scores and admission to the Neonatal intensive care unit (NICU). Cesarian section was significantly more common in women with post-term pregnancy (Relative R

[Diagnostic possibility in neonatal bacterial infections].

Bacterial infections in neonates may be acquired in utero, as is the case in congenital syphilis, or after delivery, such as late onset B streptococcal meningitis. Although some infections remain localized and may even be self-limiting, the majority of neonatal bacterial infections require speedy diagnosis and immediate and appropriate treatment. Severe invasive bacterial infections are rare but catastrophic; they are more common in intensive care wards. Neonatal, maternal, and environmental factors favoring bacterial infections are discussed together with their main clinical features. Useful

Chromosomal Microarray Analysis in Critically Ill Neonates and Children: Diagnostic Yield and Clinical Utility.

Chromosomal microarray analysis (CMA) is widely used to detect chromosomal aneuploidies and copy number variants (CNVs) in pediatric patients with congenital anomalies or developmental concerns. However, its diagnostic utility in critically ill neonates and children admitted to intensive care units (ICUs) remains undercharacterized. We conducted a retrospective review of 679 patients admitted to the neonatal, pediatric, or cardiovascular intensive care units (NICU, PICU, CVICU) at Phoenix Children's Hospital between 2019 and 2024 who underwent CMA. Demographic data, clinical indications, and C

Endogenous distress in ventilated full-term newborns with acute respiratory failure.

OBJECTIVE: The main rationale behind the continuous analgesia/sedation currently practiced in the treatment of neonates with severe respiratory failure in intensive care is an attempt at shielding the sick newborn from exogenous stress and pain caused by diagnostic and therapeutic interventions. Until now, however, the factors which influence endogenous, disease-related distress have been largely ignored. METHOD: We retrospectively studied the daily need for analgesics and sedatives (fentanyl, midazolam, pentobarbital, thiopental) of 40 full-term newborns with severe respiratory failure who h

The Sound Environment in the PICU: A Scoping Review Describing Sound Levels, Interventions and Patient Impacts.

Elevated sound levels are associated with increased morbidity in adult and neonatal intensive care; however, their impact in the paediatric intensive care unit (PICU) remains poorly characterised. The aim of this scoping review was to characterise sound exposure in the PICU, including its measurement, sources, and associations with patient outcomes. MEDLINE, Embase, PsycINFO (Ovid), and PubMed were searched up to 5 November 2025 for full-text, peer-reviewed publications within the past 10 years to reflect the contemporary PICU environment. Titles, abstracts, and full texts were independently s

Managing diabetic chronic kidney disease in pregnancy: Current clinical practice and uncertainties.

BACKGROUND: Pre-gestational diabetes occurs in approximately 1% of pregnancies in the UK and increases the risk of adverse maternal and fetal outcomes. More women with type 2 than type 1 diabetes are now becoming pregnant and tend to have higher rates of obesity and other multi-morbidities. Chronic kidney disease (CKD) affects approximately 5%-10% of pregnant women with type 1 diabetes and about 2%-3% with type 2 diabetes. Diabetic chronic kidney disease (DCKD) increases the risk of preeclampsia, preterm birth, Caesarean section, small for gestational age (SGA) infant and infant admission to n

Fetomaternal outcomes in pregnant women with congenital heart disease: a comparative analysis from an apex institute.

OBJECTIVE: With advancements in cardiac surgical interventions during infancy and childhood, the incidence of maternal congenital heart disease (CHD) is increasing. This retrospective study compared fetal and cardiac outcomes in women with and without CHD, along with a sub-analysis between cyanotic versus non-cyanotic defects and operated versus non-operated cases. METHODS: A 10-year data were retrospectively collected from pregnant women with CHD and a 1:1 ratio of pregnant women without any heart disease. Adverse fetal and cardiac outcomes were noted in both groups. Statistical significance

Chronic Enlarged Iatrogenic Radial Arteriovenous Fistula.

Intravenous and arterial lines are common procedures performed in neonatal intensive care units. We report a case of a 16-year-old male patient with a chronic radial arteriovenous fistula (AVF) resulting from radial arterial access obtained shortly after birth. The patient subsequently developed a pulsatile mass at the insertion site, which was monitored without intervention given lack of symptoms. With time, the mass gradually enlarged, and intervention was pursued due to increased risk for trauma and/or hemorrhage due to participation in contact sports. Ultrasonography and angiography confir

All-Dry Self-Adhesive Soft Electrodes with Subsurface Pores for Long-Term Skin Recording.

Long-term electrophysiological recording remains challenging, particularly on delicate skin, due to reliance on adhesive backings and conductive gels. Here, a class of all-dry, self-adhesive epidermal electrodes is presented by integrating low-density, octopus-like microstructures with porous subsurface architectures. This design offers key advantages, including low electrode-skin impedance, high air permeability, dynamic adhesion to prevent interfacial delamination, and mild adhesion strength for safe-and-easy removal. The electrodes enable stable, high-quality recordings over 24 h, maintaini

Unexpected Culprit: A Case Report of Pleuropulmonary Pantoea septica Infection in a Ketamine User.

Pantoea septica, a gram-negative bacillus typically associated with opportunistic bloodstream infections in neonatal intensive care units, rarely causes pulmonary infections in immunocompetent individuals. We present a case of a 30-year-old male with multifocal cavitary pneumonia, bilateral parapneumonic effusions, and positive blood cultures for Pantoea septica, occurring in the setting of prior ketamine abuse. The patient presented with fever, productive cough, chest pain, and worsening dyspnea, without significant medical history or immunocompromising conditions. Diagnostic evaluation revea

Posterior uterine rupture in early first trimester.

Uterine rupture can be associated with severe maternal and neonatal morbidity and mortality. It should be considered as a differential diagnosis in all pregnant women who present with acute abdomen, haemoperitoneum and have specific risk factors, even during the first trimester. This is a case report of a 25-year-old woman who presented to emergency department with abdominal pain and vaginal bleeding at approximately 6-8 weeks gestation. She developed an acute surgical abdomen and required urgent surgical management. Despite intervention, she had massive haemorrhage, disseminate intravascular

Management of Pregnancy in Women of Advanced Maternal Age: Improving Outcomes for Mother and Baby.

Pregnancy at advanced maternal age (age >35 years old) is considered a risk factor for adverse maternal and perinatal outcomes. Yet, pregnancies of advanced maternal age have become more prevalent over the last few decades. Possible maternal complications of pregnancy at age 35 or older include increased risk of spontaneous miscarriage, preterm labor, gestational diabetes mellitus, pre-eclampsia, stillbirth, chromosomal abnormalities, and cesarean delivery. Possible adverse fetal outcomes include infants small for gestational age and intrauterine growth restrictions, low Apgar score, admission

Genetics of the patent ductus arteriosus (PDA) and pharmacogenetics of PDA treatment.

Patent ductus arteriosus (PDA) is a frequent, complex, and difficult to treat clinical syndrome among preterm infants in the neonatal intensive care unit. In addition to known clinical risk factors, there are emerging data about genetic predisposition to PDA in both animal and human models. Clinical response and toxicity from drugs used to treat PDA are highly variable. Developmental and genetic aspects of pharmacokinetics and pharmacodynamics influence exposure and response to pharmacologic therapies. Given the variable efficacy and toxicity of known drug therapies, novel therapeutic targets

Outpatient, Wide-Field, Digital Imaging of Infants With Retinopathy of Prematurity.

Eight infants were closely monitored for retinopathy of prematurity (ROP) while in the neonatal intensive care unit (NICU); two required laser therapy. After discharge from the NICU, they had outpatient examination with dilated, scleral depressed ROP staging, and each had digital widefield retinal imaging (Optos, Dunfermline, United Kingdom) without lid speculum, or with lid speculum if not cooperative. Images were shared remotely using ROP-Check software (Glacier Medical Software, Anchorage, AK), from which ophthalmologists made clinical impressions. The images did not afford clear view of ma
