Two cases of neurotoxic effects resulting from therapy with vidarabine are described. Both patients were undergoing treatment for cutaneously disseminated herpes zoster complicating therapy for solid malignant tumours. Both had normal renal function. The serum levels of hepatic enzymes were normal in one patient and slightly elevated in the other. Neurotoxicity was first manifested in both patients by the development of intention tremors that progressed to gross tremors. This article describes the decisions made relating to cleft surgery pathway design in a single-surgeon cleft service in the United Kingdom at the start of a cleft surgeon's consultant career. Pathways are described according to cleft phenotype and detail aspects of surgical technique, sequence and timing. Transparency is vital to ensure clinical governance and external analysis, especially in the setting of a single-surgeon service. This early declaration of surgical intent aims to lead the way for ongoing reflection, so that future changes to practice can be identified and outcomes monitored.

Pathogenic variants of sacsin (SACS) gene cause autosomal recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS). It is a hereditary neurological disorder manifested with gait ataxia, intentional tremor, lower limb pyramidal signs and pes cavus. It was originally described in the late 1970s and has a high prevalence in northeastern Quebec, Canada. Here, we present for the first time a new SACS frameshift variant in two Turkish siblings. We detected a new homozygous frameshift variant of the SACS gene in the Turkish siblings diagnosed with ARSACS for the first time.

Aluminum phosphide is a metal phosphide widely implemented as a pesticide. In the country of Iran, it is recognized by the colloquial name of rice pill. Any trace of accidental or intentional ingestion of aluminum phosphidecan result in severe hemodynamic disorders and metabolic acidosis, ultimately leading to the patient's mortality. In this report, we document the unfortunate demise of an 85-year-old man who lived in isolation, having lost his wife to COVID-19. The patient consumed aluminum phosphide tablets and, despite resuscitation efforts, inevitably succumbed to the toxic effects.

The insertion of inert spherical objects under the skin of the penile shaft with the intent of enhancing the sexual experience of one's partner has been reported mostly among Southeast Asian men. This practice is gaining popularity among the Hispanic jail population and prison inmates in southern California. We present a case series of 4 inmate patients with genital modifications (artificial penis nodules [APNs]), including one patient with vitiligo that was induced by his APN. More than 25% of unintentional gunshot injuries and more than 15% of intentional gunshot injuries involve extremities. In these patients, early acute multidisciplinary treatment is essential and critical to achieve a satisfactory mobility and composite wounds repair. We hereby report a logistic delayed successful treatment of a complex arm gunshot wound with a free fibula flap. The significance of this case report is related to the necessity of emphasising the organised and multidisciplinary approach required to cure these patients in the proper way from a surgical and medical point of view.
