Posttraumatic stress disorder and obesity: evidence for a risk association.

BACKGROUND: There is evidence from cross-sectional studies that posttraumatic stress disorder (PTSD) may be associated with obesity. The aim of this study was to examine prospective longitudinal associations between PTSD and obesity in a community sample. METHODS: A prospective, longitudinal, epidemiologic study with a representative community sample of adolescents and young adults (N=3021, aged 14-24 years at baseline) was conducted in Munich, Germany. Participants were assessed four times between 1995 and 2005 with the Munich-Composite International Diagnostic Interview. Associations betwee

Impulsivity in bipolar-II disorder: trait, state, or both?

BACKGROUND: In bipolar-II (BP-II) disorder impulsivity (defined as excessive risky activities by DSM-IV-TR) is one of the symptoms of hypomania. It is unclear if impulsivity is also a trait in BP-II. STUDY AIM: The aim was to test if impulsivity was also a trait in BP-II. METHODS: Consecutive 136 remitted BP-II outpatients (assessed when presenting for depression by a mood disorder specialist psychiatrist using the Structured Clinical Interview for DSM-IV), self-assessed trait impulsivity during follow-ups, using the Personality Questionnaire of the Structured Clinical Interview for DSM-IV A

Testing new diagnostic criteria for hypomania.

BACKGROUND: A recent series of studies has questioned DSM-IV diagnostic criteria for hypomania, suggesting that overactivity (increased goal-directed activity) should have priority over mood change as stem criterion. Angst has suggested new criteria for hypomania, giving priority to overactivity. Study aim was to test the validity of Angst's diagnostic criteria for hypomania. METHODS: A consecutive sample of remitted 213 DSM-IV bipolar-II disorder (BP-II) and major depressive disorder (MDD) outpatients were re-diagnosed, during a follow-up visit, by the Structured Clinical Interview for DSM-I

Stroke care in the home: the impact of social support on the general health of family caregivers.

BACKGROUND: Throughout the stroke rehabilitation process, community care is one of the most important elements. Learning to live with and take care of a family member with a stroke is immensely complex and demanding. Without appropriate support, family caregivers are at risk of their own general health decreasing, thus becoming a patient themselves - the second patient in the family. AIM: The aim of this study was to examine aspects of social support available to family caregivers during the first 12-week transitional period following hospital discharge. DESIGN AND METHOD: This study adopted

Nicotine dependence measures among adolescents with psychiatric disorders: evaluating symptom expression as a function of dependence severity.

Using methods based in item response theory, we examined a structured interview assessment of Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) nicotine dependence and the Modified Fagerström Tolerance Questionnaire (mFTQ) symptoms to explore the expression of particular symptoms as a function of level of nicotine involvement in a sample of 191 adolescents with psychiatric disorders. Despite our attempts to capture a broad range of smokers, 64% of teens were daily smokers and 68% met DSM-IV criteria for nicotine dependence. This paper describes the relative severity o

Childhood adversities associated with major depression and/or anxiety disorders in a community sample of Ontario: issues of co-morbidity and specificity.

It has been well established that early adversity is a major risk factor for depression and for anxiety disorders in various populations and age groups. Few studies have considered the relative strength of these associations and the possible role of co-morbid depression/anxiety in understanding them. Using data from a large community sample of Ontario, Canada, we examined the relative strength of the associations between early physical abuse, sexual abuse, and/or parental strain with depression alone, anxiety alone, and co-morbid depression/anxiety. The current sample consisted of 6,597 indivi

Reliability of interview-assessed hostility ratings across mode of assessment and time.

These studies addressed 2 questions concerning interview-based hostility assessments: whether they are affected if the interview is conducted face-to-face versus telephone and whether they are stable across an extended time period. In Study 1A, 54 students were interviewed face-to-face and by telephone in a laboratory setting. Half the sample was reinterviewed in the laboratory 6 weeks later. The other half was reinterviewed by telephone at home. With 1 exception, all intraclass correlation coefficients (ICCs) comparing interview modes were above .62. In Study 1B, 48 adults were interviewed fa

Characteristics and psychosocial problems of patients with bipolar disorder at high risk for suicide attempt.

BACKGROUND: Bipolar disorder with a history of substance abuse or suicide attempt is a strong predictor of suicide. A high comorbidity of substance use disorders may obscure the specificity of findings about suicide behaviors in Western patients with bipolar disorder. METHODS: The clinical data of Chinese bipolar patients (DSM-III-R) in Taiwan who had been naturalistically followed up for at least 15 years were obtained by a combination of chart reviews and interviews with patients and family members. The national identity numbers were used to search for deceased subjects. RESULTS: There wer

What are the real problems for patients with functional dyspepsia?

BACKGROUND: Patients with functional dyspepsia (FD) have more complaints than just the dyspepsia. METHOD: One hundred FD patients were assessed with regard to psychologic, medical, and social factors, before randomization, in a study of cognitive therapy. They were asked to list their main problem areas or 'target complaints'. RESULTS: Dyspepsia was the third most frequent target complaint (26 patients), and anxiety was the most frequent one (65 patients). Patients identifying dyspepsia as a target complaint differed from the other FD patients in several aspects. They had significantly more

A comparison of two structured diagnostic interviews: CIDI and SCAN.

The relationship between and the inter-rater reliability of the Composite International Diagnostic Interview (CIDI) and the Schedules for Clinical Assessment in Neuropsychiatry (SCAN) for anxiety and depressive disorders were explored. The CIDI and the SCAN were administered by trained interviewers in counterbalanced order. A subsample of interviews was observed to determine the inter-rater reliability of the instruments. Subjects were 101 patients accepted for treatment at an Anxiety Disorders Clinic; 29 of the 101 patients participated in the inter-rater reliability study. Concordance betwee

[Personality structure and subjective illness concepts of phobic anxiety neurotic patients. Qualitative comparison of 12 individual analyses of initial psychotherapy interviews].

Verbatim transcripts of 12 psychotherapeutic interviews with patients suffering from phobia and panic disorders were examined, focussing on subjective theories of illness, biography, and descriptions of the own personality. The results of our qualitative content analysis allow the reconstruction of some central features of these patients, like characterizing the own personality as normal and without any problems in a stereotype way (1), at the same time feeling wrong understood and exploited from other persons (2), feeling as an outsider and left alone (3), and being no more able to compete wi

Hearing and premorbid personality in paranoid states.

The author studied the hearing and premorbid personality of 79 patients whose illnesses began before they were 60 years old. Hearing was assessed by pure-tone audiometry and premorbid personality by interviewing relatives. Thirty-two patients had paranoid states, 32 matched patients had recurrent unipolar depression, and 15 patients had schizophrenia. Three paranoid patients and two depressed patients met DSM-III criteria for premorbid paranoid personality. There was no consistent difference in hearing loss between paranoid and depressed patients or between paranoid patients and a population c
