CD19-directed chimeric antigen receptor T-cell (CAR T-cell) therapy has markedly improved the prognosis of patients with relapsed or refractory large B-cell lymphoma (R/R LBCL). However, disease progression during the manufacturing period remains a major barrier to successful treatment. Bridging therapy (BT), defined as anti-lymphoma treatment administered between leukapheresis and lymphodepleting chemotherapy, serves two primary purposes: to prevent disease progression ensuring eligibility for CAR T-cell infusion, and to modulate the immune microenvironment to potentially enhance CAR T-cell e

BACKGROUND/AIM: Ifosfamide is a well-known chemotherapeutic agent used to treat various cancer types but is associated with central nervous system toxicity, termed ifosfamide-induced encephalopathy (IIE). Rarely, this toxicity manifests as catatonia.
CASE REPORT: We present the case of a 61-year-old woman with diffuse large B-cell lymphoma who developed profound catatonia after ifosfamide infusion. Her symptoms resolved completely following a single intravenous dose of lorazepam.
CONCLUSION: This case underscores the importance of recognizing catatonia as a manifestation of IIE and supports

For clinical prediction applications, we are often faced with small sample size data compared to the number of covariates. Such data pose problems for variable selection and prediction, especially when the covariate-response relationship is complicated. To address these challenges, we propose to incorporate external information on the covariates into Bayesian additive regression trees (BART), a sum-of-trees prediction model that utilizes priors on the tree parameters to prevent overfitting. To incorporate external information, an empirical Bayes (EB) framework is developed that estimates, assi

A 65-year-old man presented with painful swelling and thickening of both lower limbs, initially responding to oral steroid treatment. However, the patient later developed similar symptoms, accompanied by general malaise. No abnormalities were detected upon fluorine-18 fluorodeoxyglucose positron emission tomography/computed tomography. Magnetic resonance imaging (MRI) showed extensive fascial edema in the thigh and gluteal regions, mimicking fasciitis. Concurrently, a bone marrow biopsy revealed intravascular large B-cell lymphoma (IVLBCL), a rare lymphoma with a generally poor prognosis. This

Time to progression is the strongest predictor of outcome in relapsed diffuse large B-cell lymphoma. Second-line treatment with chimeric antigen receptor (CAR) T-cell therapy is recommended for patients with progression within 12 months of first-line chemoimmunotherapy. In patients with late relapse, platinum-based chemotherapy followed by high-dose chemotherapy with autologous stem cell rescue is recommended. In second relapse, CAR T-cell or CD3xCD20 bispecific antibody therapy is recommended in eligible patients. Other treatment options are available for less fit patients. Specific recommend

BACKGROUND: ZUMA-7 is the largest randomized controlled trial (RCT) for chimeric antigen receptor (CAR) T-cell therapy, which compared axicabtagene ciloleucel (axi-cel) to historical standard of care (HSoC) for the treatment of relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL). Both arms of ZUMA-7 contained potentially curative treatments; however, differences in the treatment completion rate and timing to receive definitive treatment led to differences in the extent and timing of cure. Mixture cure modeling (MCM) has been suggested as a superior method in designing and powering

BACKGROUND Primary adrenal lesions pose significant diagnostic challenges, as noninvasive imaging often fails to differentiate malignant from benign lesions. Percutaneous fine-needle aspiration (FNA) could be risky and undiagnostic, particularly in patients with hematologic disorders and low platelet counts. Performing surgery risks adrenal dysfunction, potentially resulting in poor prognosis. Since adrenal glands are located in the retroperitoneal space, endoscopic ultrasound-guided FNA (EUS-FNA) may be a good choice to obtain biopsy. While adrenal metastasis diagnosed by EUS-FNA has been rep

Despite great advances in treatment, 30-40% of patients with DLBCL undergo relapses. Patients with a relapse within 1 year or beyond have a distinct outcome. Few clinical characteristics and survival data in the Chinese population have been published. We aimed to define the incidence and clinical features of DLBCL patients with very early relapse after front-line immunochemotherapy who may benefit greatly from the emerging chimeric antigen receptor T-cell therapy. Data of 564 DLBCL patients were analyzed. Among the 413 patients achieving a first complete remission, 59 underwent relapses: 32 pa

BACKGROUND Primary central nervous system diffuse large B-cell lymphoma (DLBCL) is an extremely aggressive brain disease that rarely affects immunocompetent non-elderly patients, particularly with hemorrhagic presentation. Brain magnetic resonance imaging (MRI) plays an important role in the diagnosis of this entity, which typically demonstrates restricted diffusion and a T2 hypointense appearance, suggesting hypercellularity. CASE REPORT A 44-year-old man came to the emergency department with a persistent and treatment-resistant bilateral frontal headache that had been bothering him for the p

Rationale: The composition and spatial structure of the lymphoma tumor microenvironment (TME) provide key pathological insights for tumor survival and growth, invasion and metastasis, and resistance to immunotherapy. However, the 3D lymphoma TME has not been well studied owing to the limitations of current imaging techniques. In this work, we take full advantage of a series of new techniques to enable the first 3D TME study in intact lymphoma tissue. Methods: Diverse cell subtypes in lymphoma tissues were tagged using a multiplex immunofluorescence labeling technique. To optically clarify the

Richter transformation (RT) is the development of aggressive lymphoma - most frequently diffuse large B-cell lymphoma (DLBCL) and rarely Hodgkin lymphoma (HL) - arising on the background of chronic lymphocytic leukaemia (CLL). Despite recent advances in CLL treatment, RT also develops in patients on novel agents, usually occurring as an early event. RT incidence is lower in CLL patients treated with novel agents in the front line compared to relapsed/refractory cases, with a higher incidence in patients with TP53 disruption. The genetic heterogeneity and complexity are higher in RT-DLBCL than

BACKGROUND: Epstein-Barr virus (EBV) is detected in a variety of B-cell lymphomas (BCLs) and B-cell lymphoproliferative disorders (B-LPDs). Immunodeficiency has been considered to play a key role in the pathogenesis of these diseases. In addition, immune escape of tumor cells may also contribute to the development of EBV+ BCLs and B-LPDs. The PD-1/PD-L1 pathway is particularly important for immune escape of tumor cells that contribute to development of lymphoma through suppression of cytotoxic T-cell function. We now consider PD-L1 immunohistochemistry (IHC) a very useful method for predicting

BACKGROUND: Elderly classical Hodgkin lymphoma (cHL) (ecHL) is a rare disease with dismal prognosis and no standard treatment. Fitness-based approaches may help design appropriate treatments. Sarcopenia has been associated with an increased risk of treatment-related toxicities and worse survival in various solid tumours, but its impact in ecHL is unknown. The aim of this retrospective multicentre study was to investigate the prognostic role of sarcopenia in ecHL.
METHODS: We included newly diagnosed >64 years old cHL patients who performed a baseline comprehensive geriatric assessment and hig

Mycosis fungoides (MF) is an indolent, uncommon, non-Hodgkin T-cell lymphoma of the skin. It classically presents with patches, plaques, and tumors and may rarely show spread to internal organs or bone marrow. Up to 7.5% of MF patients may be diagnosed with a second malignancy. Intravascular large B-cell lymphoma (IVLBCL) is an exceedingly rare non-Hodgkin B-cell lymphoma characterized by predominant growth of large neoplastic cells in the lumina of blood vessels. This case presents with an unusual confluence of two rare diagnoses, MF and IVLBCL, made more remarkable by the presence of both di

Various grading systems are currently used for chimeric antigen receptor (CAR) T-cell-related toxicity, cytokine release syndrome (CRS), and immune effector cell-associated neurotoxicity syndrome (ICANS). We compared the recently proposed American Society for Transplantation and Cellular Therapy (ASTCT) grading system to other grading scores in 2 populations of adults: patients (n = 53) with B-cell acute lymphoblastic leukemia (B-ALL) treated with 1928z CAR T-cells (clinicaltrials.gov #NCT01044069), and patients (n = 49) with diffuse large B-cell lymphoma (DLBCL) treated with axicabtagene-cilo

DLBCL is the most common subtype of non-Hodgkin lymphoma, representing about 30% to 40% of cases. Patients are typically diagnosed with an excisional lymph node biopsy or a biopsy of another affected organ. When pathologists look under the microscope, they can see a diffused proliferation of large neoplastic B cells. These B cells are identified by flow cytometry or immunohistochemistry that identifies pan-B-cell antigens such as CD19, CD20, and CD79a, as well as CD45. DLBCL is known to be an aggressive lymphoma, but it is also known to be both clinically and molecularly heterogeneous.[1] Pati

OBJECTIVE: To investigate the prognostic evaluation value of fluorodeoxyglucose (FDG) interim positron emission tomography/computed tomography (PET/CT) for diffuse large B cell lymphoma (DLBCL).
METHODS: Two hundred and twenty-seven patients with pathologically diagnosed DLBCL underwent 18F-FDG scans at baseline and before 3 cycles of a rituximab-containing chemotherapy regimen. The Visual Deauville score (DS) and changes in maximum standard uptake values (ΔSUVmax) were calculated for tracer for the predominant lesion of each patient, for prediction of progression-free survival (PFS) and over

We report a very unusual presentation of primary intraocular lymphoma masquerading as anterior uveitis with atypical symptoms. A 68-year-old man, initially presented with a 4-day history of painless, left blurred vision. Examination revealed 2 or more anterior chamber cells, mutton-fat keratic precipitates and posterior synechiae. Treatment was initiated, the eye settled, and the patient was discharged. He re-presented 7 months later with hypopyon, a hazy cornea, fibrin deposits and a narrowed anterior chamber angle. Over the coming weeks, it became apparent that the temporal iris was thickene
