The consequences of ultra-processed foods on brain development during prenatal, adolescent and adult stages.

Ultra-processed foods (UPFs) are highly refined formulations of non-nutritive compounds containing elevated amounts of sugar, fat, sodium, food additives, and dietary emulsifiers. Consumption of UPF is robustly linked to a range of non-communicable diseases, including obesity, type 2 diabetes, cardiovascular disease, and mental disorders in adults. While substantial research highlights the negative health impacts of UPFs in adulthood, their effects on brain development during critical periods of biological vulnerability, pregnancy, childhood, and adolescence, remain underexplored. During pregn

Beyond the guidelines: participants' perspectives on sustained MPDSR implementation in Nigeria, North Macedonia, and Sri Lanka.

BACKGROUND: Maternal and perinatal death surveillance and response (MPDSR) was developed as a quality improvement intervention to reduce preventable maternal and newborn deaths and stillbirths. To gain deeper insight into the key components enabling sustained MPDSR implementation, we examined how MPDSR systems are organized and function in Nigeria, North Macedonia, and Sri Lanka. METHODS: We conducted 61 interviews with participants who were knowledgeable about the MPDSR system of their country, including policymakers, healthcare providers, and public health officials, at the national, subnati

Breaching the family walls: Modelling the impact of prolonged visa insecurity on asylum-seeking children.

OBJECTIVE: Children in asylum-seeking families are increasingly subject to deterrent host nation policies that undermine security in the post-migration context, however, little is known on the mental health consequences of such policy. This study examined the impact of prolonged visa insecurity on child mental health, by comparing two cohorts of refugee children entering Australia between 2010 and 2013, distinguished by visa security. METHODS: The insecure visa sample comprised children from Tamil asylum-seeking families, while the secure visa sample was drawn from refugee families participat

Exposure to caring parenting and adult mental health in extremely low birthweight survivors.

AIM: Individuals born at extremely low birthweight (ELBW; <1000 g) are exposed to early adversities that increase the risk of mental health problems in later life. Caring parenting has been shown to offset the negative effects of early adversity in general population samples. However, the long-term impact of caring parenting on the mental health of preterm survivors in adulthood is not known. METHODS: Using data from the world's oldest longitudinally followed cohort of ELBW survivors (n = 179) and matched normal birthweight (NBW) control participants (n = 145), we examined if caring parenting

Using human-centered design to re-vision the emergency obstetric and newborn care framework: Insights from Bangladesh, Malawi and Senegal.

The Emergency Obstetric and Newborn Care (EmONC) Framework has been instrumental in helping countries plan and monitor maternal health services for over 20 years. Given evolving health systems, the changing expectations of both patients and health providers, and the expanded evidence base, the "Re-Visioning EmONC" project was initiated to update this framework to better meet countries' needs. To understand the needs of its primary intended users, the project used human-centered design (HCD) to conduct in-depth studies in three countries with extensive experience using the EmONC Framework: Bang…

Effect of community based health education on knowledge and attitude towards iron and folic acid supplementation among pregnant women in Kiambu County, Kenya: A quasi experimental study.

INTRODUCTION: Iron and Folic Acid Supplementation (IFAS) services are currently provided free of charge to pregnant women in Kenya during antenatal care (ANC) but compliance remains low. Poor awareness is an important factor contributing to low utilization of IFAS. Inadequate counselling is one of the key factors associated with poor awareness on IFAS. Community based health education is a promising diversification strategy for IFAS health education to curb this problem. OBJECTIVES: To determine effect of community based IFAS health education, utilizing CHVs, on IFAS knowledge, levels of coun

Barriers to formal emergency obstetric care services' utilization.

Access to appropriate health care including skilled birth attendance at delivery and timely referrals to emergency obstetric care services can greatly reduce maternal deaths and disabilities, yet women in sub-Saharan Africa continue to face limited access to skilled delivery services. This study relies on qualitative data collected from residents of two slums in Nairobi, Kenya in 2006 to investigate views surrounding barriers to the uptake of formal obstetric services. Data indicate that slum dwellers prefer formal to informal obstetric services. However, their efforts to utilize formal emerge

Maternal High-Fat Diet, During Preimplantation or Gestation/Lactation, Alters Mouse Offspring Metabolic, Molecular and Locomotor Phenotypes.

Maternal nutrition during offspring development influences the offspring physiology and risk of long-term chronic disease. Here, we investigate the effects of maternal high-fat diet during distinct developmental windows (preimplantation alone, EmbHFD; or throughout gestation and lactation, HFD) on adult offspring metabolic, molecular and motor behavioural outcome. Our mouse model, utilising healthy nonobese MF1 mothers, induced distinct offspring outcomes dependent upon dietary duration with some sex-specific effects. EmbHFD offspring had high serum leptin (females), elevated systolic blood pr

Socioeconomic and Clinical Risk Factors for Meconium-Stained Amniotic Fluid and Associated Maternal-Neonatal Morbidity in Ethiopia: A Prospective Case-Control Study.

Meconium-stained amniotic fluid (MSAF) results from premature release of meconium by the fetus under stressful conditions and is associated with increased risk of maternal and neonatal morbidity and mortality. Risk factors for stressful conditions may differ between women living in highly developed countries and those in low-income countries. This study aimed to evaluate known and potential risk factors for MSAF and to assess the association between MSAF and maternal and neonatal morbidity. This prospective case-control study was conducted at a tertiary care hospital in Wolisso, Ethiopia. A to

Neonatal Outcome Ascertainment in Mother-Infant Paired Claims.

PURPOSE: Claims data are a valuable source to study neonatal outcomes across a wide range of clinical questions. Infants' delayed enrollment in infant insurance poses challenges in capture of neonatal outcomes, which may be charged to the maternal health plan, posing misclassification risks. We evaluated outcome ascertainment across three infant enrollment scenarios. METHODS: We used Merative MarketScan databases (2012-2018) in the United States to construct a mother-infant linked cohort and assess the outcome ascertaiment precision with varying infant enrollment requirement. RESULT: We foun…

A perinatal approach for unraveling mitochondrial function in preeclampsia and fetal growth restriction: a role for mitochondrial-targeted therapies.

Preeclampsia (PE) is a complex hypertensive disorder resulting from placental insufficiency during pregnancy. PE contributes to maternal and fetal morbidity and mortality and often co-occurs with fetal growth restriction (FGR); these two are both considered placental insufficiency syndromes. Alterations in mitochondrial function levels due to placental insufficiency play an important role in the pathophysiology of PE and FGR. Changes in these processes can lead to maternal and fetal organ damage with subsequent risk to develop cardiovascular disease. This review therefore investigates the effe

Trends in the Prevalence of Down Syndrome (Trisomy 21) in Texas by Maternal Race/Ethnicity and Maternal Age Groups, 1999-2020.

Down syndrome (DS) is a common chromosomal aneuploidy characterized by intellectual disability. Older maternal age is the strongest known risk factor for DS. The purpose of this study was to describe DS prevalence among major racial/ethnic groups stratified by maternal age, and to assess trends in prevalence over time in Texas. Cases with DS diagnoses delivered between 1999 and 2020 were identified from the Texas Birth Defects Registry (TBDR). Birth prevalence and crude prevalence ratios (PRs) by maternal race/ethnicity, maternal education, residence along the Texas-Mexico border, and Texas pu
