The health-related quality of life of children, adolescents, and young adults with neurofibromatosis type 1 and their families: Analysis of narratives.

PURPOSE: Provide an in-depth description of the health-related quality of life (HRQoL) in youth diagnosed with neurofibromatosis type 1 (NF1) and their families. DESIGN AND METHODS: Data were drawn from qualitative interviews conducted for a larger study aimed at developing the Pediatric Quality of Life Inventory™ (PedsQL™) NF1 module. RESULTS: Youth with NF1 and their families experience a wide range of concerns related to HRQoL due to the varied symptom expression and uncertain trajectory of the disorder. PRACTICE IMPLICATIONS: Pediatric nurses should routinely assess for HRQoL in this po

Committee Opinion No. 696: Nonobstetric Surgery During Pregnancy.

The American College of Obstetricians and Gynecologists' Committee on Obstetric Practice acknowledges that the issue of nonobstetric surgery during pregnancy is an important concern for physicians who care for women. It is important for a physician to obtain an obstetric consultation before performing nonobstetric surgery and some invasive procedures (eg, cardiac catheterization or colonoscopy) because obstetricians are uniquely qualified to discuss aspects of maternal physiology and anatomy that may affect intraoperative maternal-fetal well-being. Ultimately, each case warrants a team approac

Use of Robotic Pets in Providing Stimulation for Nursing Home Residents with Dementia.

Trial experiments utilized robotic pets to facilitate self-reliance in nursing home residents. A remote-control robot modeled clear and meaningful behaviors to elderly residents. Special attention was paid to its effects on mental and social domains. Employing the robot as a gaze target and center of attention created a cue to initiate a communication channel between residents who normally show no interest in each other. The Sony AIBO robot in this study uses commercially available wireless equipment, and all its components are easily accessible to any medical or welfare institution interested

Natural products for cancer prevention.

OBJECTIVES: To review the clinical trial literature on the use and effects of natural products for cancer prevention. DATA SOURCES: Clinical trials published in PubMed. CONCLUSION: There is a growing body of literature on the use of natural products for cancer prevention. To date, few trials have demonstrated conclusive benefit. Current guidelines recommend against the use of natural products for cancer prevention. IMPLICATIONS FOR NURSING PRACTICE: Clinicians should ask patients about their use of natural products and motivations for use. If patients are using natural products for cancer p

Supraventricular tachycardia in the pediatric primary care setting: Age-related presentation, diagnosis, and management.

As many as 1 in 250 children experience supraventricular tachycardia (SVT), but its presentation is often vague and its symptoms mistakenly attributed to other common pediatric conditions. If SVT is correctly identified in a timely manner, most children will go on to live normal healthy lives. SVT is not covered in depth in most pediatric advanced practice nursing programs, but because of its prevalence, it should be familiar to all pediatric primary care providers. This article reviews common mechanisms of SVT and their age-related presentation, diagnosis, and management. A case study of an 8

On call: alert or unsafe? A report of the AORN on-call electronic task force.

Research has linked health care providers' long work hours with patient care errors. Perioperative staff members often work extended hours because of long shifts and being on call. AORN's on-call electronic task force conducted a survey of nurses to determine their work load, call scheduling, and experiences related to fatigue. Of the 1,013 respondents, 67.7% said they had experienced effects of sleep deprivation. A follow-up survey revealed that few call programs are in compliance with AORN's recommended practices. Compliant call programs are detailed for adaptation in other perioperative set

Clinical applications of argon plasma coagulation in endoscopy.

The argon plasma coagulator is a device used for noncontact thermal coagulation of tissue. The device was first used in open and laparoscopic surgical procedures and in 1991 was adapted for use in endoscopy. Since then, argon plasma coagulation has expanded its clinical applications in the treatment of various gastrointestinal conditions. The endoscopy nurse plays an important role in the care of patients before, during, and after argon plasma coagulation treatment. This article reviews the principles and components of the argon plasma coagulator, and provides a summary of the various clinical

Wound care issues in the patient with cancer.

As cancer continues to represent a major health problem in the United States and in other developed countries, MCWs will continue to represent a complex problem for patients and health care professionals alike. Goals of care may range from healing to palliation, depending on the underlying pathology and patient preferences regarding their personal goals of wound or disease management. Palliative wound care should focus on patient comfort and quality of life as the goals, instead of wound. As evidence related to wound healing, products, and technology continues to increase, nurses can be at the

Rabies treatment of health care staff.

This article deals with post-exposure treatment of contacts of human cases of rabies. Experience suggests that rabies is hardly ever transmitted from person to person if at all. With the modern generation of vaccines against rabies, the use of post-exposure treatment is more widespread. A lot of contacts of patients with rabies in developed countries are treated, most of them being health care workers. This treatment is however not practised so often in developing countries where the supply of highly protective vaccines with a few side effects is low. According to reports good hygienic practic

UNLABELLED: Volvulus with or without malrotation are infrequent in the extremely premature newborn. CASE REPORTS: Intestinal volvulus in seven premature newborns are reported with abdominal distention, bright and tense skin without visible bowel loops and spiraled bowel loops on the abdominal X-ray. Intestinal resection was avoided due to early diagnosis. We identified abdominal wall massages as a risk factor, because no new cases have occurred since interdiction of these practices. CONCLUSIONS: Symptoms and radiologic findings are relatively specific for excluding the diagnosis of necrotizi

[Treatment of scabies with special consideration of the approach in infancy, pregnancy and while nursing].

Infestation of human skin with scabies mites remains an persistent problem in daily dermatological practice. There are several therapeutic regiments against scabies which have been proven effective and without major side effects. Because of known neurotoxicity of lindane as well as intoxication/problems following systemic absorption of benzyl alcohol (the metabolite of benzyl benzoate), these two compounds have to be considered with caution in early childhood scabies as well as during pregnancy and nursing. The pharmacology and spectrum of clinical side effects of the approved anti-scabies pre

Runaway youths do not fit into a typical profile; they are adolescents of every race, ethnic group, and religious orientation; they are representative of every socioeconomic status and geographic location in America. 2. Psychosocial events including physical and sexual abuse are common antecedents to runaway behaviors. 3. Implications for psychiatric mental health nursing practice include working with community programs focused on reducing runaway episodes; implementing preventive measures focused on strengthening parenting and problem-solving skills for reducing conflict with families with

Intrauterine growth retardation. Etiology, diagnosis, and management.

Intrauterine growth retardation (IUGR) occurs in 3% to 10% of all pregnancies. Although it is an uncommon occurrence in a low-risk nurse-midwifery practice, as many as 65% of the cases of IUGR are not identified until after the birth of the infant. Identification of IUGR is important because it carries a high risk of antepartum fetal death, anomalies, intrapartum asphyxia, and long-term morbidity. Additionally, identification of IUGR is essential for appropriate prenatal and intrapartum management. A review of etiologic factors, current assessment, and management of the pregnancy complicated b

Continuous morphine infusion for postoperative pain in children.

Previous erratic use of intermittent intramuscular and intravenous morphine or pethidine for postoperative analgesia prompted a prospective trial of continuous intravenous morphine delivered by an infusion pump. The rate was adjusted to keep the patient free of pain--as assessed by observation in the infant and enquiry in the older child. Serum morphine levels were monitored for the first 36 hours. The results of a cohort of 20 patients (aged 3 months-12 years) are presented. Thanks to positive parental and nursing staff support, and absence of complications, administration of postoperative an

Understanding respiratory rate as a predictor of clinical deterioration and mortality.

Respiratory rate (RR) is a fundamental physiological observation and a key component of early warning systems used to identify deterioration in hospitalised adults. Accurate RR measurement and interpretation, alongside other clinical observations and timely escalation of abnormal findings, are essential to ensure safe patient care. However, despite its clinical importance, RR remains one of the least reliably measured vital signs in practice and it is frequently estimated rather than counted accurately. This article explores the evidence on RR as a predictor of clinical deterioration and morta

Falls prevention in hospitalised older adults with cognitive impairment: strategies and ethical implications.

Falls are the most commonly reported patient safety incident in hospitals in the UK, and a major cause of patient harm. Older adults with cognitive impairment, such as dementia or delirium, are at particular risk of falls in acute settings due, for example, to attempting to mobilise without support, inability to communicate their needs or misjudging danger. Nurses have a central role in falls prevention, balancing patient safety with preserving individuals' dignity and autonomy to ensure that practice is consistent, compassionate and ethically sound. This article explores strategies for reduci

This retrospective single-center study describes the use and content of Advance Care Planning (ACP) forms in a sample of 50 anonymized medical records of patients enrolled in a home-based primary palliative care program. ACP was documented in 62% of cases, with frequent reporting of hospitalization preferences (90.3%) and appointment of a healthcare proxy (90.3%), whereas treatment decisions and palliative sedation were less frequently addressed (45.2% and 16.1%, respectively). ACP discussions always involved the patient and the general practitioner (100%), with frequent participation of famil

Stepping it Up on Temporary Mechanical Circulatory Support: Mobilization and Prehabilitation of Patients in ICUs.

Early mobilization and prehabilitation in patients supported with temporary mechanical circulatory support (tMCS) have emerged as promising strategies to mitigate the deconditioning and sarcopenia associated with critical illness and immobility. While small studies have demonstrated that ambulation with tMCS is both safe and feasible in selected patients, significant variability exists in practice patterns across centers. Implementation of early mobility protocols requires a multidisciplinary team, including cardiology, surgery, critical care, physical therapy, respiratory therapy, nursing, an

The Evolution of Transcatheter Aortic Valve Replacement: From Novel to Normal.

Over the past decade, transcatheter aortic valve replacement (TAVR) has become a transformative intervention for patients with aortic stenosis (AS). With a minimally invasive approach, TAVR reduces overall risks to the patient compared to the traditional surgical aortic valve replacement. Improvements in technology, training, and access to registered TAVR-designated sites, as well as the high success rate of the procedure, have turned TAVR from a novel approach into the preferred, first-line treatment for patients with AS. The purpose of this review article is to educate RNs and advanced pract
