Updates in Enhanced Recovery Pathways for Total Knee Arthroplasty.

Over 800,000 total knee arthroplasties (TKAs) are performed annually in the United States, with costs exceeding $18 billion. As the prevalence of knee osteoarthritis rises, enhanced recovery after surgery (ERAS) protocols have become vital to improve outcomes and reduce costs. ERAS in TKA focuses on optimizing pain control, minimizing hospital stays, and reducing complications. Key preoperative interventions include correcting anemia, improving nutritional status, encouraging smoking cessation, and cautiously evaluating carbohydrate loading. These measures have been shown to lower transfusion

Vitamin A fortification: key factors and considerations for effective implementation.

Vitamin A fortification plays a crucial role in achieving long-term economic development in developing countries by supporting the growth and development of human capital. While fortification programs involve a range of nutritional, agricultural, economic, and political considerations, there is a lack of a comprehensive overview of the topic in the literature. Our review highlights the importance of holistic approach in designing fortification programs: the effective program should consider (a) the agronomic, economic, and administrative capability of the target regions in regard of fortificat

The use of metronidazole in acute diarrhea in dogs: a narrative review.

Acute diarrhea is a common reason for non-wellness veterinary visits in dogs. Treatment for acute diarrhea usually consists of supportive care with nutritional intervention, fluid therapy, anthelmintics, and often an antibiotic - commonly metronidazole in North America. The empirical use of metronidazole for acute diarrhea in dogs has been a common practice in veterinary medicine for many decades; however, recent studies evaluating its use suggest it may be inappropriately utilized in many cases. Herein, we review the evidence evaluating the use of metronidazole and other antibiotics in acute

Diagnosis and management of coeliac disease in children.

Coeliac disease (CD) is an autoimmune gluten-dependent condition with a prevalence of 1% in the population, if screened. However, approximately only a third of children with CD are diagnosed. When CD is suspected, serological screening with anti-tissue transglutaminase titres should be performed. Children with a positive result should be referred to a specialist in CD for confirmation of the diagnosis. The European Society for Paediatric Gastroenterology Hepatology and Nutrition revised their diagnostic guidance for CD in 2020 and this article discusses the current diagnostic pathways. Lifelon

Perineuronal oligodendrocytes in health and disease: the journey so far.

Perineuronal oligodendrocytes (pn-Ols) are located in the cerebral gray matter in close proximity to neuronal perikarya and less frequently near dendrites and neurites. Although their morphology is indistinguishable from that of other oligodendrocytes, it is not known if pn-Ols have a similar or different cell signature from that of typical myelinating oligodendroglial cells. In this review, we discussed the potential roles of these cells in myelination under normal and pathophysiologic conditions as functional and nutritional supporters of neurons, as restrainers of neuronal firing, and as po…

Morbidity and long-term follow-up in CDH patients.

Overall survival of congenital diaphragmatic hernia (CDH) patients has increased since the widespread implementation of delayed operative repair with extracorporeal membrane oxygenation/gentle ventilation strategies starting in the mid-1990s. With the improvements in survival, CDH survivorship has been marked by increased morbidity with a greater need for monitoring and supportive interventions in infancy and early childhood. The multisystem morbidity and complexity of care for these patients mandate the dedicated resources and focus of medical specialties to ensure that the neurodevelopmental

[Progressive muscle atrophy. A rarely diagnosed variant of amyotrophic lateral sclerosis].

Progressive muscle atrophy (PMA) is a degenerative disease of the lower motor neuron. The course of the illness and the fatal prognosis correspond to those of amyotrophic lateral sclerosis (ALS). Neuropathologic and genetic findings support categorizing PMA within the spectrum of ALS, even though no clinical sign of a disorder of the upper motor neuron is demonstrable. The diagnosis of PMA is based on advanced extremity pareses and atrophies with a high progression rate. Respiratory insufficiency is determinative of the prognosis. Absent or late affection of bulbar functions is characteristic

Critical care nutrition: getting involved as a gastrointestinal endoscopist.

The route, timing, and volume of enteral feeding delivered to a patient in the intensive care unit have a profound effect on clinical outcome. At the height of critical illness, problems with ileus, aspiration, and the systemic inflammatory response syndrome make the provision of enteral nutrients a difficult and somewhat risky endeavor. The gastrointestinal endoscopist has the technical skills to place feeding tubes deep within the jejunum and an underlying expertise in gut physiology to monitor patients effectively once feeds are initiated. Attention to detail in the techniques for attaining

A therapeutic approach to wean total parenteral nutrition in the management of short bowel syndrome: three cases using nocturnal enteral rehydration.

Short bowel syndrome is characterized by severe dehydration and malnutrition and requires total parenteral nutrition (TPN). Prolonged TPN has serious complications. Caloric requirements can be met orally but oral fluid replacement is problematic. Noncompliance and an inability to discontinue TPN earlier increase the likelihood of complications. Discontinuation of parenteral support requires an assessment of gastrointestinal anatomy and absorption capacity. Fluids must be replaced independently of feedings because the osmotic gradients decrease fluid absorption. Nocturnal enteral rehydration is

Adult respiratory distress syndrome after extensive cytoreductive surgery. A case report.

BACKGROUND: Adult respiratory distress syndrome (ARDS) is characterized by progressive hypoxemia, diffuse bilateral pulmonary infiltrates and normal left ventricular function. CASE: A 58-year-old woman developed ARDS following extensive cytoreductive surgery for a widely disseminated intraperitoneal leiomyosarcoma. The patient had a favorable outcome after 10 days of ventilatory support, sedation, analgesia, parenteral and enteral nutrition, and fluid restriction. The ARDS might have been caused by multiple transfusions or the massive cytoreductive surgery and was probably mediated by cytokin

Nutritional factors associated with wound healing in the elderly.

A number of biological and sociological factors affect the ability of the elderly to ingest and utilize nutrients believed to be important in wound healing. Lack of economic resources, lack of the ability or motivation to prepare foods, difficulty digesting certain foods, and chronic illness are all examples of factors which affect the nutritional well-being of many of our elderly population. A deficiency of those nutrients which are involved in the healing process may delay wound healing. Research supports the importance of protein, vitamin C and zinc on wound healing. Many other nutrients su

Graft-versus-host disease following sibling allogeneic bone marrow transplantation--the Singapore experience.

Fourteen patients with haematological malignancies received allogeneic bone marrow transplantation from Major Histocompatility Complex compatible siblings. Five (35.7%) patients developed mild and 4 (28.6%) severe graft-versus-host disease (GVHD). There were correlations between the age of recipient (p less than 0.05), the degree of haematological support (p less than 0.1) and GVHD. The effects of the Mixed Lymphocyte Culture reactivity, donor/recipient sex match, cyclosporin A levels and the use of Total Parenteral Nutrition were not apparent. Two patients had relapses of their initial diseas

Primary eye care in rural sub-Saharan Africa.

Primary eye care in rural sub-Saharan Africa is reviewed. In the context of eye care delivered by village health workers (VHW's) living in and supported by the community, such a system of health care does not exist in Africa today. There are no VHW's, and primary health care is currently a matter of experimentation and conjecture only. However, most basic eye care is rendered by non-ophthalmologists; such care consists of screening, treatment of infections and inflammations, and in some cases, cataract surgery. Lower levels of non-ophthalmic general health workers are being tasked to promote m

Perinatal death due to abruptio placentae in an African city.

Abruptio placentae was a common cause of perinatal death in Addis Ababa, Ethiopia in 1974-1975 with a frequency of 5.5/1000 births. This disorder had its peak frequency at term. No abnormalities were found in the placentas to explain the placental abruptions but there were other clues to their genesis. There was a strong association of the fatal abruptions with severe poverty in the mothers. These poor mothers were both undernourished and malnourished during pregnancy. Their fetuses and neonates who died had multiple evidences of undernutrition including a relative undergrowth of adrenals, spl

Digital health technologies for the management of sarcopenia in patients receiving maintenance hemodialysis: a narrative review.

Digital health technologies are increasingly being explored as supportive tools for managing sarcopenia in patients undergoing maintenance hemodialysis (MHD). Wearable devices, mobile health applications, telemedicine platforms, artificial intelligence, and virtual or augmented reality may support monitoring, patient engagement, and intervention delivery. However, the evidence remains heterogeneous, and the clinical readiness of different digital modalities varies substantially. Activity monitors, mobile applications, and remote follow-up platforms are relatively feasible for clinical or home-
