Asthma during pregnancy: how obstetricians regard asthmatic patients' treatment.

BACKGROUND: Asthma is the most common pulmonary condition encountered during pregnancy. Based on our clinical experience as a pulmonologist and an internal medicine specialist, we frequently manage pregnant women with asthma, often without prior referral from an obstetrician. This study aimed to evaluate obstetricians' clinical approaches to asthma management during pregnancy using a structured questionnaire. METHODS: A 12-item questionnaire was developed and distributed electronically to obstetricians nationwide. Participation was voluntary, and respondents were assured of full anonymity. Th

EUS-Guided Gallbladder Drainage for Acute Cholecystitis in the Western World: Heterogeneity in Current Practice and the Relation With Patient Outcome.

BACKGROUND: Laparoscopic cholecystectomy is the standard treatment for acute cholecystitis (AC), but some patients are unfit for (immediate) surgery. Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) using a lumen-apposing metal stent (LAMS) is increasingly used in this population. OBJECTIVE: This multicenter cohort study aimed to evaluate practice patterns in EUS-GBD across Western centers and to identify factors that influence outcomes. DESIGN: We retrospectively analyzed data from patients undergoing EUS-GBD for AC across 18 centers. Patient-related, procedural and outcome data

Factors affecting general practitioners' referrals of patients to hip and knee arthroplasty: a focus group study from Northern Norway.

BACKGROUND: In Norway, patients have the right to choose their healthcare provider for elective specialized treatments, including the approximately 20,000 annual hip and knee arthroplasty surgeries. There is considerable regional variation in mobility - 'patient leakage' - out of a regional health authority for patients seeking these surgical procedures. General practitioners play a vital role in this decision-making process, acting as gatekeepers in referring patients to specialized healthcare. This study investigates the factors influencing Norwegian general practitioners' referral decision

Variations in the clinical practice of blood pressure and sedo-analgesia management for intracerebral hemorrhage: a case-based survey of Chinese healthcare workers.

BACKGROUND: Blood pressure (BP) management during the acute phase of intracerebral hemorrhage (ICH) is critical to improving patient prognosis. Besides routine antihypertensive medications, sedo-analgesia (i.e., a combination of sedation and analgesia) can enhance BP control during the acute phase. Nevertheless, the optimal approach remains elusive. The understanding of the evidence base and its application by healthcare practitioners significantly impact patient outcomes, yet studies on this topic are limited. Therefore, the aim of this study was to investigate the practice patterns and belie

Prescription Sequence Symmetry Analysis for Detection of Chronic Opioid Use Adverse Event Signals Using Administrative Claims Data.

INTRODUCTION: Opioids are commonly used to manage chronic pain in older adults, despite their well-documented adverse events (AEs) and the potential for additional less well-known risks. Post-marketing surveillance methods applied in real-world settings are essential to monitor AEs. Prescription Sequence Symmetry Analysis (PSSA) is a method that compares medication initiation patterns within individuals and can detect signals of prescribing cascades and drug AEs. This study applied PSSA to Medicare claims data to explore potential AEs following the initiation of long-term opioid therapy (LOT).
