The threefold value of surgery in previously irradiated brain metastases.

PURPOSE: Differentiating radiation necrosis (RN) from tumor recurrence (TR) in previously irradiated brain metastases (BM) remains a challenge. Imaging techniques lack sufficient diagnostic reliability, while therapeutic implications differ between both entities. This study aims to evaluate the diagnostic, therapeutic, and prognostic value of surgery in patients with progressive irradiated BM. METHODS: We studied patients who underwent surgical resection for progressive BM after prior irradiation. Histopathological diagnosis was classified as pure RN or TR. Clinical, radiological, surgical, a

Microvascular health status as a novel predictor of acute side effects after radiotherapy: a quantitative analysis across three cancer sites.

PURPOSE: To quantitatively assess how microvascular conditions estimated via a non-invasive method can predict acute side effects after radiotherapy in patients with breast, prostate, and head and neck cancers. MATERIALS AND METHODS: We studied 314 patients, assessing their MicroVascular Health Score (MVHS) before radiotherapy using a sublingual microscope that records videos of red blood cells in microvessels, analyses the microvasculature's functional and geometric properties, and calculates the MVHS. The MVHS was incorporated into a logistic model to predict the risk of developing acute ra

Clinical Considerations of Splenic Dose Constraints to Mitigate Radiation-Induced Lymphopenia.

BACKGROUND: The spleen dose-volume threshold for lymphopenia in abdominal radiotherapy has not yet reached a consensus. Our previous research indicated a correlation between these factors, but the threshold has not been determined. Therefore, we investigated the dynamic changes in lymphocytes during radiotherapy (RT), identified the spleen dose threshold, and examined how these factors affect patient prognosis. METHODS: The absolute lymphocyte counts (ALC) of gastric cancer patients were collected before, during, and after RT. Lymphocyte recovery status was assessed using the lymphocyte recov

Usability of a Reminder App that May Help to Reduce Radiotherapy-related Skin Toxicity in Patients With Breast Cancer.

BACKGROUND/AIM: Patients irradiated for breast cancer may experience acute skin toxicity. To reduce this risk, a mobile application was created which reminds patients to carry out skin care. Before using this app in patients, a pre-study evaluating its usability in healthy volunteers was required. PATIENTS AND METHODS: In this prospective study, 30 healthy volunteers from Germany and Denmark were asked to test the app and complete a questionnaire including ten statements ("yes or "no") in three sections. RESULTS: Satisfaction rates (statements affirmed with "yes") were 66.7% (18/27 participa

A cross-sectional protocol for experimental tongue high-density surface electromyography to detect and classify radiation-associated hypoglossal neuropathy.

BACKGROUND: Hypoglossal neuropathy is the most common lower cranial neuropathy detected as a delayed sequelae of Human Papillomavirus (HPV) -driven oropharyngeal cancer (OPC). Needle electromyography (EMG) is the gold standard for electrodiagnostic testing, but it is invasive and relies on subjective interpretation of the EMG signal. This study explores the potential of non-invasive high-density surface electromyography (HDSEMG) to detect and quantify hypoglossal neuropathy in OPC survivors. OBJECTIVE: In an exploratory study, examine the feasibility of HDSEMG for rapid, non-invasive screenin

[Chronological patterns of lacrimal drainage obstruction following radioactive iodine therapy for differentiated thyroid cancer].

UNLABELLED: Radioactive iodine (I-131) therapy for differentiated thyroid cancer is associated with a risk of delayed complications affecting radiosensitive organs. One such complication is secondary obstruction of the lacrimal drainage system caused by accumulation of I-131 in the wall of the nasolacrimal duct, leading to chronic inflammation and fibrosis. However, the precise chronological pattern of this pathology's onset remains undefined. PURPOSE: This study investigated the chronological patterns of lacrimal drainage obstruction following radioactive iodine therapy. MATERIAL AND METHOD

Impact of Nutritional Status on Severe Radiation-Induced Mucositis in Oropharyngeal Cancer Patients Undergoing Chemo-Radiotherapy.

Background/Objectives: Severe radiation-induced mucositis (RIM) is the most distressing acute side effect experienced by oropharyngeal squamous cell carcinoma (OPSCC) patients during chemo-radiotherapy (CRT), with a prevalence between 40 and 68%. RIM severity exhibits a multifactorial etiology that remains unclear. We aimed to analyze nutritional and morphofunctional predictive factors for severe RIM in OPSCC patients undergoing CRT. Methods: A prospective cohort study was conducted. Global Leadership Initiative on Malnutrition (GLIM) criteria, bioelectrical impedance vector analysis (BIVA), f

Chronic Oral Complications of Oral Cancer Therapy.

Head and neck cancer (HNC) patients are uniquely susceptible to adverse oral health outcomes following head and neck radiation therapy (RT). Various risk factors for these oral health outcomes post-RT were identified in the OraRad study, which followed HNC patients from pre-RT to 2 y post-RT. Chronic oral complications include hyposalivation, trismus, dental caries, periodontal disease, tooth loss, exposed bone, and osteoradionecrosis of the jaw. Results from OraRad provide a framework for oral health care providers both in preparing patients for RT and in managing sequelae of RT for best outc

Evaluation of a comprehensive set of normal tissue complication probability models for patients with head and neck cancer in an international cohort.

BACKGROUND/PURPOSE: Normal tissue complication probability (NTCP) models can be used to guide radiation therapy (RT) decisions by estimating side-effect risks pretreatment to minimize (late) side-effects. Recently, a comprehensive individual toxicity risk (CITOR) profile of NTCP models addressing common side-effects in head and neck cancer (HNC) patients was developed. This study investigates the generalizability of these models in an international setting, with different treatment approaches and side-effect assessments, promoting their integration into more widespread clinical practice. The late paid price: The lived experience of late radiation associated dysphagia.

PURPOSE: Late radiation-associated dysphagia is a rare side effect of radiation treatment for head and neck cancer. Arising over five years after treatment, it may profoundly impact survivors' health and quality of life. This study sought to gain insight into the lived experience of late radiation associated dysphagia. METHOD: This qualitative study utilised semi-structured phone interviews. Research assistants completed transcription using a consensus process with the first author for unclear speech segments. A patient partner was consulted at all study stages. Participants were purposively

Patient reported respiratory symptoms and lung radiation doses 11 years after loco-regional breast cancer radiation therapy in the DBCG RT Nation Study.

BACKGROUND AND PURPOSE: The Danish Patient Association of Late Effects has received numerous inquiries from breast cancer (BC) survivors suspecting that adjuvant radiotherapy (RT) was the reason for respiratory symptoms. This study investigated patient-reported respiratory symptoms after locoregional RT and their association with ipsilateral lung radiation dose. Patient/material and methods: Patient-reported outcomes (PROs) and RT plans from BC patients treated at a single institution over 2008-2016 were collected. PROs included dyspnoea (EORTC QLQ-C30), cough (PRO-CTCAE), smoking and comorbid

Retrospective evaluation of oral complications following radiotherapy and chemoradiation in patients with head and neck Cancer.

Patients receiving head and neck radiotherapy/chemoradiation may experience a notable and frequent decline in their oral health due to oral complications, including restricted mouth opening, oral mucositis, dry mouth, halitosis, infection, and oral pain. The main aim of this study was to describe the oral complications arising from post-Radiotherapy/chemoradiation among HNC patients through a retrospective survey of 80 patient records. This study collected patients' comorbid illness and treatment details alongside oral complications. The record review included one-year follow-up details of HNC

Nanomedicine-Based Strategies for Mitigating Chemo/Radiotherapy-Induced Cardiac Inflammation and Fibrosis.

Chemotherapy and thoracic radiotherapy are essential for cancer control but commonly induce cardiac inflammation and progressive fibrosis that undermine long‑term survivorship. Chemo/radiation‑induced oxidative stress and cardiomyocyte death promote immune cell infiltration, establishing a self‑sustaining inflammatory milieu that activates fibroblasts and drives inflammatory responses. The resulting myocardial stiffening and contractile dysfunction contribute to symptomatic heart failure, arrhythmias, and reduced quality of life. Conventional cardioprotective agents offer partial benefit yet a

Dose de-escalation of elective neck irradiation in head and neck cancer: A secondary analysis of acute toxicity findings from the randomized controlled UPGRADE-RT trial.

BACKGROUND: The multicenter, randomized UPGRADE-RT trial (NCT02442375) evaluated the safety and clinical benefit of elective neck radiotherapy with reduced versus standard dose in head and neck squamous cell carcinoma (SCC). This is a secondary analysis of acute toxicity. METHODS: Three hundred patients with cT2-4 N0-2 M0 SCC of the oropharynx, larynx or hypopharynx received definitive accelerated radiotherapy alone (34 fractions of 2 Gy in 5.5 weeks). Dose for elective neck irradiation was randomly assigned (ratio 2:1 to dose reduction, 43 Gy versus control, 50 Gy). Radiation induced acute t

Chronic inflammation in a radium dial painter cohort: elevated neutrophil to lymphocyte ratio and radiation-induced hearing loss.

The radium dial painters (RDPs) are a well-described group of predominantly young women who incidentally ingested226Ra and228Ra as they painted luminescent watch dials in the first part of the twentieth century. In 1974 pathologist Dr William D. Sharpe published complete clinical and autopsy results for 42 former RDPs evaluated in the New Jersey Radium Research Project. This was an important paper due to the completeness of the observations. Surprisingly, in this study, clinicians noted a 35.5% incidence of hearing loss, both conductive and mixed etiologies. Since the 1974 publication, there h

Potentially radiation-induced late toxicity after irradiation of pituitary adenoma, a retrospective dosimetric study.

BACKGROUND: Radiotherapy is considered as a second or third-line treatment for recurrent pituitary adenomas. As it is a benign tumor, the balance should clearly favor the benefit and induce the least amount of side effects. PURPOSE: This retrospective study aims to evaluate the incidence of side effects after treatment and their actual causality with radiotherapy through dosimetric analysis. METHODS: A cohort of 35 patients who underwent normofractionated radiation therapy following unsuccessful surgical or medical interventions was identified. 48.6% (17/35) had functioning adenomas (29.41%
