[Fractures of a single bone of the forearm in children].

Primary cilia are sensory membrane protrusions whose dysfunction causes ciliopathies. INPP5E is a ciliary phosphoinositide phosphatase mutated in ciliopathies like Joubert syndrome. INPP5E regulates numerous ciliary functions, but how it accumulates in cilia remains poorly understood. Herein, we show INPP5E ciliary targeting requires its folded catalytic domain and is controlled by four conserved ciliary localization signals (CLSs): LLxPIR motif (CLS1), W383 (CLS2), FDRxLYL motif (CLS3) and CaaX box (CLS4). We answer two long-standing questions in the field. First, partial CLS1-CLS4 redundancy

Long-term results of resection of the distal ulna for post-traumatic conditions.

Sixty-two patients underwent resection of the distal ulna because of pain or limited motion after wrist trauma. The median length of resection was 23 mm. Injury to the distal radioulnar joint occurred primarily during fractures of the distal radius. Followup time averaged 87 months. The primary surgical indication was pain, but some procedures were performed in an effort to increase motion. All patients improved after surgery. Three patients had residual pain, 25 had mild pain, and 34 had no pain. Supination was greatly improved, with only modest improvement in other wrist motions. Four patien

Impact of Methamphetamine Use on Injury Characteristics and Postoperative Outcomes in Upper Extremity Fractures.

BACKGROUND: Methamphetamine use in orthopedic trauma carries an indeterminate risk, with prior studies suggesting increased rates of deep vein thrombosis and elevated mortality. Upper extremity fractures (UEFs) have distinct mechanisms of injury and operative risk. This study evaluates outcomes following operative management of UEFs in patients with and without documented methamphetamine use. MATERIALS AND METHODS: A retrospective chart review was conducted of adults undergoing surgical fixation of UEFs (distal radius/ulna to clavicle; phalangeal, metacarpal, and carpal fractures were exclude

Whole-exome sequencing reveals a heterozygous LRP5 mutation in a 6-year-old boy with vertebral compression fractures and low trabecular bone density.

Juvenile osteoporosis (JO) is characterized by bone fragility during development, low bone mass and absence of extraskeletal features. Heterozygous loss-of-function mutations in LRP5 have been found in a few patients, but bone tissue and bone material abnormalities associated with such mutations have not been determined. Here we report on a 6-year-old boy who presented with a history of seven low-energy long-bone fractures starting at 19months of age and absence of extraskeletal involvement. Spine radiographs revealed multiple vertebral compression fractures. Despite tall stature (95th percent

Do peri-operative prophylactic antibiotics reduce pin-site infection after K-wire application in closed fractures? An international prospective cohort study.

BACKGROUND AND PURPOSE: The role of peri-operative prophylactic antibiotics in preventing pin-site infection after percutaneous K-wire fixation of closed fractures is unclear, and guidance does not address this procedure. This study evaluated whether prophylaxis reduces infection risk in this setting. METHODS: International, multicentre, prospective cohort study across 30 hospitals in three countries (United Kingdom, Spain, Greece) from December 2023 to October 2024, including patients with distal radius or supracondylar elbow fractures managed by percutaneous K-wire fixation. Antibiotic admi

Impact of Repeated Use on Cyclic and Torsional Fatigue of 3 Rotary Files: Implications for Clinical Safety.

INTRODUCTION: This study aimed to compare the cyclic and torsional fatigue of S2, Logic, and Flat File NiTi files, new and after repeated use by an undergraduate student. METHODS: Ninety (n = 90) artificial mandibular molars were prepared using S2, Logic, and Flat File systems. For each system, 10 new instruments were tested mechanically, and the other 10 were tested after preparing three prototyped molars with curved mesial roots (60°). Cyclic fatigue tests were conducted in a custom device simulating a curved canal (60° curvature angle, 5 mm radius), while torsional strength tests followed

Upper limb non-unions treated with BMP-7: efficacy and clinical results.

The management of upper limb non-unions can be challenging and often with unpredictable outcomes. In this study we present our experience with the use of BMP-7 in the treatment of upper limb non-unions. Between 2004 and 2011 all consecutive patients who were treated with BMP-7 were followed up prospectively until fracture union. Fracture union was assessed with regular radiological and clinical assessment. At the final follow up clinical assessment included the short Disabilities of the Arm, Shoulder and Hand (DASH) score. The minimum follow up was 12 months (12-36). In total 42 patients met t

Upper extremity interaction with a deploying side airbag: a characterization of elbow joint loading.

Computer simulations, dummy experiments with a new enhanced upper extremity and small female cadaver experiments were used to analyze the small female upper extremity response under side airbag loading. After establishing a worst case initial position, three tests were performed with the fifth percentile female hybrid III anthropometric test dummy and six experiments with small female cadaver subjects. A new fifth percentile female enhanced upper extremity was developed for the dummy experiments that included a two-axis wrist load cell in addition to the existing six-axis load cells in both th

[Intramedullary nailing of fractures of forearm in adults.].

The authors present a three-year retrospective group of 32 adult patients with the diaphyseal fractures of the forearm treated surgically. Only in 3 cases a standard plate technique was used. All other surgeries were performed by intramedullary nailing using Prevot nails, strong Kirschner wires, True/Flex flexible nails and a bilaterally locked Foresight nail. Long-term results were in detail evaluated in a group of 15 patients with the implanted True/Flex nail. Minimum period of the follow-up was 6 months after operation, the average was 14,7 months. The nailing of both bones of the forearm w

[Traumatic TFCC lesions in childhood and adolescence-A previously neglected injury?].

Although injuries to the triangular fibrocartilage complex (TFCC) are less common in children and adolescents than in adults, can lead to chronic pain and instability in the distal radioulnar joint (DRUJ), ultimately predisposing individuals to long-term osteoarthritis. A dislocated distal radius fracture with avulsion of the styloid process of the ulna following high-energy trauma is a risk factor for a TFCC injury. The diagnostics include a thorough clinical examination and imaging procedures, such as X‑ray and magnetic resonance imaging (MRI). If there is no instability of the DRUJ, conserv

HR-pQCT Measures of Bone Microarchitecture Predict Fracture: Systematic Review and Meta-Analysis.

High-resolution peripheral quantitative computed tomography (HR-pQCT) is a noninvasive imaging modality for assessing volumetric bone mineral density (vBMD) and microarchitecture of cancellous and cortical bone. The objective was to (1) assess fracture-associated differences in HR-pQCT bone parameters; and (2) to determine if HR-pQCT is sufficiently precise to reliably detect these differences in individuals. We systematically identified 40 studies that used HR-pQCT (39/40 used XtremeCT scanners) to assess 1291 to 3253 and 3389 to 10,687 individuals with and without fractures, respectively, ra

Physeal fracture in the wrist and hand due to stress injury in a child climber: A case report.

RATIONALE: In competitive athletes, the upper extremity is subject to tremendous torsional forces with axial loading due to repetitive weight bearing. Approximately 25% of injuries in sports are related to the hand or wrist. Skeletal deformity on the wrist physis is common in athletes due to repetitive loading and presents at early ages between 6 and 13 years. Additionally, it is more common in female than in male athletes. PATIENT CONCERNS: An 11-year-old girl who was a climber complained of pain on her left wrist without direct trauma. She had participated in climbing exercise for several y

[Isolated irreducible anterior radial head dislocation in a child: a rare case report].

Anterior radial head dislocation in pediatric population is related to Monteggia fracture-dislocations. Isolated radial head dislocation is uncommon. Sometimes, radial head dislocation becomes irreducible. This entity can develop into chronic conditions such as nerve injuries, early osteoarthritis, limited range of motion and cubitus valgus. We describe a case of a 9-year-old patient who suffered elbow trauma. He was admitted to our institution 40 days after, where radial head dislocation was diagnosed. This condition was misdiagnosed at first stage. It was irreducible due to a tear in the ann

Unique model evokes the supination/pronation deficits found after Mason II fractures.

A rapid prototyping model of Mason II fracture was used to investigate baseline recommendations for surgical intervention founded on kinematic forearm rotational blockage. Exact replicas of the radial heads in nine cadaveric specimens were produced and specimens were tested in a physiologic elbow simulator. After testing supination/pronation, the rotations were repeated with native replicas and with replicas modeling 3 mm depressed Mason II fractures with and without a gap of 1 mm between the body and fragment. The fragments were located circumferentially around the radial head at 10, 2 and 6
