Headache in Intracranial and Cervical Artery Dissections.

Dissection refers to a tear in the wall of an artery, with the two main types being intracranial or extracranial. Dissections tend to occur most commonly in the young, sometimes secondary to trauma involving the neck. To confirm a dissection, some type of vessel imaging is necessary, including magnetic resonance angiography (MRA), computed tomography angiography (CTA), or angiography. The most common presentation of a dissection (especially extracranial) is pain, usually head and neck pain along with a Horner's syndrome. Patients may also present with ischemic symptoms, including transient isc

Multimodal imaging in the diagnosis and evaluation of intestinal malrotations in adults: a case report.

Midgut malrotation is a congenital anomaly referring to either lack of or incomplete rotation of the fetal intestines around the axis of the superior mesenteric artery during fetal development. It is rare in adulthood and the true incidence is difficult to estimate because most patients are asymptomatic. The diagnosis is usually performed with several radiological and surgical methods. We report a case of a woman who presented with cramp-like abdominal pain localized to the right iliac fossa. The patient underwent abdominal ultrasound, radiological examination without and with contrast, and co

Neuropathic itch of the back: a case of notalgia paresthetica.

Notalgia paresthetica refers to an isolated mononeuropathy involving chronic localized itch or paresthesia most often at the skin of the scapula or surrounding regions. There are no specific skin manifestations except those arising from chronic scratching and rubbing. The specific etiology remains unknown; however, it has been theorized that the neuropathic itch is caused by sensory nerve entrapment involving the posterior rami of the T2 to T6 nerve root. The entrapment is due to degenerative changes in the vertebrae. We report here a particular case of notalgia paresthetica in a 55-year-old w

Scheuermann's disease: an update.

Scheuermann's disease is a juvenile osteochondrosis of the spine. It is a disease of the growth cartilage endplate, probably due to repetitive strain on the growth cartilage weakened by a genetic background. The radiographic aspects are related to the vertebral endplate lesions and include vertebral wedging, irregularity of the vertebral endplate, and Schmorl's node (intraossous disk herniation). Disc alterations are frequent and may be secondary to dysfunction of the disc-vertebra complex. The definitions of Scheuermann's disease are varied; it can refer to the classical form of juvenile kyph

Clinical evaluation of the efficacy of Khadiradi yoga avachoornana in Kachchu with special reference to genitoinguinal intertrigo.

Kachchu is a simple localized dermatological infection of the genitoinguinal region, which occurs because of unhygienic observance, and manifests in the form of inflammation followed by secondary bacterial or fungal infections that can be best correlated to genitoinguinal intertrigo. Even though it rarely causes systemic manifestations, its mere presence itself is disturbing because of intractable itching and pain. It may delay the proper healing of the episiotomy wound and may cause difficulty in walking because of severe pain. Candida powder is the highest selling medicine for the intertrigo

A case report of a septic hip secondary to a psoas abscess.

Psoas abscess was first described by Mynter in 1881. Though rare, its prevalence is increasing with advances in radiology and an increasing ability to accurately diagnose the condition. The symptoms of a psoas abscess can be insidious and nonspecific, and patients often present with a limp, fever, weight loss, and flank or abdominal pain.A psoas abscess can be classified as either primary or secondary depending on the presence or absence of an underlying disease. Primary psoas abscess has become more prevalent in the developed world, especially in immuno-compromised patients. We present the cas

[The insufficiency fracture of the sacrum. An often unrecognized cause of low back pain: results of bone scanning in a major hospital].

AIM: The insufficiency fracture of the sacrum is often radiographically occult. Bone scintigraphy is a method of reference for the diagnosis; the results have been analysed retrospectively. METHODS: Bone scintigraphy was done on patients who, predominantly after minor trauma, suffered from lower back pain and whose radiographic findings were negative. RESULTS: During a 24 month period, a sacral insufficiency fracture was diagnosed in 102 patients. H-shaped hyperfixation of the tracer--described as typical--was present in only 19.6% of the cases. Tracer accumulation was found as follows: loca

Elective Laparoscopic Resection Following Transarterial Embolization and Anticoagulation for a Giant Hepatocellular Adenoma with Hemorrhage and Venous Thrombosis: A Case Report.

INTRODUCTION: Hepatocellular adenoma (HCA) is a rare benign liver tumor that can be complicated by hemorrhage, whereas concomitant major venous thrombosis is uncommon. CASE PRESENTATION: A 25-year-old woman presented with epigastric pain and nausea. Contrast-enhanced CT showed a 17-cm liver tumor involving the anterior and medial segments, which suggested HCA with intratumoral hemorrhage. She initially underwent transarterial embolization (TAE) for hemostasis at a previous hospital. Subsequent CT showed thrombi in the inferior vena cava and pulmonary artery, and she was referred to our instit

Lingual osseous choristoma of the posterior tongue: A case report.

Lingual osseous choristoma is a rare benign lesion characterized by ectopic mature bone within the soft tissues of the tongue, with just over 100 cases reported in the literature. Its clinical presentation is often nonspecific, making diagnosis challenging. A 45-year-old woman was referred for evaluation of a slowly enlarging mass on the posterior dorsal surface of the tongue, initially noted during a routine dental examination. The lesion was asymptomatic, with no associated pain, dysphagia, or bleeding. Clinical examination revealed a firm, whitish, non-ulcerated mass suggestive of a fibroma

When beta-blocker unmasks the diagnosis: pheochromocytoma mimicking acute coronary syndrome. A case report.

Pheochromocytoma is a catecholamine-producing neuroendocrine neoplasm classically associated with secondary hypertension. We describe the case of a 63-year-old male who presented with intermittent precordial discomfort, paroxysmal hypertensive episodes, and a mild rise in cardiac troponin levels. He was referred to our institution with a presumptive diagnosis of acute coronary syndrome. Serial admission electrocardiograms showed no dynamic ischemic changes, and transthoracic echocardiography demonstrated preserved global systolic function without regional wall motion abnormalities. Hemorrhagic Rupture of a Hepatic Adenoma in a Postmenopausal Female: A Case Report.

Hepatic adenoma (HA) is a rare benign liver lesion most commonly observed in females of reproductive age using oral contraceptive pills (OCPs). Though typically asymptomatic, HA may lead to complications such as hemorrhage or malignant transformation. Current literature highlights an increased risk of hemorrhage in larger lesions among premenopausal females and transformation to hepatocellular carcinoma in postmenopausal females. This report discusses a case of a 58-year-old postmenopausal female who presented with a sudden onset of back pain, syncope, nausea, and vomiting. CT imaging revealed

A Fresh Look at Post-ACDF Dysphagia and SLP Role in Acute Management.

Anterior cervical discectomy and fusion (ACDF) is a commonly performed procedure for the treatment of cervical spine pathologies, such as disc disorders and myelopathy. Patients with cervical spine disorders suffer neck pain, numbness, and in severe cases, weakness of the extremities. When patients fail conservative management, surgical treatment can be beneficial. An anterior approach to the cervical spine is preferential due to relatively low complication rates; however, dysphagia is a common and troublesome complication of this approach. The pathophysiology of dysphagia is multifactorial an

Case report of a 33-year-old patient with a niche in a cesarean section scar.

Worldwide, cesarean section procedures are being performed with increasing frequency, which is associated with a growing number of postoperative complications. One of these is the formation of a triangular, anechoic defect of the uterine myometrium at the site of a previous cesarean section scar, referred to as a niche. This condition may lead to abnormal bleeding, pain, fertility problems, and complications in subsequent pregnancies. This paper presents the case of a 33-year-old patient with a history of two cesarean sections, in whom ultrasonographic examination revealed a defect within the

[A Case of Intussusception Due to Small Intestinal Cancer Diagnosed by Laparoscopy].

Most cases of primary small intestinal cancer are diagnosed at an advanced stage, and when accompanied by intussusception or obstruction, emergency surgery may be required. Preoperative evaluation of small intestinal cancer is challenging. The patient was a 52-year-old man who was referred to our hospital with abdominal pain and vomiting, and was diagnosed with small intestinal intussusception. Abdominal CT at our hospital revealed bowel wall thickening at the site where the intussusception had resolved, raising suspicion for small intestinal cancer. Based on the patient's clinical history and

Successful Endoscopic Vacuum Therapy for a Postoperative Esophagogastric Fistula Following Sleeve Gastrectomy: A Case Report.

Esophagogastric fistulas are a rare but serious complication after sleeve gastrectomy. Their management remains a clinical challenge, especially when conventional endoscopic approaches, such as stent placement, fail. We report the case of a 49-year-old female with a history of hypothyroidism, major depressive disorder, body mass index greater than 50 kg/m², significant smoking history, and obstructive sleep apnea, who developed acute abdominal pain and bleeding after laparoscopic sleeve gastrectomy. She was referred to our institution in hypovolemic shock and underwent exploratory laparotomy w

Acute Pancreatitis in Second Trimester of Pregnancy.

Acute pancreatitis is an uncommon complication in pregnancy. Here we present a case of 24-year-old gravda 2 para 1 woman with an acute abdominal pain in her 24-week period of gestation. Her laboratory reports suggested the diagnosis of acute pancreatitis. However, the root cause was unidentifiable through various investigations. She was accordingly managed. Acute pancreatitis during pregnancy is rare with incidence ranging from 1 in 1,000 to 10,000. Gall stones remains the main causative factor. The exact etiology in some cases cannot be identified and hence they are referred as idiopathic pan

Incidental Grade 2 Appendiceal Neuroendocrine Tumor Presenting as Acute Appendicitis in a Young Adult Male Patient.

Neuroendocrine tumors (NETs) are epithelial neoplasms with neuroendocrine differentiation that can arise throughout the gastrointestinal tract, including the appendix. Appendiceal NETs are rare neoplasms that frequently present as or mimic acute appendicitis. We report the case of a 28-year-old male patient who presented with right lower quadrant abdominal pain and was found to have early acute appendicitis on imaging. He underwent laparoscopic appendectomy, and final histopathology revealed a 0.5 cm well-differentiated Grade 2 NET at the tip of the appendix. The tumor was confined to the subm
