Bilevel positive airway pressure worsens central apneas during sleep.

STUDY OBJECTIVES: While most patients with sleep-disordered breathing are treated with continuous positive airway pressure (CPAP), bilevel p … hypopneas, obstructive apneas, and central events, including periodic breathing (PB), Cheyne-Stokes respiration (CSR), and non-CSR central apneas were evaluated. PATIENTS: Ninety-five of the 719 patients who underwent sleep studies met inclusion criteria. Eighty of the 95 patients treated with BLPAP were also treated with CPAP. RESULTS: BLPAP was more likel …

Successful pregnancy in a patient with spinal muscular atrophy and severe kyphoscoliosis.

Pregnancy imposes a load on the respiratory system that is usually easily assumed because of alterations in the thoracoabdominal architecture. It is presumed that the respiratory mechanical disadvantage of severe kyphoscoliosis and the muscle weakness of spinal muscular atrophy impede these adaptations sufficiently to preclude a successful gestation. We report the case of a successful pregnancy in a woman with spinal muscular atrophy, severe uncorrected scoliosis, and the lowest spirometric values reported in the literature without the use of ventilatory support. This patient demonstrates that

Diaphragm kinetics during pneumatic belt respiratory assistance: a sonographic study in Duchenne muscular dystrophy.

The principal aim of this study was to demonstrate the usefulness of M-mode sonography as a noninvasive technique to evaluate diaphragm excursion. The secondary aim was to assess the efficacy of pneumatic abdomino-diaphragmatic belt ventilation in patients with Duchenne muscular dystrophy. Using M-mode sonography, we measured the amplitude of diaphragm excursion in seven patients with Duchenne muscular dystrophy in various positions (0 degrees, 45 degrees, 75 degrees ) with and without pneumatic abdomino-diaphragmatic belt respiratory assistance. The belt significantly increased mean amplitude

Effect of hypothermia on ventilation in anesthetized, spontaneously breathing rats: theoretical implications for mechanical ventilation.

OBJECTIVE: To test if hypothermia, induced by a sustained pentobarbital anesthesia, in rats can reduce ventilatory demands without compromising pulmonary gas-exchange efficiency. DESIGN: Prospective study. SETTING: Research laboratory in a hospital. SUBJECTS: One group of 11 female Sprague Dawley rats. INTERVENTIONS: The rats were anesthetized with 45 mg/kg pentobarbital, tracheostomized and intubated; their femoral veins and arteries were cannulated. After surgery, anesthesia and fluid balance were maintained (10 mg/kg per h pentobarbital, and 5 ml/kg per h saline, i.v.). Rectal temperatu

The cost of ventilator-dependent spinal cord injuries-patients in the hospital and at home.

The number of ventilator-dependent SCI-patients is increasing steadily but also are the costs of adequate treatment in and outside hospital. Various technical aids for ventilation, mobility, nursing, and individual needs are available for a qualified life in the family, but the costs of these add up to $116.450. As phrenic nerve stimulators are very expensive, the cost for permanent ventilation cannot be reduced although diaphragmatic pacemakers have a real significant benefit for the patient's independence and quality of life. As the hospital charge is equal for all paraplegic, tetraplegic an

Pulmonary hypertension in the obstructive sleep apnoea syndrome: prevalence, causes and therapeutic consequences.

"Cor pulmonale" is a classic feature of the "Pickwickian syndrome". Earlier studies have reported a high prevalence of pulmonary hypertension (PH) in obstructive sleep apnoea (OSA) patients, but this has not been confirmed by recent studies with a more adequate methodology, including larger groups of patients. The first part of this review is devoted to the prevalence of PH in OSA; most recent studies agree on prevalence of 15-20%. The second (and major) part of the study deals with the causes and mechanisms of PH in OSA. Pulmonary hypertension is rarely observed in the absence of day-time hyp

The biphasic ventilatory response to hypoxia in preterm infants is not due to a decrease in metabolism.

The mechanism underlying the biphasic ventilatory response to hypoxia in neonates is poorly understood. Because alveolar PCO2 (PaCO2) decreases and remains low during hypoxia, it has been argued that a decrease in metabolism may occur. We hypothesized that if the late decrease in ventilation during hypoxia is due to a decrease in CO2 production, an increase in PACO2 should abolish it. We studied 27 preterm infants [birth weight, 1,700 +/- 41 g (mean +/- SEM); study weight, 1,760 +/- 36 g; gestational age 32 +/- 0.2 weeks; postnatal age, 17 +/- 1 days]. Accuracy of breath-by-breath analysis of flow-volume loop in identifying sleep-induced flow-limited breathing cycles in sleep apnoea-hypopnoea syndrome.

1. Inspiratory flow limitation is involved in the pathophysiology of sleep-related breathing disorders. Since the definition of flow-limited cycle is based on a dissociation between flow and respiratory efforts, identification of inspiratory flow limitation requires upper airway or intrathoracic pressure measurements. We examined the accuracy of the analysis of the flow-volume loop of a tidal breath in identifying inspiratory flow limitation during sleep in ten patients with a sleep apnoea-hypopnoea syndrome. 2. Measurements were taken during continuous positive airway pressure trials. After d

Breathing frequency and pattern are poor predictors of work of breathing in patients receiving pressure support ventilation.

OBJECTIVE: To evaluate the relationships between directly measured work of breathing (WOB) and variables of the breathing pattern commonly used at the bedside to infer WOB for intubated, spontaneously breathing patients treated with pressure support ventilation (PSV). DESIGN: In vivo measurements of the WOB were obtained on a consecutive series of adults. Breathing frequency (f), tidal volume (VT), the index of rapid, shallow breathing (f/V T), the duration of respiratory muscle contraction expressed as the ratio of inspiratory time over total respiratory cycle time (TI/TTOT), and a breathing

Modelling of passive expiration in patients with adult respiratory distress syndrome.

The time-course of volume change during passive expiration preceded by an end-inspiratory hold was studied with a biexponential model in six adult respiratory distress syndrome (ARDS) patients. We measured the initial volumes and time constants of the fast (tau 1), and the slow (tau 2) compartments of expiration, as well as the static elastance of the respiratory system. The results were compared to those of 11 normal subjects. We observed that: 1) the biexponential model fitted closely the volume decay; 2) the fast compartment was responsible for 81 +/- 7% (ARDS) versus 84 +/- 10% (controls)

Myocardial perfusion as assessed by thallium-201 scintigraphy during the discontinuation of mechanical ventilation in ventilator-dependent patients.

Patients who cannot be separated from mechanical ventilation (MV) after an episode of acute respiratory failure often have coexisting coronary artery disease. The authors hypothesized that increased left ventricular (LV) wall stress during periods of spontaneous ventilation (SV) could alter myocardial perfusion in these patients. Using thallium-201 (201TI) myocardial scintigraphy, the authors studied the occurrence of myocardial perfusion abnormalities during periods of SV in 15 MV-dependent patients (nine women, six men; aged 71 +/- 7 yr, mean +/- SD). Fourteen of these patients were studied

End-tidal carbon dioxide pressure in neonates and infants measured by aspiration and flow-through capnography.

In 25 anesthetized, intubated, artificially ventilated, and paralyzed healthy neonates and infants, end-tidal PCO2 (PETCO2) measured by remote multiplexed mass spectrometry was 1.86 +/- 1.58 mm Hg lower than arterial PCO2 (PaCO2). PETCO2 measured by a flow-through cuvette was 1.02 +/- 1.64 mm Hg lower than PaCO2. The difference between the two methods of capnography was not significant. Values for PETCO2 obtained by mass spectrometry changed -0.43 +/- 1.43 mm Hg from baseline after 15 minutes of aspiration at a sample flow rate of 240 ml/min. Values for PETCO2 obtained with flow-through capnog

Reevaluation of hemodynamic consequences of positive pressure ventilation: emphasis on cyclic right ventricular afterloading by mechanical lung inflation.

To examine the cyclic changes in right ventricular (RV) function induced by controlled ventilation, right heart catheterization and two-dimensional echocardiography were combined in a group of 20 patients requiring respiratory support for an episode of acute respiratory failure. Simultaneous measurements of RV pressure (using a modified pulmonary artery catheter), RV stroke output (thermodilution), and RV dimensions (two-dimensional echocardiography), permitted a beat to beat evaluation of RV function throughout the mechanical respiratory cycle. When compared with expiration, lung inflation pr

The effect of hypopnea on low-pressure pulmonary edema.

The effects of the mechanical factors involved in ventilation on pulmonary edema are only partially understood. To clarify the effect of ventilation on the adult respiratory distress syndrome (ARDS), we examined the effect of reducing rate and tidal volume on oleic acid-induced low-pressure pulmonary edema in dogs, hypothesizing that hypopnea would reduce lung edema. We placed the experimental animals on venous-venous extracorporeal membrane oxygenation (ECMO) for CO2 clearance and oxygenation 1 h after the injury. This allowed reduction of the ventilatory rate from 17.2 +/- 4.8 to 3.3 +/- 0.8

Diaphragmatic flutter resulting in failure to wean from mechanical ventilator support after coronary artery bypass surgery.

Diaphragmatic dysfunction, most commonly elevation of the left hemidiaphragm and/or phrenic nerve paralysis, are well-known complications of coronary artery bypass grafting (CABG). Diaphragmatic flutter (DF) is an easily overlooked breathing pattern characterized by rapid (greater than 40 times/min) involuntary contractions of the diaphragm, at times superimposed on a more normal breathing pattern (dirhythmic breathing). Using respiratory inductive plethysmography, we were able to record this unusual ventilatory pattern in four patients after CABG. All procedures were performed via median ster

Respiratory compliance and resistance in mechanically ventilated patients with acute respiratory failure.

The purpose of this study was noninvasive assessment of respiratory compliance and resistance in mechanically ventilated patients with acute respiratory failure (ARF). To this end, flow, change in lung volume, and airway pressure were measured at the proximal tip of the endotracheal tubes in twenty nine critically ill unselected patients. Eleven had acute exacerbation of chronic obstructive pulmonary disease (COPD), 8 had adult respiratory distress syndrome (ARDS) and 10 had ARF of various etiologies. Static compliance (Cst,rs), 'intrinsic' PEEP (PEEPi), as well as minimum and maximum resistan
